The ability to see deeply affects how human beings perceive and interpret the world around them. For most people, eyesight is part of everyday communication, social activities, educational and professional pursuits, the care of others, and the maintenance of personal health, independence, and mobility. Functioning eyes and vision system can reduce an adult's risk of chronic health conditions, death, falls and injuries, social isolation, depression, and other psychological problems. Abstract: Glaucoma is a devastating disease, causing significant loss of independence and diminished quality of life for millions of people. Glaucoma is a major public health problem in the United States, and blindness from glaucoma is believed to impose significant costs annually on the federal government in Social Security benefits, lost tax revenues, and health care expenditures [1]. The following project involves the Healthy People 2010 objective 28-6 of preventing vision loss due to glaucoma, and focuses on our community of optometrists to effectively diminish glaucomatous vision loss. The project objectives are as follows: evaluate current glaucoma treatment standards in Ohio, assess "best practice" glaucoma standards and encourage implementation, develop glaucoma management tools for optometrists, and ultimately, prevent vision loss. There are 1800 licensed optometrists in Ohio, of which 1200 are Ohio Optometric Association (OOA) members. A survey was administered by mail to 600 randomly selected member doctors. Additionally, attendees of the 2005 American Optometric Association (AOA) conference were selected to be surveyed. Surveys were passed out at a glaucoma course, a primary care course, and one course on dry eye. There were 90 surveys collected in total from the AOA conference. The surveys were then statistically analyzed. Focus groups were also organized to obtain optometrists' opinions on glaucoma management and potential tools to be used in patient education. The total number of surveys collected was 290. Answers were tabulated, and it was found that the survey respondents manage 142 ± 275 glaucoma patients on average, with a median of 50 and a range of 3 to 2200. Glaucoma screening tests were in concordance with professional guidelines, from both the American Optometric Association (AOA) and the American Academy of Ophthalmology (AAO). Measurement of intraocular pressure and stereoscopic appearance of the optic nerve head were found to be done by almost every optometrist surveyed. Fundus photography was only found to be used 20% of the time. Visual field testing is used 75% of the time in screening for glaucoma, and 90% of the time in managing glaucoma by our survey respondents; it is possible that imaging is taking over as a final tool in making the glaucoma diagnosis and managing it as well. Pachymetry is currently valued as part of the standard of care, thanks to recent research. Prostaglandins are found to be the first choice in medication in treating glaucoma. Focus groups were found to agree that standard of care is proper patient management to the highest level, more widespread education is needed, and that public screenings may soon be necessary to find those at risk and those with glaucoma. Although not many studies have been done on evaluating how and if optometrists are meeting the standard of care, or "best practice" as outlined by professional eye care organizations, this project provides insight into the realm of care in glaucoma diagnosis and management. Optometrists in Ohio are following clinical care guidelines in the majority of recommendations by both the AOA and the AAO. However, there does exist various shortfalls in diagnosis and management, and the need for further education of the general public. The standard of care in glaucoma is a dynamic body that must be continually re-evaluated and revised in order to provide the best care to patients.

Glaucoma Glaucoma is an eye disease that has the potential to damage your optic nerve. The optic nerve supplies visual information to your brain from your eyes. Glaucoma is usually, but not always, the result of abnormally high pressure inside your eye. Over time, the increased pressure can erode your optic nerve tissue, which may lead to vision loss or even blindness. If it's caught early, you may be able to prevent additional vision loss. What Causes Glaucoma? The back side of your eye continuously makes a clear fluid known as aqueous humor. As this fluid is made, it fills the front part of your eye. Then, it vacates your eye through channels in your cornea and iris. If these channels are blocked or partially obstructed, the natural pressure in your eye, which is called the intraocular pressure (IOP), may increase. As your IOP increases, your optic nerve may become damaged. As damage to your nerve progresses, you may begin losing sight in your eye. What causes the pressure in your eye to increase isn't always known.
