In view of the continuing preoccupation of all industrialized countries with the rising share of national resources devoted to health care, it is valuable to compare the fmanc ing and breakdown of health care expenditure on an international basis. How far should public spending on health care be regarded as a capital investment in the improvement of the health of the population and how far as subsidies to individual consumption? This question is of major importance to policy makers, including the me dical profession, politicians, employers, social security officials as well as to the public at large. In order to obtain some insight into the incentive structures enhancing competition among suppliers which have been built into the health care delivery systems in the various countries, the Interdisciplinary Research Centre for Public Health at St. Gall, in close co operation with the Institute of Insurance Economics and the Institute of Public Finance and Fiscal Law, both afftliated with the Saint Gall Graduate School of Economics, Business and Public Administration, initiated an international seminar held at Wolfsberg, Switzerland, 20-23 March 1979. The purposes of the Seminar were: 1. to review present experience on the development of health care costs and their financing - particularly the role of health insurance and the institutional relationships between public !illd private health insurance policies; 2.

This study, done at the request of the Hungarian government, presents evidence on cost-sharing in the health sector, and its application in Hungary. It presents results on the impact of cost-sharing on revenues in health facilities and insurance, financial sustainability, informal payments, overall service use, and equity in access. Five keyfindings emerge: cost-sharing could lead to a reduction in unnecessary care provided to insured patients who do not have to pay the full price of care; cost-sharing could help reduce informal payments and keep patient payments in the system; cost-sharing with exemption policies are a prerequisite to provide equity in access to care; cost-sharing could support cost containment strategies; experience from OECD countries provides examples of successful cost-sharing policies. Based on these findings. the study recommends that Hungary continues to monitor and evaluate the impact of cost-sharing on access, to identify possible negative effects on equity in service use. In Hungary, financial incentives through the provider payment system could eventually support the effect of the government supply-side strategy. Such financial incentives could be set to providers through capitation payment to reward better quality of care, and Diagnosis Related Group (DRG) payments with expenditure ceilings with strict utilization review and quality assurance control to set an incentive for efficient provision of care.

This paper presents findings from the 2008 EBRI/MGA Consumer Engagement in Health Care Survey, focusing on public opinion regarding variation in cost sharing as it relates to consumer engagement in health care. Overall, 58 percent of individuals support lower cost sharing for patients who are actively participating in a program to maintain or improve their health; 40 percent support lower cost sharing for patients who use treatments that have been scientifically proven to be effective for their medical condition; 34 percent support lower cost sharing for patients who choose to see high-performing health care providers; 47 percent support lower cost sharing for patients who choose less invasive procedures to treat their medical conditions; and 58 percent support lower cost sharing for patients who carefully follow their treatment regimens. Between 13 and 25 percent disagreed with the use of lower cost sharing, while 28-40 percent neither supported nor opposed it. The paper also examines how the support for lower cost sharing related to patient engagement varies by health status and behaviors, demographics, and work status variables. Employers and insurers are going to continue experimenting with various ways in which they can use features of their benefits plan to increasingly engage workers and their families in their health care in order to manage health care costs more effectively. They will find that some things work while others do not. The support for consumer engagement initiatives will vary across employees and may ultimately affect the success of specific programs.

This report analyzes the possible distributional consequences of several government policies for controlling health care costs. Drawing on the results of RAND's Health Insurance Study, it evaluates the potential savings from policies designed to increase cost sharing--deductibles and copayments--in both government and employer financed health insurance programs. It then examines how the cost of realizing these savings is distributed among major actors in the health care system--patients, doctors, hospitals, suppliers, insurers, and state and federal governments. The study assesses consequences of specific proposals to increase income taxation of employer financed insurance programs and to increase copayments in Medicaid.
