The first conference on concomitant infusion chemotherapy and radia tion therapy was organized with the intention of bringing together some of the investigators who have tested, during the last few years, the hypo thesis that continuous infusion chemotherapy could modulate the cytotoxic effect of radiation therapy to the point of having a strongly additive, if not synergistic activity on certain malignant tumors. This volume represents the detailed proceedings of this conference presented in a way that offers the reader a review of the on-going re search in the field. We have stressed a number of subjects from basic biologic research and influence of cell kinetics to the practical methods of drug delivery systems and early clinical experiences. The rationale for this new type of combined modality therapy has been presented by some of its pioneers. Early clinical investigations as well as the preliminary data of many that have not yet completely matured have also been included. The reader should look at these data with some reser vations. Ultimately, these results must be confirmed by larger prospective randomized studies with proper controls before becoming accepted as the treatment of choice in locally advanced tumors.

Abstract: Importance The number of older adults with head and neck squamous cell carcinoma (HNSCC) is increasing, and these patients are underrepresented in clinical trials. It is unclear whether the addition of chemotherapy or cetuximab to radiotherapy is associated with improved survival in older adults with HNSCC. Objective To examine whether the addition of chemotherapy or cetuximab to definitive radiotherapy is associated with improved survival in patients with locoregionally advanced (LA) HNSCC. Design, Setting, and Participants The Special Care Patterns for Elderly HNSCC Patients Undergoing Radiotherapy (SENIOR) study is an international, multicenter cohort study including older adults (≥65 years) with LA-HNSCCs of the oral cavity, oropharynx/hypopharynx, or larynx treated with definitive radiotherapy, either alone or with concomitant systemic treatment, between January 2005 and December 2019 at 12 academic centers in the US and Europe. Data analysis was conducted from June 4 to August 10, 2022. Interventions All patients underwent definitive radiotherapy alone or with concomitant systemic treatment. Main Outcomes and Measures The primary outcome was overall survival. Secondary outcomes included progression-free survival and locoregional failure rate. Results Among the 1044 patients (734 men [70.3%]; median [IQR] age, 73 [69-78] years) included in this study, 234 patients (22.4%) were treated with radiotherapy alone and 810 patients (77.6%) received concomitant systemic treatment with chemotherapy (677 [64.8%]) or cetuximab (133 [12.7%]). Using inverse probability weighting to attribute for selection bias, chemoradiation was associated with longer overall survival than radiotherapy alone (hazard ratio [HR], 0.61; 95% CI, 0.48-0.77; P .001), whereas cetuximab-based bioradiotherapy was not (HR, 0.94; 95% CI, 0.70-1.27; P = .70). Progression-free survival was also longer after the addition of chemotherapy (HR, 0.65; 95% CI, 0.52-0.81; P .001), while the locoregional failure rate was not significantly different (subhazard ratio, 0.62; 95% CI, 0.30-1.26; P = .19). The survival benefit of the chemoradiation group was present in patients up to age 80 years (65-69 years: HR, 0.52; 95% CI, 0.33-0.82; 70-79 years: HR, 0.60; 95% CI, 0.43-0.85), but was absent in patients aged 80 years or older (HR, 0.89; 95% CI, 0.56-1.41).

Objectives: To compare outcomes of patients with cervical cancertreated by chemoradiation alone (CRT) versus chemoradiationplus completion hysterectomy (CRT+CH).Methods: This is a retrospective comparison of 44 patients withinvasive cervical carcinoma treated by the combination ofchemoradiation plus completion hysterectomy (CRT+CH) and130 patients treated by chemoradiation alone (CRT), in a singleinstitution, from 2008 to 2018. CRT was the same to bothgroups: combination of external beam radiotherapy (EBRT) tothe pelvis and intracavitary brachytherapy and concomitantplatin-based chemotherapy. CH were performed after 6 to 12weeks from the end of CRT. All surgeries were Piver Ihysterectomy without lymph node dissection, performed bytraditional laparoscopic approach.In adittion to the raw data analysis, we used a propensitymatched analysis after pairing FIGO stage, histologic type andage of each group, to minimize selection bias and prove theresults.Results: In the raw data, FIGO (2009) stage in the study were as follow:30 IB2, 19 IIA, 125 IIB. There were 137 squamous cellcarcinomas (SCC) and 37 adenocarcinomas (ADC). Amongall cases, there were 36 recurrences (20.7%), being 29 (22.3%)in the CRT group and 7 (15.9%) in the CRT+CH group. Deathsoccurred in 79 cases (45.4%), being 71 in the CRT group(54.6%) and 8 in the CRT+CH group (18.2%). There was nodifference in complications among the two groups.29 pairs of cases were matched for the propensity analysis 1:1according to variables of FIGO, age and histologic type. Relapsesoccurred in 5 cases in the CRT + CH group and in 3 in the CRTalone group. Disease free was no different in the two group(p=0.933). Death occurred in 5 cases (17.2%) in the CRT + CHgroup, and 15 (51.7%) in the CRT alone group.Overall survival of the operative group was higher than the nonoperative group (97.01 VS 56.79 months) (p
