In recent years, guidelines and protocols have gained support as the vehicles for promoting best practices in clinical medicine. They offer the possibilities of reducing unwarranted practice variations, of containing cost while maintaining quality of care, and of defining standards of care for quality assurance purposes. These promises have led to an explosion of guideline publications. Yet studies have shown that dissemination and effective use of guidelines in clinical care remains a major bottleneck. A number of researchers have developed different technologies for delivering computerized guidelines in clinical care. These technologies range from alerts and reminders to knowledge-based systems, information-retrieval systems, and others. The tasks to which guidelines have been applied include classic clinical decision support, workflow management, quality assurance, and resource-requirement estimates. The research has spanned several communities (information retrieval, artificial intelligence, medical informatics, software engineering, clinical medicine), but unfortunately, there has been little cross-fertilization between the communities working in this area. This publication brings together researchers from different communities to examine cutting-edge approaches to guideline modeling and application development and to consider how different communities can leverage each other's strengths.

th 2002 DEXA, the 13 International Conference on Database and Expert Systems Applications was held on September 2–6, 2002, at the Université Aix–Marseille II, France. The quickly growing field of information systems required the establishment of more specialized discussion platforms (the DaWaK conference, EC-Web conference, eGOV conference and DEXA workshops), and there were held in parallel with DEXA, also in Aix-en-Provence. The resulting book was prepared with great effort. Starting with the preparation of submitted papers, the papers went through the reviewing process. The accepted papers were revised to final versions by their authors and arranged to the conference program. This year 241 papers were submitted and our thanks go to all who have contributed. The program committee and the supporting reviewers produced altogether about 730 referee reports, on average three reports per paper, and selected 89 papers for presentation. The papers presented here encompass the extensive domain of databases; together with the other conferences and workshops of the DEXA event cluster a vast part of applied computer science was covered. In this way DEXA has blazed the trail. At this point we would like to acknowledge to all institutions which actively supported this conference and made it possible. The central themes of the 14th International Conference on Knowledge Engineering and Knowledge Management (EKAW 2004) were ontological engineering and the Semantic Web. These provide the key foundational and delivery mechanisms for building open, Web-based knowledge services. However, consistent with the tradition of EKAW conferences, EKAW 2004 was concerned with all aspects of eliciting, acquiring, modelling and managing knowledge, and its role in the construction of knowledge-intensive systems. Indeed a key aspect of the Knowledge Acquisition Workshops (KAWs) held in the US, Europe and Asia over the past 20 years has been the emphasis on ‘holistic’ knowledge engineering, addressing problem solving, usability, socio-technological factors and knowledge modelling, rather than simply analyzing and designing symbol-level inferential mechanisms. The papers included in this volume are thus drawn from a variety of research areas both at the cutting edge of research in ontologies and the Semantic Web and in the more traditionally grounded areas of knowledge engineering. A Semantic Web service can be seen as the addition of semantic technologies to Web services to produce Web-accessible services that can be described using appropriate ontologies, reasoned about and combined automatically. Since Web services can be seen as Web-accessible computational objects, much of the work in this area is also concerned with problem-solving methods (PSMs).

Advances in medical, biomedical and health services research have reduced the level of uncertainty in clinical practice. Clinical practice guidelines (CPGs) complement this progress by establishing standards of care backed by strong scientific evidence. CPGs are statements that include recommendations intended to optimize patient care. These statements are informed by a systematic review of evidence and an assessment of the benefits and costs of alternative care options. Clinical Practice Guidelines We Can Trust examines the current state of clinical practice guidelines and how they can be improved to enhance healthcare quality and patient outcomes. Clinical practice guidelines now are ubiquitous in our healthcare system. The Guidelines International Network (GIN) database currently lists more than 3,700 guidelines from 39 countries. Developing guidelines presents a number of challenges including lack of transparent methodological practices, difficulty reconciling conflicting guidelines, and conflicts of interest. Clinical Practice Guidelines We Can Trust explores questions surrounding the quality of CPG development processes and the establishment of standards.
