Drawing from the work of academics and practitioners from ten states across the country, this edited volume showcases and synthesises the diversity and richness of efforts to understand and act on the social determinants of health in India, the conditions in which we are born, grow, live work and age. Such an effort is salient in the current era of Sustainable Development Goals (SDG), which have foregrounded the issue of equity and the need for a comprehensive, multi-sectoral agenda for health and development. In India, particularly in the last decade, there have been myriad efforts to more critically theorise and intervene in areas with bearing on health, like conflict, nutrition or urbanisation, or to address the concerns of vulnerable groups like women, children and the elderly. From these efforts emerge lessons of convergence for academic and policymaking institutions in India who are looking to operationalise and bring life to the SDG agenda in India and other Low and Middle Income Country settings. In recent years, the number of publicly available tools and indices assessing social determinants of health (SDOH) has grown exponentially. While many of these indices have been developed to assist researchers and practitioners with identifying vulnerable communities, it is difficult to determine the most appropriate measure, index, or combination of indices to use given the research question of interest. This paper presents an overview of the most commonly included indices, highlights commonalities, and identifies some differences in what they measure. We also discuss challenges with using these measures, including the use of state level data to examine local level issues and how the use of atheoretical indices challenges the application of SDOH measurement. Findings are intended to provide researchers and practitioners with information about SDOH data available through these common indices to inform how they are applied based on the needs of their work.

Pembangunan yang sesungguhnya senantiasa menempatkan manusia sebagai titik sentral perhatian atau sebagai subjek yang berperan aktif sehingga pembangunan mempunyai ciri dari rakyat dan untuk rakyat. Dalam kerangka ini maka pembangunan nasional ditujukan untuk meningkatkan partisipasi masyarakat dalam semua proses dan kegiatan pembangunan. Untuk mencapai tujuan tersebut perlu dilakukan upaya peningkatan kualitas penduduk sebagai sumber daya manusia, baik dari aspek fisik, mental, dan spiritual (kesehatan), aspek intelektualitas (pendidikan), aspek ekonomi (daya beli), serta aspek moralitas (iman dan taqwa) sehingga pembangunan diselenggarakan dengan pendekatan holistik (menyeluruh) pada seluruh aspek kehidupan. Pengembangan manusia (human development) dibedakan dengan pengembangan sumber daya manusia (human resource development)yang dianggap berkonotasi ekonomi semata. Sebagai sumber daya manusia, manusia semata-mata dipandang sebagai faktor produksi dalam proses ekonomi. Di lingkungan United Nations Development (UNDP) telah berkembang gagasan untuk menekankan pentingnya pengembangan manusia (human development). Dua aspek pengembangan manusia yang perlu dilakukan. Pertama, upaya mengembangkan kemampuan (capability) manusia, yaitu memiliki kemampuan untuk menempuh hidup dengan usia harapan hidup (life expectancy) yang panjang dan sehat, memiliki kesempatan dan kemampuan untuk memperoleh pendidikan agar dapat menanggapi kemajuan zaman, dan memperoleh kesempatan akses dalam mengusahakan sumber-sumber pendapatan, alam, atau pembiayaan pembangunan untuk meningktakan tingkat kehidupan. Kedua, mengembangkan penggunaan kemampuan dan mempertinggi partisipasi dalam kegiatan ekonomi produktif, sosial budaya, dan politik.

Since 1965 the foreign-born population of the United States has swelled from 9.6 million or 5 percent of the population to 45 million or 14 percent in 2015. Today, about one-quarter of the U.S. population consists of immigrants or the children of immigrants. Given the sizable representation of immigrants in the U.S. population, their health is a major influence on the health of the population as a whole. On average, immigrants are healthier than native-born Americans. Yet, immigrants also are subject to the systematic marginalization and discrimination that often lead to the creation of health disparities. To explore the link between immigration and health disparities, the Roundtable on the Promotion of Health Equity held a workshop in Oakland, California, on November 28, 2017. This summary of that workshop highlights the presentations and discussions of the workshop.

The World Health Organization defines the social determinants of health as "the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life." These forces and systems include economic policies, development agendas, cultural and social norms, social policies, and political systems. In an era of pronounced human migration, changing demographics, and growing financial gaps between rich and poor, a fundamental understanding of how the conditions and circumstances in which individuals and populations exist affect mental and physical health is imperative. Educating health professionals about the social determinants of health generates awareness among those professionals about the potential root causes of ill health and the importance of addressing them in and with communities, contributing to more effective strategies for improving health and health care for underserved individuals, communities, and populations. Recently, the National Academies of Sciences, Engineering, and Medicine convened a workshop to develop a high-level framework for such health professional education. A Framework for Educating Health Professionals to Address the Social Determinants of Health also puts forth a conceptual model for the framework's use with the goal of helping stakeholder groups envision ways in which organizations, education, and communities can come together to address health inequalities.

Era paradigma sehat dicanangkan oleh Presiden RI pada 1 Maret 1999 sebagai Gerakan Pembangunan Berwawasan Kesehatan yang menjadi landasan visi, misi, dan strategi pembangunan kesehatan. Secara mikro, paradigma sehat berarti pembangunan kesehatan harus ditekankan pada upaya promotif dan preventif, tanpa mengesampingkan upaya kuratif dan rehabilitatif. Istilah promosi kesehatan di dunia baru muncul pada tahun 1980-an, sedangkan di Indonesia baru muncul pada tahun 1990-an. Sebelum itu, promosi kesehatan lebih dikenal dengan istilah pendidikan kesehatan, penyuluhan kesehatan, komunikasi kesehatan sebagai komunikasi-informasi-edukasi (KIE), pemasaran sosial, penggerakan peran serta masyarakat, dan lain-lain. Kementerian Kesehatan RI mendefinisikan promosi kesehatan sebagai proses pemberdayaan atau pemandirian masyarakat untuk memelihara, meningkatkan, dan melindungi kesehatannya melalui peningkatan kesadaran, kemauan dan kemampuan, serta pengembangan lingkungan sehat. Promosi kesehatan dipandang sebagai kombinasi dari kegiatan pendidikan kesehatan dan penerapan kebijakan publik yang sehat. Pendidikan kesehatan berfokus pada pembangunan kapasitas individu (building individuals' capacities) melalui pendidikan, teknik motivasi, pembangunan keterampilan, dan peningkatan kesadaran (consciousness-raising). Kebijakan publik yang sehat menyediakan dukungan lingkungan (environmental support) yang akan mendorong dan meningkatkan perubahan perilaku (behaviour change). Buku ini ditulis berdasarkan pengalaman implementasi penulis dalam teori pendidikan dan promosi kesehatan selama puluhan tahun.
