The rapid expansion of the RAF during the mid/late 1930s necessitated a massive storage requirement for high explosive bombs and other ordinance. Drawing on the experience of the Great War ammunition factories, the authorities set about identifying underground sites around Britain safe from the threat of air attack. Unfortunately in the haste of the moment safety arrangements were not a high priority and, as a result, there were a number of appalling accidents; the most serious occurred in November 1944 when 3,800 tons of HE bombs detonated at the same time resulting in the deaths of 68 people in a Staffordshire quarry.

Death Underground: The Centralia and West Frankfort Mine Disasters examines two of the most devastating coal mine disasters in United States history since 1928. In two southern Illinois towns only forty miles apart, explosions killed 111 men at the Centralia No. 5 mine in 1947 and 119 men at the New Orient No. 2 mine in West Frankfort in 1951. Robert E. Hartley and David Kenney explain the causes of the accidents, identify who was to blame, and detail the emotional impact the disasters had on the survivors, their families, and their communities. Politics at the highest level of Illinois government played a critical role in the conditions that led to the accidents. Hartley and Kenney address how safety was compromised when inspection reports were widely ignored by state mining officials and mine company supervisors. Highlighted is the role of Driscoll Scanlan, a state inspector at Centralia, who warned of an impending disaster but whose political enemies shifted the blame to him, ruining his career. Hartley and Kenney also detail the New Orient No. 2 mine explosion, the attempts at rescue, and the resulting political spin circulated by labor, management, and the state bureaucracy. They outline the investigation, the subsequent hearings, and the efforts in Congress to legislate greater mine safety. Water inrush (WI) is one of the most dangerous geological disasters in underground engineering, with significant human casualties and economic losses. To prevent and control WI disasters, great efforts have been made to address WI mechanisms for more than half a century. In particular, the seepage instability theory is hitherto one of the most widely used theoretical models. This theory portrays that the seepage system will undergo structural instability when the initial values of permeability and boundary pressure meet certain conditions, which manifests that the permeability parameter is one of the most valuable indicators to unveil WI mechanisms. However, rock permeability is determined by its internal structural characteristics, which can be affected by water chemical composition, stress environment, and temperature. In this regard, it is of great necessity and importance to facilitate a better understanding of the holistic impacts of multi-field coupling on rock internal structure and deformation failure characteristics. This Research Topic aims to initiate a global forum for presenting and disseminating the latest advancements of WI mechanisms, which entails the characterization of physical and laboratory tests, 3D reconstruction of rock internal structure, numerical approaches, theoretical models under multi-field coupling, and filed date analysis methods.

Coal mine disasters in the United States are relatively rare events; many of the roughly 50,000 miners underground will never have to evacuate a mine in an emergency during their careers. However, for those that do, the consequences have the potential to be devastating. U.S. mine safety practices have received increased attention in recent years because of the highly publicized coal mine disasters in 2006 and 2010. Investigations have centered on understanding both how to prevent or mitigate emergencies and what capabilities are needed by miners to self-escape to a place of safety successfully. This report focuses on the latter - the preparations for self-escape. In the wake of 2006 disasters, the U.S. Congress passed the Mine Improvement and New Emergency Response Act of 2006 (MINER Act), which was designed to strengthen existing mine safety regulations and set forth new measures aimed at improving accident preparedness and emergency response in underground coal mines. Since that time, the efforts of the National Institute of Occupational Safety and Health (NIOSH) and the Mine Safety and Health Administration (MSHA) have contributed to safety improvements in the mining industry. However, the Upper Big Branch mine explosion in 2010 served as a reminder to remain ever vigilant on improving the prevention of mine disasters and preparations to help miners survive in the event of emergencies. This study was set in the context of human-systems integration (HSI), a systems approach that examines the interaction of people, tasks, and equipment and technology in the pursuit of a goal. It recognizes this interaction occurs within, and is influenced by, the broader environmental context. A key premise of human-systems integration is that much important information is lost when the various tasks within a system are considered individually or in isolation rather than in interaction with the whole system. Improving Self-Escape from Underground Coal Mines, the task of self-escape is part of the mine safety system.
