Anergic reactions and anergic diseases are found in many fields of medicine. As wen as affecting the skin, allergic diseases occur in internal organs such as the lungs and the kidneys and in the vascular and nervous systems. Interdisciplinary understanding and cooperation thus seem necessary to get a better insight into allergic reactions in the different fields of medicine in order that we may give our patients the best diagnostic and therapeutic care. Allergic diseases are on the increase. Studies from Sweden show that 15% of the population there are atopic, and some people speculate that in Germany about 10%-15% of all adults suffer from one of the many different allergic conditions. We in our clinic also have the impression that the incidence of atopic reactions has increased in recent years. The figure of 15% would mean something like 200 000 people in Munich alone! We all know that a wide variety of environmental factors can induce allergic reactions in the skin, the respiratory system, or internal organs. The most important anergens in our environment are pollen, molds, animal dander, housedust mite, food or food additives, various insects, industrial compounds and drugs. The direct costs of hospital and community medical care, other services, and drugs to help patients with asthma, hay fever, and skin allergies have in creased considerably in the last 10 years. The figures should make it clear to everybody that we are dealing with a health and environmental problem of great practical importance.

Anaphylaxis is an immediate-type allergic reaction involving the whole organism. It is the most life-threatening allergic condition. Although there are few exact epidemiological data regarding prevalence, estimates regarding insect sting anaphylaxis range from 1-3% in the general population, but much higher values have been reported by some authors for food and drug-induced anaphylaxis. Anaphylaxis is the main acute killer of allergic individuals. Although anaphylaxis was discovered at the beginning of the 20th century, there are still many unresolved issues. These include non-IgE-mediated anaphylactoid reactions, non-immunologically mediated anaphylactoid (pseudo-allergic) reactions, pathophysiological events at the microcirculatory level, appropriate therapy for the acute reaction, strategies for prevention, public education about the problem and new approaches to prevention and therapy at the IgE level. All these subjects are discussed in this book. Since anaphylaxis occurs acutely and is unforeseen, it is very difficult to organize controlled studies regarding therapy and prevention. The spectrum of symptomatology covers many clinical areas (skin, respiratory, cardiovascular and gastrointestinal system), therefore inter-disciplinary approaches are necessary for progress in the field. There is widespread uncertainty among physicians about therapy, especially concerning self-administered treatment. In this important book, an multidisciplinary group of experts explore the pathophysiology of different types of anaphylactic and anaphylactoid reactions. Evidence is presented on the epidemiology of these conditions while problems relating to diagnosis, therapy and prevention are examined in detail. According to most studies, allergic reactions represent 35%-50% of all untoward reactions to drugs, yet the pharmacological literature concerning the clinical aspects, diagnosis, and pathophysiological mechanisms of drug allergy is markedly less extensive than reports dealing with the toxicological or pharmacological effects of drugs. The main reasons for this state of affairs may be on the one hand that until a few years ago the pathophysiological mechanisms of the various types of allergic reactions were not well understood, and on the other hand that objective diagnosis of a drug allergy is still fraught with serious difficulties. Drug allergy is still an unpopular topic for most allergologists and pharmacologists; this is reflected by the fact that despite their frequency, allergic reactions to drugs still occupy a relatively small proportion of space in most pharmacology handbooks and in classical books devoted to the side effects of drugs. There has recently been considerable progress in research into the immunologi cal and pathophysiological events occurring in allergic reactions, and on that basis investigations of various drug allergies have also yield~d new objective findings. Consequently, it was natural to attempt a review of the most frequent and important drug allergies in the form of a handbook. We originally intended to present a comprehensive review of all drug allergies, but the realization of this goal soon became more difficult than we had at first imagined.

Nine years have passed since the first edition of Care of the Critically III Patient was published. As its editors Maurice Rapin and I were fortified by the many positive and helpful comments of reviewers and readers: the work appeared to have fulfilled our original aims and objectives. Intensive care medicine has, of course, advanced and changed in many ways since 1982 and it therefore seemed appropriate to consider the preparation of a second edition. Encouraged by the reception of the first and the timely prompting of the publishers we began, in 1986, to prepare for a new edition. Caring for critically ill patients is of universal concern and we were therefore delighted when Warren Zapol joined us as C'n editor widening our international base. The three of us subsequently spent many happy and productive times together planning the content, priorities, scale and contributions for the new edition. A great tragedy then overcame us: Maurice Rapin died suddenly. His death deprived us of a true friend and colleague who had been an international pioneer during the development of intensive care medicine. I will always remember, with affection and professional pride, the many hours he and I worked together in preparing the first edition of this book and also as founding editors of the now firmly established journal Intensive Care Medicine. An obituary, written by Warren Zapol, that appeared in the journal follows the Preface.

Since surgery became a method of treating patients, progress in the field has been intimately associated with experimentation and serendipitous research. As in other clinical specialties advances in surgery can be considered to result from experimental attempts to increase basic knowledge and to improve technical skills. However, virtually in no other area do concepts and approaches of experimental research enter clinical routine as fast as in surgery. There are numerous examples of this. Thus, allocation of manpower and resources for surgical research can be considered particu- 1arly profitable as convincingly shown, for instance, in renal transplantation by comparison of the long-term burden of hemodialysis vs. kidney grafting, apart from the relief of suffering and misery. Surgery is a continuously spreading field, and so is surgical research. This volume is a case in point. Its spectrum reaches from basic molecular biological aspects of immune mechanisms to the current state of the art of pulmonary surgery of cancer metastases, and from the molecular processes of cell swelling in ischemic brain edema and blood-brain barrier damage to novel forms of resuscitation or of treatment of insulin-dependent diabetes mellitus. Surgical research faithfully reflects a constant reorientation of medical disciplines. Treatment of renal or gallbladder concrements was a major domain of surgery, where the introduction of extracorporeal shock wave treatment now supplies noninvasive, virtually conservative alternatives.
