Es ist ein zusammengesetztes Gelenk, in dem Femur, Tibia und Patella sowie zwei Faserknorpelringe, auch Menisken genannt, miteinander in gelenkiger Verbindung stehen. Durch die beiden Menisken werden die Gelenkflächen von Femur und Tibia einander angepasst und die Kraft aufnehmende Fläche wird vergrößert. Bei Beugung des Kniegelenks führt der Femur eine kombinierte Gleit- und Rollbewegung auf der tibialen Gelenkfläche durch, wodurch die Menisken mit zunehmender Beugung nach hinten verlagert werden. Die Bewegungen im Kniegelenk werden durch die Seiten- und Kreuzbänder geführt (vgl. FALLER 1999, 179). Während die Seitenbänder vor allem das gestreckte Bein sichern, übernehmen die Kreuzbänder die Sicherung hauptsächlich im gebeugten Zustand der Kniege lenke. Wegen der ungleichen Krümmung der Oberschenkelkondylen sind die Seitenbänder nur im gestreckten Kniegelenk straff gespannt, während sie in Beugestellung erschlafft sind. Innen- und Außendrehung des Unterschenkels im gebeugten Kniegelenk werden durch die Kreuzbänder eingeschränkt. Muskeln, die an der Innenseite der Tibia ansetzen, rotieren nach innen. Der zweiköpfige Oberschenkelmuskel hat seinen Ansatz am Wadenbeinknöpfchen und rotiert als einziger Muskel den Unterschenkel nach außen. Alle drei Muskeln beugen im Kniegelenk, so auch der Schneidermuskel. Als wichtigster Strecker im Kniegelenk wirkt der vierköpfige Oberschenkelmuskel, der mit seiner Ansatzsehne (Lig. Patellae) an der Rauhigkeit des Unterschenkels ansetzt. Als größtes Sesambein des menschlichen Körpers ist die Kniescheibe in das Lig. Patellae eingelagert. Bei zunehmender Beugung des Kniegelenks bewegt sich die Kniescheibe nach unten. Auf Grund der großen Kraftübertragungen im Femoropatellagelenk, insbesondere in Beugestellung, ist es das am höchsten belastete Gelenk und zeigt am frühesten und häufigsten degenerative Knorpelveränderungen (vgl. FALLER 1999, 179-182)

An aberrant frenum gives rise to aesthetic and functional problems and needs to be eliminated.The frena may jeopardize the gingival health when they are attached too closely to the gingival margin, either due to an interference in the plaque or due to a muscle pull. In addition to this, the maxillary frenum may present aesthetic problems or compromise the orthodontic result in the midline diastema cases, thus causing a recurrence after the treatment. The management of such an aberrant frenum is accomplished by performing a frenectomy. This is a compilation of a brief overview about the frenum, with a focus on the indications, contraindications, advantages and the disadvantages of various frenectomy techniques, like Miller's technique, V-Y plasty, Z-plasty, frenectomy by using electrocautery. and laser.
