Diabetes in Qatar has seemingly become an enormous challenge to public healthcare systems. Qatar is one of the GCC countries which are known to have the highest diabetes incidence rates. In a global ranking, Qatar is ranked six whereas the United Arab Emirates and Saudi Arabia who are members of the GCC countries are ranked second and third, respectively in regard to incidence rates of diabetes. Current epidemiological reports indicate that an estimated 16 percent of the adult population in Qatar is suffering from diabetes, in which type 2 diabetes is the most prevalent compared to type 1 diabetes. However, other clinical reports indicate that diabetes prevalence among children below the age of five years in Qatar has also assumed upward trends. For instance, prevalence rate for diabetes among children was reported to be 13.7% in 1997, but this rate has more than doubled to reach 28.2% in 2007. From an epidemiological perspective, the burden of diabetes to Qatar’s healthcare system seems to be overwhelming. Therefore, it has become a significant concern in community health nursing because the number of Qatari diabetic patients seeking for medical treatment has increased significantly. This situation has been worsened by the rising trends of obesity prevalence rates. Currently, 45% of the population in Qatar is obese in which 55% of diabetic patients are obese. Therefore, this correlation in incidence and prevalence rates of the two diseases complicates prevention and health promotion measures in Qatar. As a result, Qatar’s efforts to reduce the prevalence of diabetes have been faced with enormous challenges. However, the country is optimistic that the Qatar Diabetes Project and Stem Cell Projects will be successful. In regard to the risk factors, the unprecedented increase of diabetes prevalence is attributable to lifestyle and genetic factors. Clinical studies reveal that most people in Qatar have changed their cultural lifestyle to western-related lifestyle, and this is believed to have contributed significantly to the current diabetes crisis. In general, diabetes issue in Qatar needs appropriate epidemiological measures to reduce its burden to the country’s healthcare system in which social health determinants and epidemiological indicators will help in addressing the issue.

Background: Qataru2014an Arabian Gulf country with the highest per capita income in the worldu2014has the tenth highest diabetes mellitus prevalence worldwide (16.7%). Prevalence of obesity, active smoking, and physical inactivity are reported as being 41.4%, 16.4%, and 45.9%, respectively. It is therefore critical to assess the future burden of type 2 diabetes mellitus (T2DM) in Qatar and its risk factor drivers.Aims: To develop and demonstrate a novel mathematical modelling approach to forecast the burden of T2DM and to investigate its epidemiology for the purpose of informing public health policy and programming. Method: A population-level compartmental mathematical model was constructed and applied to Qatar. The model was stratified according to sex, age group, risk factor status, and T2DM status, and was parameterized by nationally-representative population-based data, including demographic, epidemiological, and cost data. The model disaggregates the population into 20 age groups, with each group representing a five-year age band and incorporates four main susceptible classes: healthy (i.e. non-obese, non-smoker, physically active, and non-diabetic), obese, smoker, and physically inactive. The model accounts for overlaps between risk factors by further stratifying the susceptible population into compartments with overlapping risk factors. Spending attributed to T2DM of Qataru2019s total health expenditure was estimated based on per capita health expenditure. The effect of the risk factors on T2DM was estimated by calculating the total T2DM incident cases attributed to each of the risk factors through population attributable fraction approach. Sensitivity and uncertainty analyses were conducted to assess the robustness of the findings.Results: T2DM prevalence was predicted to increase from 16.7% in 2012 to at least 24.0% by 2050. The rise in T2DM was most prominent in age groups 45-49 and 50-54 years. T2DM health expenditure was estimated to increase by 200-600% and to account for up to 32% of total health expenditure by 2050. Prevalence of obesity, smoking, and physical inactivity was predicted to increase from 41.4% to 51.0%, from 16.4% to 19.4%, and from 45.9% to 53.0%, respectively by 2050. The proportion of T2DM incidence attributed to obesity, smoking and physical inactivity was estimated at 57.5%, 1.8%, and 5.4%, respectively in 2012, and 65.7%, 2.1%, and 6.0%, respectively by 2050. Exploring different scenarios for the trends in T2DM-related risk factors, T2DM prevalence could potentially reach as high as 37.7% by 2050. Sensitivity and uncertainty analyses affirmed the validity of our predictions.Discussion: Using our innovative modelling approach, a rising T2DM epidemic in Qatar was predicted to continue in the next three decades, driven by population growth, ageing and adverse trends in risk factors. Obesity was found to be the principal risk factor explaining over half of T2DM incidence. T2DM must be a national priority addressed by preventive and therapeutic interventions targeting T2DM and its modifiable risk factors. We applied the model to Qatar, a high-burden T2DM country, but the approach is general and can be applied broadly across countries.

Background: There is an urgent need for effective public health strategies to reduce the burden of type 2 diabetes mellitus (T2DM) in countries with high prevalence, such as Qatar where T2DM prevalence was measured in 2012 at 16.7%.Aims: The objective of this study was to estimate (for Qataris) the epidemiological impact, on T2DM incidence and prevalence by 2050, of reducing the prevalence of the key T2DM risk factors of obesity, smoking and physical inactivity by 2031. Method: We extended a recently developed epidemiological deterministic mathematical model, by adding an intervention component, to estimate the implications of various u201cwhat ifu201d scenarios for T2DM prevention. Our model was stratified by sex, age, risk factor status, T2DM status, and intervention status and was parameterized using nationally-representative data. In the modeled intervention scenarios, the prevalence of obesity, smoking and physical inactivity was reduced by various scales; from a u201cconservativeu201d (10%) to an u201cidealu201d reduction (50%). We also modeled the impact of physical activity as an intervention, beside modelling the reduction in physical inactivity. Physical activity was assumed to reduce the risk of developing T2DM by 20% (by walking) to 50% (by vigorous activity). The epidemiological outcomes from the intervention scenarios were compared to a counterfactual scenario with no intervention.Results: By 2050, T2DM prevalence reached 24.0% in the counterfactual scenario of no intervention. The intervention scenario implementing the WHOu2019s Global Action Plan targets of simultaneously halting obesity and reducing smoking and physical inactivity predicted a T2DM prevalence of 21.0%; a 12.0% reduction in T2DM prevalence. This scenario predicted also that 13.2% of new T2DM cases and 3.2% of T2DM mortality cases would be averted by 2050. The intervention scenario modelling a 10-50% reduction in obesity prevalence reduced T2DM prevalence by 7.8-33.7%, averting 8.4-38.9% of new T2DM cases and 2.1-13.2% of T2DM mortality cases by 2050. The intervention scenarios modelling a 10-50% reduction in smoking or physical inactivity prevalence did not lead to large reductions in T2DM prevalence, incidence or mortality. The intervention scenario modelling an increase in the level of physical activity reduced T2DM prevalence by 3.3-9.2%, averting 4.2-11.5% of new T2DM cases and 1.9-5.2% of T2DM mortality cases by 2050.Discussion: Future T2DM burden among Qataris can be reduced by reducing key T2DM risk factors. The level of reduction depends on the targeted risk factor and scale of reduction. A modest reduction in obesity prevalence can result in considerable reduction in T2DM prevalence, incidence and mortality. Feasibility of implementing such reductions in T2DM risk factors remains to be explored and determined.

Background: Type II diabetes mellitus (DM) is increasing worldwide, leading to greater health expenses, and its complications were responsible for 4.6 million deaths in 2011 (International Diabetes Federation [IDF], 2011). This study investigates how diabetes knowledge, self-efficacy, social support, and self-management relate to diabetes control. Self-efficacy and social support were examined, respectively, as a mediator and a moderator. The hypothesis is that there is a directional relationship between the concepts of diabetes knowledge, self-efficacy, social support, self-management, and the outcome, glycemic control. Method: A convenience sample of 259 Qataris with type II diabetes mellitus were recruited from Hamad Medical Corporation’s outpatient clinics and Home Healthcare Services (HHCS) in Qatar. Using a cross-sectional correlational design, correlation/regression coefficient tests were used to examine the relationships among these variables and their effect on the dependent variable, glycemic control. Participants responded to a set of questionnaires independently, via telephone, or in-person interviews. The following instruments were used: the Diabetes Knowledge Test (DKT), Self-Efficacy for Diabetes (SED) scale, Social Support by Medical Outcomes Study (MOS), the Self-Management Profile for Type 2 Diabetes (SMP-T2D), and the patients' glycemic control was measured by glycosylated haemoglobin (HbA1c). The collected data were entered into a computer database and patient confidentiality was strictly maintained. Pearson correlation coefficients, multiple, and hierarchical multiple regression were used to analyze the relationships among the variable; in addition, the mediating effect of self-efficacy, and the moderating effect of social support were tested. Results: The study sample’s age averaged 50.7 years (SD=13.0). The duration of diabetes averaged 9.30 years (SD=8.1); the average blood glucose was 176.8 mg/dl (SD= 77.8), and the average HbA1c was 8.6% (SD= 2.2). There was no relationship between diabetes knowledge, self-efficacy, social support, self-management and glycemic control for adult Qataris with type 2 diabetes. Furthermore, self-efficacy did not mediate the relationship between diabetes knowledge and self-management, nor did social support moderate the relationship between self-management and HbA1c. Conclusion: No relationships were found in this sample of Qatari older adults with type 2 diabetes. Future research is needed with larger samples to examine how these and other cultural factors explain glycemic control in this population.
