Clinical trials are the most definitive tool for evaluation of the applica bility of clinical investigations. The main objective of clinical investiga tions is to assess the potential value of a therapeutic entity in the treat ment or prophylaxis of a disease or a condition. It is also deemed necessary at this stage to obtain information regarding the undesirable side effects, associated risks, and their interrelationship with clinical assessments. Most of these clinical investigations conform, in some form or fashion, to the guidelines adopted by the Food and Drug Administration (FDA) for a given class of compounds. Clinical investigations in the past have not included specific studies in special or subpopulations, e.g., the elderly. Because of an ever-increasing elderly population, newer policies for clinical investigations are now being debated with the recognition of enhanced drug sensitivity in this special population. This key research activity can lead not only to improved health care in the elderly but also to control of its costs.

Reports in the popular press about the increasing longevity of Americans and the aging of the baby boom generation are constant reminders that the American population is becoming older. Consequently, an issue of growing medical, health policy, and social concern is the appropriate and rational use of medications by the elderly. Although becoming older does not necessarily correlate with increasing illness, aging is associated with anatomical and physiological changes that affect how medications are metabolized by the body. Furthermore, aging is often related to an increased frequency of chronic illness (often combined with multiple health problems) and an increased use of medications. Thus, a better understanding of the absorption, distribution, metabolism, and excretion of drugs; of the physiologic responses to those medications; as well as of the interactions among multiple medications is crucial for improving the health of older people.

There is a void in evidence-based information for making informed decisions on how to optimize care for older adults, particularly those 80 years and over. Because older adults are vastly underrepresented in clinical trials, there is a dearth of information about the appropriate use of drugs in this population. Yet older adults have higher rates of comorbidities and simultaneous use of multiple medications than the general population, and are the majority users of many medications. Additionally, age-related physiological and pathological changes, particularly for adults 80 years of age and older, can lead to significant differences in the pharmacokinetics (PK)2 and pharmacodynamics (PD)3 of a given drug compared to the general population. On August 5 and 6, 2020, the National Academies of Sciences, Engineering, and Medicine hosted a workshop titled Drug Research and Development for Adults Across the Older Age Span. The workshop was designed to examine the challenges and opportunities in drug research and development for older adult populations, explore barriers that impede safety and efficacy studies in these populations, and share lessons learned for better understanding clinical pharmacology for populations over age 65. This publication summarizes the presentation and discussion of the workshop.

The appropriate and rational use of drugs by the elderly is a matter of growing medical and social concern. Persons aged 65 years and older constitute about 12%-15% of the population in the Western world, and the total number of the elderly will increase significantly in the coming years. This population accounts for 30% of all the prescription drugs used. Aging, specifically the transition from middle to old age, is a complex process. From the perspective of clinical pharmacology, these pathophysiological changes may reasonably be expected to alter responsiveness to drugs. The age-related differences in response to drugs can arise from alterations in pharmacokinetics or pharmacodynamics. This makes it mandatory that clinical pharma cological studies be carried out in the elderly during extended phase I studies. The older the population likely to use the drug, the more important it is to include the very old. It is also important not to exclude .... :''lecessarily patients with concomitant illnesses; it is only by observing, such patients that drug-disease interactions can be detected. Reports from surveillance systems have greatly increased our awareness of problems associated with drug therapy in old age.

One of the greatest advances in Geriatric Medicine during the last quarter of a century has been the increased realization of the possibilities of treatment for elderly patients. Neglect has been replaced by a more optimistic therapeutic endeavour and countless old people have benefitted from this approach. But there is also a drawback, and this is the risk of hazardous side effects of medication which are often directly proportional to the biological potency of the drug and may be unpredictably increased due to changes in the senile organism. In fact the anatomical and biological changes in old age alter both the kinetics of most drugs and the receptor response. On account of these changes the individual tolerance of aged patients to drug therapy may be quite different from that of younger subjects. Thus for a variety of reasons elderly patients receive more drugs, but they are at a higher risk of encountering adverse reactions, which often show atypical clinical features. We can therefore speak of "geriatric iatrogenic disorders" and point out that some of these side effects are determined by a complex pathogenesis due to the particular pathophysiological condition in the elderly. It is important to encourage the habit of individ ually evaluating potential risk versus expected advantages of drugs in keeping with the same principles adopted in the evaluation of cost/benefit ratio.
