Der V. Band der allgemeinen und speziellen Operationslehre, die Oto-Rhino Laryngologischen Operationen, erscheint in diesen Jahren in der dritten Auflage. Er beginnt mit dem vorliegenden Band V/1, der die Operationen an der Nase und im Nasapharynx mit Berücksichtigung der transsphenoidalen Operationen an der Hypophyse und der Eingriffe am vegetativen Nervensystem des Kopfes abhandelt. Band V /2, der anschließend erscheint, wird die Operationen an den Nasennebenhöhlen und der angrenzenden Schädelbasis beinhalten. Band V/3 mit der Darstellung der oto-rhino-larnygologischen Operationen im Mund-und Halsbereich (DENECKE) sowie Band V /4, die allgemeinchirurgischen Operationen am Hals (SCHWEMMLE), sind 1980 erschienen. Band V /5 wird die Operationen am Ohr und der zugehörigen Schädelbasis enthalten. Diese Ausweitung des V. Bandes gegenüber seiner zweiten Auflage von 1953 wurde durch das Hinzukommen einer großen Anzahl neu entwickelter Operatio nen an den einzelnen Organen im Bereich von Hals, Nase und Ohr notwendig. Neue Erkenntnisse über die Pathophysiologie und die Funktion der Organe sind in die entwickelten Oprationstechniken eingeflossen. Dadurch wurde eine völlige Neubearbeitung des Stoffes erforderlich. Als Co-Autor für den vorliegenden Band V/1 konnte Professor WERNER EY, Darmstadt, gewonnen werden, der sich seit Jahrzehnten mit der Chirurgie der äußeren und inneren Nase besonders intensiv befaßt hat. Bei der korrigierenden Rhinoplastik und bei den rekonstruktiven Verfahren haben die Autoren versucht, die neuen Erkenntnisse über eine funktionelle Chir urgie unter Berücksichtigung der Pathophysiologie der Nase herauszuarbeiten.

Cancer of the head and neck continues to be a challenge. Increasing incidence has pushed malignancy of the upper aerodigestive tract into the first rank of cancer. In some countries it follows bronchial carcinoma in frequency and is more common than gastro-intestinal and gynaecological cancer. This increasing incidence makes it difficult to train enough highly specialised staffwho are also responsible for the care of many other patients, requiring sophisticated microsurgery of the ear and nose. The question of quality is even more difficult. Oncological success in the treat ment of head and neck cancer is bought at the price of crippling of vital functions such as eating, breathing, voice and sight and furthermore of striking aesthetic de formity. Mutilation of this highly functional collection of organs is more keenly felt than that of any other region of the body. It is vital, therefore, that the surgeon keeps up with the recent achievements of functional surgery in order to offer the best ser vice to his patients. Cancer of the larynx is no exception. Despite newer techniques of radiotherapy and chemotherapy, surgery still gives the best oncological results. This requires a wide spectrum of operations varying from minimal ablation to total laryngectomy.

With a great appreciation of history, Heinz Sarkowski has impressively reconstructed the company cor- spondence, which is fortunately almost completely preserved, and made it speak. * There is an inexhaustible amount of c- respondence pertaining to the period I have taken it upon myself to cover, and working through it properly not only would have required many years, but also would have detracted from the immediacy of the account. Thus, I decided to proceed from personal experience, to describe what has happened and to provide details gleaned from the correspondence. I have - counted here by no means only my own, but rather the personal experiences of the many company members and employees who are mentioned below. With the founding of the New York firm, developments branch out, becoming parallel but separate, and the change from one scene to another repeatedly interrupts the continuing course of events and the chronological flow of the report. In this connection, the occasional repetition of certain facts was - avoidable. In some places, however, it seemed more appropriate not to interrupt particular lines of development, but to describe them in continuity without regard to specific periods of time.

Seit der 1953 erschienenen 2. Auflage des "Ha1sbandes" dieser Operationslehre sind 27 Jahre ins Land gegangen. Jahre, in denen die Chirurgie nicht stehenge blieben ist. Der Wissenszuwachs und die Erweiterung der operativen Moglichkei ten druckt sich nicht zuletzt in einer Aufteilung des 5. Bandes auf vier Einzel bande aus. Teil 3, die oto-rhino-Iaryngologischen Operationen im Mund- und Halsbereich von H.J. Denecke erschien Ende letzten Jahres. Der vorliegende Teil 4 erganzt dieses Buch und setzt den fruheren Beitrag von N. Guleke mit einer neuen Bearbeitung der allgemein-chirurgischen Eingriffe am Hals fort. Neben den Standardoperationen haben wir fUr den interessierten Leser auch weniger gebrauchliche Eingriffe (lymphovenose Anastomose, Entfernung des Glomus caroticum) aufgenommen. Das einleitende Kapitel uber die chirurgische Anatomie des Halses hielten wir als Basis einer erfolgreichen chirurgischen Therapie fUr besonders wichtig. Woes notwendig erschien, wurden Klinik und Pathophysiologie der Erkrankungen in gebotener Kurze dargestellt. Den Herren Professoren V. Schlosser (Eingriffe an den Blutgefiil3en) und W. Wolfart (Media stinoskopie) mochte ich fUr ihre Mitarbeit herzlich danken. Einen Teil der ausgezeichneten Abbildungen aus der letzten Auflage konnten wir wieder verwenden. Vieles mul3te neu gestaltet werden, wofUr wir den Zeich nern, dem verstorbenen Hans Brandt und Albert Gattung, zu grol3em Dank verpflichtet sind. With improvements in respirator therapy and intensive care, congenital malformations and various acquired pathologic deformities of the trachea or bronchi are more often observed than used to be the case. For a while it seemed that tracheostomy would be unnecessary, but it has since become quite clear that severe disturbance of the trachea would be the outcome owing to primary or secondary pathologic changes that had not been given adequate consideration previous. These changes can lead to urgent life-threatening episodes or definite mutilation for the rest of the child's life. Tracheal surgery thus repre sents a new and special challenge for the pediatric surgeon. A solution to these serious problems must be found and merits discussion.
