Deininger and Mpuga use household level data for Uganda for 1999-2000 and 2002-03, before and after the abolition of user fees for public health services, to explore the effect of this policy on different groups' ability to access health services and morbidity outcomes. They find that the policy change improved access and reduced the probability of sickness in a way that was particularly beneficial to the poor. Although the challenge of maintaining service quality remains, aggregate benefits are estimated to be significantly larger than the estimated shortfalls from the abolition of user fees.This paper - a product of Rural Development, Development Research Group - is part of a larger effort in the group to assess the impact of public services provision on household welfare.

Despite large investments in determining the demand effects of raising health care user fees in developing countries, there has been little welfare analysis of user fees. Conventional wisdom assumes that the more that user fees discourage demand, the worse is a user-fee policy. This paper shows that this conventional wisdom is contradicted by neoclassical analysis. It is then argued that empirically resolving these conflicting predictions using standard consumer surplus measurement is inadequate. This is because market imperfections make individual private revealed values potentially different from actual social benefits. An alternative "health-valuation" approach to social welfare measurement is proposed here instead. This involves a reorientation of analyses towards direct measurement of health outcome effects, which is argued to be the preferred research strategy.

Abstract: As part of the recent health reform effort, the government of Georgia launched a Medical Assistance Program in June 2006 to provide health insurance to its poor population. So far the program covers slightly over 50 percent of the poor and provides benefit coverage for outpatient and inpatient care. This paper estimates initial impact of the Medical Assistance Program and assesses whether the benefits have reached the poorest among those eligible, using utilization data from June 2006 to December 2006. Based on the analysis using a regression discontinuity design and a three-part model, the paper presents two main findings. First, the Medical Assistance Program has significantly increased utilization of acute surgeries/inpatient services by the poor. Second, the benefits have successfully reached the poorest among the poor. These two findings indicate that government efforts to improve the poor's access to and utilization of health services are yielding results. The paper emphasizes that the initial dramatic increase in surgeries must be interpreted with caution, given the possible misclassification or misreporting of acute surgeries in the data. The paper also stresses the need to continue monitoring implementation of the Medical Assistance Program and further improve program design, particularly the targeting mechanism, to achieve better efficiency, effectiveness and overall equity in access to health care services.
