This book is divided into eleven chapters. Chapters 1, 2 and 3 present analyses of the concepts of public health, sustainability and policy change. Chapters 4 and 5 describe the stakeholder analysis and national health accounts frameworks. These chapters determine the attributes, characteristics and other features of these concepts and frameworks. The aim is to improve general clarity and understanding of these concepts and frameworks that contribute to the Sustainability Impact Assessment framework and the case study methodological approach that exemplifies its role in sustainability assessment of policy change in immunization systems. Chapter 6 outlines the Sustainability Impact Assessment framework itself, setting out the steps involved in a typical SIA with examples of methodologies used in the case study. This book disentangles the issues in connection with the advancement of Health Technology Assessment (HTA) and its interface with health policy. It highlights the factors that should shape its progress in the near future. Interdisciplinary and critical views from a number of professionals are put together in a prescient order to cast some light and make recommendations as to the next steps HTA should take to be fit for purpose. A wealth of documents dealing with HTA have been published over the last three decades. HTA allegedly is one of the bedrocks of regulation and medical decision making. However, counter vailing visions contend that geographical variations in the role that HTA is actually playing within countries pinpoints specific room for improvement. Given our social preferences, cherry-picking HTA’s features and successes over the last decades moves it away from its possibility frontier. Some of the most noteworthy hindrances that HTA faces, in several countries, to making headway towards its consolidation as an efficient tool for regulation and decision making are as follows: insufficient resources, delays in assessment, inadequate priority setting, regulatory capture, public distrust, actual influence on regulatory decisions, the need for strengthening international cooperation and harmony, the lack of sound and consistent assessments of diagnostic tests, medical devices and surgical innovations and limited dissemination. Time has come for HTA to take a renewed stand. There is a pressing need to submit HTA to in-depth critical scrutiny.

The global health community is broadly in agreement that achievement of the health-related Sustainable Development Goals (SDGs) hinges upon both an escalation of the financial resources dedicated to primary health care (PHC) and a more effective use of those resources: more money, better spent. This book introduces and explicates the end-to-end resource tracking and management (RTM) framework, which includes five components that determine effective and efficient financing for PHC: resource mobilization, allocation, utilization, productivity, and targeting.In addition, this book compiles detailed results from the most recent RTM-based resource tracking efforts for PHC in selected countries. This is to demonstrate how the RTM framework can be used to bring a set of separate resource tracking efforts at different stages of flow of funds into a comprehensive process with an end-to-end 'storyline'. In order to build a functional PHC system that addresses access, quality, and equity issues, this book highlights the key (public) financing issues that researchers, technical advisors, and policy makers would need to address in addition to more resources.

The framework is based on elements commonly used in general governance and institutional assessment, and is tailored to the specific characteristics of HA institutionalization. The common factors are grouped into four domains: (1) demand, (2) governance and financing, (3) institutional technical capacity, and (4) dissemination and use of data. For each domain, a set of questions is proposed to facilitate the application of the framework and provide a clear picture to understand the progress and challenges in each country and for cross-country comparisons. The framework aims to assess where countries are in the process of institutionalizing HA, to identify areas where more effort is needed to advance or sustain progress in institutionalization, and to enable cross-country learning, mainly targeting policy makers and health accounts teams.
