For more than a generation haemodialysis has been the principal method of treating patients with both acute and chronic renal failure. Initially, developments and improvements in the system were highly technical and relevant to only a relatively small number of specialists in nephrology. More recently, as advances in therapy have dem onstrated the value of haemofiltration in the intensive therapy unit and haemoperfusion for certain types of poisoning, the basic principles of haemodialysis have been perceived as important in many areas of clinical practice. Exit site infection (ESI) is an important clinical problem in peritoneal dialysis (PD) patients and is a significant cause of peritonitis and catheter loss. While most ESIs are caused by skin commensals, rising incidence of atypical and resilient organisms such as mycobacteria, Pseudomonas and Burkholderia species has been observed. The diagnosis and management of these emerging pathogen remain difficult and poorly defined. This chapter highlights the evaluation and management of ESI in PD patients. The clinical features, microbiology, and ultrasonographic findings are discussed. The general and specific management of ESI due to different organisms will also be elaborated. ESI is usually a clinical diagnosis, but the use of bedside ultrasound can help assess for any collection around the cuff and tunnel tract involvement. Topical prophylaxis remains an effective way to prevent ESI. While the majority ESIs are related to skin flora and can be managed successfully by topical or systemic antimicrobials, clinicians should be alert to the emergence of resistant and atypical microorganisms. Surgical treatment should be reserved for ESI refractory to medical treatment or those with associated peritonitis.

All rights not granted by the above license are retained by the author. Abstract: Background: End Stage Renal Disease (ESRD) is one of the commonest diseases in Hong Kong. Patient with ESRD needs to start dialysis for life maintenance. Peritoneal dialysis (PD) is the predominant dialysis modality for home dialysis patients. More than 80% of dialysis patients in Hong Kong receive PD. However, it also brings out some PD-related infectious complication such as tenckhoff exit-site infection, tenckhoff tunnel infection and PD peritonitis. These complications markedly contribute to treatment failure in PD patients. Especially PD peritonitis, it remains a leading complication of PD. Also it is a main cause of patients switch to haemodialysis (HD)and discontinue PD. Nevertheless, if the primary prevention of PD education do better, research evidences have shown that peritonitis infection rate of PD patients can be effectively reduced. It can be achieve by the utilization of effective education strategies and advanced training skills to enhance patients' knowledge and skills of peritoneal dialysis. Purpose: This written proposal aims to identify the best evidence of PD education and to develop a guideline for this health education programme. The goal of the programme is to reduce the rate of PD-related infection for patients who started PD treatment at home after first CAPD training and education. Method: A total of 12 studies which focused on PD education and strategies for reducing PD-related infections were searched from electronic databases. Data extraction and critical appraisal were performed on these 12 studies. After the integrative review, the implementation potential was assessed. The results shown that the transferability of finding is high and it is feasible to conduct the proposed innovation. Then, the evidence-based guideline for PD education programme were developed and based on the high and medium level of evidence with grades of recommendation stated. Before implementing the proposed innovation, a communication plan was developed and targeted the various stakeholders (the administrators, nurses, patients and their relatives). The proposer would initiate the change and the programme leading group would guide and sustain the proposed innovation. The next process was planning a pilot study to examine the feasibility of the proposed innovation before implementation. Finally, different outcomes of the programme has been identified and evaluated in the evaluation plan. The methods for data analysis were formulated. Conclusion: The proposed peritoneal dialysis education programme with best evidences support is worthy to be adopted in the clinical setting for the beneficial of PD patients to reduce their PD-related infectious complications. Peritoneal dialysis (PD) has become a real alternative to hemodialysis (HD) in recent decades, with comparable survival rates, lower costs, and improved patient quality of life. Nevertheless, PD-related infections, including peritonitis, exit-site infections (ESI), and tunnel infections, are important complications, resulting in significant morbidity and a 3.5-10.0% risk of death. Patients with peritonitis usually present with cloudy PD-fluid and abdominal pain; however, PD-associated peritonitis should always be included in differential diagnosis of PD patients with abdominal pain. The most common causative organisms for PD-associated peritonitis are gram-positive bacteria; however, gram-negative species are clinically important, due to the antibiotic resistance. The selection of empiric antibiotics depends on the center-specific distribution of microorganisms and antimicrobial susceptibility profiles. Typically, a first-generation cephalosporin is used in combination with broad gram-negative coverage (e.g., aminoglycoside, ceftazidime, or cefepime). High levels of methicillin-resistant Staphylococcus epidermidis or Enterococcus spp. strains require the use of vancomycin in many centers. Furthermore, for patients without clinical improvement after 5 days, or with fungal peritonitis, catheter removal is indicated.

Abstract: The prevalence of end stage renal disease (ESRD) is increasing even with an improvement in management of diabetes, hypertension and glomerulonephritis in the past decades. This phenomenon is plausibly due to a high prevalence of diabetic mellitus (DM) and increased aging in the general population. Nowadays, more than forty percent of ESRD patients on dialysis have DM as the primary disease. For patients with ESRD, peritoneal dialysis (PD), haemodialysis and RTx are the choices of replacement therapy. Although transplantation is the most desirable modality, the waiting time may take decades because of shortage of organ donors. In Hong Kong, about 80% of ESRD patients receive PD as the dialysis treatment. The peritoneum acts as a dialysis membrane for both solute clearance and fluid removal. Inevitably, PD treatment carries some adverse effects on patient survival and various reasons may account for the associated morbidity and treatment failure. PD related peritonitis and failure of fluid removal due to ultrafiltration (UF) failure are the most common causes for discontinuation of PD treatment. Both peritonitis and UF failure impair solutes exchange and cause inadequate dialysis. The aims of this thesis are to study the risk factors for 1) reduced patient survival, 2) developing peritonitis, and 3) having UF failure during PD treatment. Any improvement in management of these adverse factors will definitely prolong the longevity of PD treatment and patient survival. Our prospective multi-centre study of patients on long-term PD has shown that patients had 2- and 4-year survival rates at 94.0% and 74.8%, respectively. The risk factors for reduced patient survival included high body mass index (BMI), low urine volume and cardiovascular disease (CVD). In another study, we found that patients with chronic inflammation after peritonitis associated with a high mortality. This happened even if the patients were in remission of the peritonitis. One possible reason was patients had lost their lean body mass and the nutritional markers was decreased significantly when they developed peritonitis. We also studied the predisposing factors for peritonitis and UF failure. Impairment of cognitive function and hand function were found to be associated with risk of touch contamination and peritonitis when they performed PD exchange. A full assessment of mental and hand function before training patients to self-perform PD exchange may reduce the incidence of peritonitis. Patients may develop UF failure after long-term PD treatment due to morphological changes of the peritoneal membrane (PM). Patients with DM and higher glucose exposure in the first year of PD treatment were found to have a risk for UF failure. For patients developed acute UF failure, apart from fluid compliance, retroperitoneal leakage was found to be the cause for this problem. In conclusion, the findings of the studies in this thesis help to identify the risk factors for mortality and complications of PD treatment.
