Malgré la mise en oeuvre de différentes stratégies en matière de santé, la performance du système de santé tchadien reste parmi les plus faibles d'Afrique. En cause, quatre facteurs essentiels: l'insuffisance des infrastructures sanitaires, la mauvaise organisation des soins et l'insuffisance quantitative et qualitative du personnel soignant et le sous-équipement.Face à cette incapacité du système public de santé de répondre aux besoins des populations, le gouvernement a décidé d'introduire la contractualisation comme outil, en vue d'améliorer la performance du système de santé. Toutefois, cette nouvelle politique, basée sur des contrats dits relationnels c'est-à-dire fondés sur la collaboration entre les parties qui permettraient d'obtenir des rapports plus équilibrés, ne peut se révéler un excellent facteur d'amélioration de la performance du système de santé qu'à la condition de bien spécifier leur régime juridique.Car le procédé contractuel a ceci de fâcheux que les partenaires croient savoir exactement ce qui se cache derrière alors qu'en réalité, ils ne "découvrent" généralement ce qu'ils ont signé qu'à l'occasion d'un différend.

Les deux premières parties sont consacrées à la présentation des concepts mais aussi des outils et des processus permettant la conception et la mise en œuvre d'une politique de contractualisation. La troisième partie détaille, pour chaque type d'acteurs, les possibilités apportées par la démarche contractuelle. Une dernière partie apporte la contribution de différentes approches pour analyser la contractualisation. Le message principal de cet ouvrage est que la contractualisation, lorsqu'elle est correctement utilisée, peut être un outil efficace pour améliorer la performance des systèmes de santé. Si la démarche contractuelle est applicable dans tous les pays, les auteurs accordent une importance particulière à la situation des pays en développement où la faiblesse des ressources et de la gouvernance donne une teneur particulière aux recommandations qui peuvent être faites.
