Krankenhäuser nehmen darin eine besondere Rolle wahr und stehen durch den zunehmenden Reformdruck auf Grund des medizinisch-technischen Fortschritts und des demographischen Wandels vor wachsenden Problemen. Trotz aller Maßnahmen zur Kostendämpfung sind die Ausgaben für Krankenhausbehandlungen aufgrund der starren Budgetierung und mangelnder Anreize zur Effizienzsteigerung gestiegen. Durch die Verpflichtung zur Entwicklung und Umsetzung eines flächendeckenden, leistungsorientierten Vergütungssystems für voll- und teilstationärer Leistungen, beabsichtigt der Gesetzgeber die Wirtschaftlichkeit und Transparenz des Gesundheitssystems zu verbessern. Die gesellschaftliche Frage nach der Ethik und die Betonung des Hauptziels des Gesundheitswesens „die Gesundheit der Versicherten zu erhalten, wiederherzustellen oder ihren Gesundheitszustand zu bessern“ scheint nicht mit den ökonomischen Ansprüchen vereinbar zu sein. Näher betrachtet stand die Notwendigkeit eines Systemwechsels unter Berücksichtigung des Kostendrucks und der Ressourcenknappheit aber außer Frage. Allerdings verbinden sich mit dieser grundlegenden und schnellen Veränderung viele komplexe und teilweise unbekannte Zusammenhänge. In dieser Ausarbeitung werden zunächst die DRGs im gesundheitsökonomischen Kontext dargestellt. Die Anforderungen an ein Vergütungssystem, sowie die gesetzlichen Rahmenbedingungen werden erläutert, bevor das bis zur Umstellung der Krankenhausfinanzierung bestehende System erklärt wird. Daraufhin wird der Prozess und die Problematik der DRG-Einführung in Deutschland beschrieben und ein Überblick über die Grundzüge des DRG-Systems verschafft. Hierbei wird insbesondere das Prinzip des Patientenklassifikationssystems skizziert und die Abrechnungsregeln sowie der Aufbau von DRGs veranschaulicht. Eine kurze Erläuterung des Kalkulationsverfahrens von DRGs führt dann in den Hauptteil. Darin werden die unterschiedlichen Auswirkungen des Systemwechsels eingehend untersucht. Neben dem Einfluss auf die Wettbewerbssituation und Handlungsoptionen der Krankenhäuser werden die Auswirkungen auf die Behandlungsqualität analysiert. [...]

Seit Anfang 2003 wird fast die gesamte stationäre Versorgung in deutschen Krankenhäusern nach DRGs (Diagnosis-related Groups) abgerechnet. Mit der neuen Vergütungsform waren Hoffnungen auf einen zügigeren und qualitativ optimierten Behandlungsverlauf bei gleichzeitigen Ausgabensenkungen verbunden. Kritiker der Umstellung befürchteten Nachteile für Patienten durch die Verkürzung der Verweildauern (bis hin zur 'blutigen Entlassung') und für Beschäftigte, etwa eine weitere Verdichtung der Arbeit.Ob diese Erwartungen eintrafen, beantwortet eine bisher einzigartige integrative Analyse, für die zwischen 2002 und 2008 tausende Krankenhaus-Patienten, Ärzte und Pflegekräfte mehrmals schriftlich mit fast identischen Fragen über ihre Wahrnehmung der Versorgungsqualität und Arbeitsbedingungen befragt wurden. Hinzu kommen Ergebnisse von qualitativen Fallstudien in vier Krankenhäusern. Ein Überblick über wesentliche Ergebnisse der nationalen und internationalen DRG-Folgenforschung rundet das Bild ab.Die Untersuchung führt unter anderem zu dem Ergebnis, dass viele der Befürchtungen noch nicht eingetroffen sind bzw. von den Betroffenen nicht wahrgenommen wurden. Trotzdem verschlechterte sich z.B. die Versorgung von Patienten mit mehreren Behandlungsanlässen, und es verschärfte sich drastisch die 'moralische Dissonanz' zwischen ethischen und professionellen Ansprüchen von Ärzten und Pflegekräften und ihrem Berufsalltag. Obwohl die Liegezeiten kontinuierlich weiter abnahmen, verbesserte sich die Aufnahme- und Entlassungsphase der Patienten auf zum Teil unzulänglichen Niveau nicht, was einer Verschlechterung gleichkommt.
