Abstract: Purpose The AT LARA 829MP is a next-generation extended depth of focus (EDOF) intraocular lens (IOL) providing continuous vision over a range of distances. The aim of this prospective multi-centre randomised trial was to compare two EDOF IOLs and one monofocal IOL. Methods Cataract patients between 50 and 80 years were randomised for bilateral implantation with either the AT LARA 829MP (EDOF), the TECNIS Symfony (EDOF) or the CT ASPHINA 409MP (monofocal). Follow-up was at 1 to 2 weeks, 1 month and 4 to 6 months. Results A total of 211 patients were randomised and included in the final analysis. Monocular depth of focus was significantly better for AT LARA 829MP eyes compared with that for TECNIS Symfony at all thresholds (p = 0.024, 0.001 and 0.006, for 0.1, 0.2 and 0.3 logMAR respectively) with no significant difference for binocular depth of focus. LARA eyes had significantly better monocular depth of focus at all levels compared with ASPHINA eyes (all p 0.0001), while there was no significant difference between Symfony and ASPHINA eyes at 0.1 logMAR and 0.2 logMAR. Both EDOF IOLs were significantly better than the monofocal ASPHINA at all levels for binocular depth of focus (LARA: all p 0.0001; Symfony: all p = 0.002). Distance visual acuity was similar for all IOLs at 6 months; intermediate and near visual acuity were significantly better for the EDOF IOLs than for the monofocal (p 0.0001). Refraction improved in all groups relative to baseline. Contrast sensitivity was higher with the CT ASPHINA 409MP but both EDOF lenses had a better spectacle independence rate. At 6 months, all IOLs were well centred with no cases of tilt. No general safety issues were raised for any of the groups.

This comprehensive 2nd edition will build on the highly successful first edition, providing an updated global perspective of the fundamentals of multifocal intraocular lenses. The varying outcomes, limitations, and the neuroadaptation process necessary for an adequate clinical success are thoroughly discussed, along with an overview of the different types of multifocal lenses, including the recently developed extended depth of focus lenses. Multifocal Intraocular Lenses: The Art and the Practice, 2nd edition opens with an introduction that will delve into current technological offerings for the correction of pseudophakic presbyopia, as well as the opportunity for refractive lens exchange in advanced presbyopic ages and the opportunity to use these lenses. The first section will include the historical background and clinical indications, while section two addresses the varying types and models of lenses currently available, including important clinical and technological highlights. Section three and four will follow, and provide an extended look at the Zeiss and Alcon Family Multifocal IOL’s. Section five will delve into extended depth of field lenses, and will contain an introduction about the concept, different models and the evidence available about their outcomes. Section six concludes the book, closely examining accommodative intraocular lenses, and a full update will be provided on these lenses, the failures of the past and the hopes for the future. Multifocal Intraocular Lenses: The Art and the Practice, 2nd edition is a thorough, resource for the practical ophthalmologist and ophthalmic surgeon interested in learning more about intraocular lenses, identifying the best technologies and lenses for the benefit of their patients.

Good vision is more than 20/20 on a Snellen visual acuity chart. The modern ophthalmologist understands that contrast sensitivity, near and distance vision, performance under light and dark conditions, and the brain's interpretation of input from the sensory apparatus, are all important elements in patients' quality of vision. In Quality of Vision: Essential Optics for the Ophthalmic Surgeon, ophthalmologist and optics expert Dr. Jack T. Holladay explores the elements of vision that lie beyond Snellen testing. He explains the Quality of Vision addresses: - Measuring and treating astigmatism in corneal and IOL surgery - The importance of correcting spherical aberration in cataract and refractive surgery - IOL calculations after refractive surgery - Promising approaches to the correction of presbyopia - Other practical topics the clinician can use every day Recognized as the leader in ophthalmic optics, Dr. Holladay incorporates practical aspects of daily practice into each chapter as he expertly explains the scientific principles, mathematical formulas, and theories behind these important issues. Additionally, Quality of Vision includes numerous color illustrations to provide visual representations of the text's key points. Dr. Holladay discusses what's on everyone's mind: - How to take the best measurements and perform the best calculations to ensure good outcomes in cataract and refractive surgery - How neural adaptation can improve outcomes - How today's excimer laser systems deliver excellent optical correction, and why tomorrow's systems will perform even better Quality of Vision: Essential Optics for the Ophthalmic Surgeon is the most complete and practical reference for ophthalmologists looking to increase their understanding of optical physics and their ability to deliver good vision to their patients after cataract and refractive surgery.

The first clinical application of the phacoemulsification cataract surgical technique (often referred to as “phaco”) was introduced in 1967 by Dr. Charles Kelman. This innovation is a big step forward for cataract surgery. With the development of intraocular lens (IOL) design, more and more premium presbyopia- and astigmatism-correction IOLs are being used in clinics. This progress has greatly improved the visual quality of cataract patients. This book discusses the basic surgical skills required to perform this procedure, premium IOL surgical design, specific surgical plans for clinically challenging cases, and more. It provides readers with a comprehensive knowledge of the current state of the art of cataract surgery and surgical design.
