The care of pregnant women presents one of the paradoxes of modern medicine. Women usually require little medical intervention during an (uneventful) pregnancy. Conversely, those at high risk of damage to their own health or that of their unborn require the help of appropriate medicinal technology, including drugs. Accordingly, there are two classes of pregnant women, the larger group requires support but not much intervention, while the other needs the full range of diagnostic and therapeutic measures applied in any other branch of medicine. This book presents the current state of knowledge about drugs in pregnancy. In each chapter information is presented separately for two different aspects of the problem seeking a drug appropriate for prescription during pregnancy, and assessing the risk of a drug when exposure has already taken place. Practising clinicians who prescribe medicinal products to women who are, or who may become, pregnant, will find this volume an invaluable reference.

This book addresses methodological aspects of epidemiological studies on maternal drug use in pregnancy. Discussing the existing sources of error and how they can produce incorrect conclusions, it examines various epidemiological techniques and assesses their strengths and weaknesses. These refer both to the identification of outcomes (with special emphasis on congenital malformations) and to the types of exposure (drug use). Further, the book discusses the problem of confounding and how to handle it, and provides a simple introduction to statistics. Special situations, e.g. different types of parental exposure, are examined. Lastly, the book discusses pharmacovigilance and the information problem, concluding with a short list of aspects to consider when one wants to evaluate a published paper in the field. Though the book is primarily intended for pharmacologists, gynecologists and obstetricians, it will benefit all doctors working in perinatal care.

With the emergence of the biological sciences in the seventeenth and eighteenth centuries, some explanations of congenital malformations with a basis in scientific fact were formulated. It was not, however, until the thalidomide tragedy in 1961 that the necessity for further study of the effects of environmental factors on congenital malformations was recognized. By the early 1970s concentrated efforts had begun to be directed toward evaluating the effects on the unborn child of mood-altering drugs taken during pregnancy. Over the past decade, research and treatment programs have led to the recognition of a full range of psychoactive drugs that affect the developing fetus either somatically or behaviorally. Programs for the treatment of the pregnant drug addict have been developed which concentrate not only on the pharmacologic therapy of the woman but also on the social and psychologic characteristics that place her and her unborn child at risk. This book is an attempt to bring together clinicians and researchers who have been active in developing programs for the recognition and management of the chemically dependent pregnant woman and her newborn. The contributions of the multiple disciplines which the authors represent emphasize the need for a multifactorial approach to the problems of drug use and abuse during pregnancy. It is hoped that through this type of approach a better future for substance-exposed infants, innocent bystanders in the process of addiction, can be assured. Ira J. Fleshes out the story that is dominated by data concerning the effect of drugs on the unborn, by listening to pregnant or recently delivered women who take addictive drugs. Drawing on interviews with 120 such women, two sociologists explore such issues as how they decide whether or not to terminate their pregnancy, what their parents and family members think about the situation, and what options are available to them if they choose to keep the baby but kick the habit.
