COPD is one of the most common diseases worldwide and is projected to be the third leading cause of death by 2020. But that does not mean it is easy to understand or manage. In everyday practice, pulmonologists face areas of controversy in COPD, for which evidence-based medicine is often unavailable. This ERS Monograph considers where the current controversies in COPD lie, discussing areas such as screening, premature birth, asthma–COPD overlap syndrome, treatment, rehabilitation and palliative care. This book will be of great interest to both clinicians and scientists, and aims to stimulate further discussion about this diverse and fascinating disease. "...contains a vast amount of information on the disease, its prevalence, signs and symptoms, diagnositc tests and treatment options. The book's format makes it quick and simple to find out what you need to know, and its size would make it easy to take to work for use in practice [...] invaluable for anyone working with patients with the disease." "When a thing ceases to be a subject of controversy, it ceases to be a subject of interest. " WILLIAM HAZLITT (1778-1830) This is the fourth volume in a series which we started in 1997. In 1976, Varco and Delaney edited a "surgical bestseller" entitled Controversy in Surgery. This was fol lowed by a second volume in 1985. The immense success of these books among both surgical trainees and experienced surgeons, and their wide international ap peal, supported the editors' notion that a view of controversy is integral to teach ing. However, despite major advances in surgical science and practice, no other similar publication has since been put together with the objective of addressing major controversies in surgery. The aim of this series is to address such deficiency by covering the most crucial current controversies in general, vascular and trauma surgery. Since almost everything is controversial in surgery, we will be able to tackle different issues almost each year. The editors have selected a nationally/internationally recognized authority for each topic. Most books available today are either "North American:' "British-Com monwealth" or "British-European:' contributing to the constant transatlantic rival ry. Our series specifically aims to bridge this "culture gap" and includes North American, British, European, and other authors while maintaining a cohesive structure. This will hopefully also broaden the appeal of such a book across both sides of the Atlantic and beyond.

This book explains how analysis of the heterogeneity of chronic obstructive pulmonary disease (COPD) enhances understanding of the condition and leads to improved, personalized treatment. State of the art knowledge is presented on a range of issues related to the heterogeneity of COPD, such as phenotypes (clinical, physiologic, radiologic, etc.), genotypes, and the tools to be used for dissecting heterogeneity (CT, MRI, biomarkers, etc.). Especially modern radiologic imaging holds promise in this context, and its role is described in detail with the aid of numerous illustrations. The implications of the heterogeneity for personalized treatment are clearly identified, with description of an appropriate tailored treatment strategy for each subgroup of patients. Information is provided on both current and emerging strategies, including bronchoscopic lung volume reduction and approaches to the management of pulmonary hypertension and comorbidities. This book will be a great asset in clinical practice and research for all who have an interest in COPD, a leading cause of morbidity and mortality worldwide.
