Excerpt from Edinburgh Medical Journal, Vol. 2: Combining the Monthly Journal of Medicine and the Edinburgh Medical and Surgical Journal The largest of these was about the size of a small walnut, and was connected with the cervix of a uterus that contained a small fibroid. These mucous polypi were all attached to some part of the cervix uteri, except one, about the size of a pea, which bled freely, and which was attacned to the mucous membrane of the body of the uterus. The great majority of them were easily felt by the examining finger. Some glandular polypi were so soft, glabrous, and mobile as to escape detection in a large vagina when the examiner was inexperienced. Some small ones were concealed within the cervix, whose external os required dilatation for their discovery. Sometimes little ones were hidden in the cervix behind a bigger one, which obstructed the passage of the finger to the more distant and smaller. On several occasions, these mucous polypi occurred as complications of pregnancy, of fibrous tumour, and of prolapsus. I have a specimen of a remarkable polypus, attached to the upper part of the cervix, which pursued an extraordinary course. Instead of growing downwards into the cervix and then into the vagina, it has grown upwards into the body of the uterus, which it distends; its bulk being that of a common boy's marble. The fibrous polypi were, like the mucous class, of various kinds. Among the sixty-six polypi already referred to, twenty-two were fibrous. Of these, one, nearly as large as an egg, with a thick pedicle, having a considerable artery pulsating in it, was a continuous outgrowth. It complicated pregnancy and gave rise to bloody discharge. It was attached to the cervix. A second smaller one of this kind occurred among the sixty-six. It complicated prolapsus uteri. Sixteen were true fibrous polypi; that is, they consisted of a fibrous tumour of the uterus, enclosed within a capsule of proper uterine muscular tissue, which generally was thin, sometimes absent at the most prominent part, and gradually thickened as the stalk was approached; the whole being invested in a layer of mucous membrane. In some of them, venous canals could be seen beneath the mucous membrane, and in some these canals seemed to open on the surface. In one profusely bleeding fibrous polypus, there were distinct venous openings on the most dependent part of the polypus, and further communication could be traced between these venous canals and a series of cavities, in the substance of the fibroid, which were filled with old fluid blood, more or less brown or decolorized. Another of these polypi, about the size of an egg, complicating pregnancy, but giving rise to no hmmorrhage, was attached to the margin of the os uteri by a large base. Excerpt from Edinburgh Medical Journal, 1890, Vol. 2 But though the rhythmic movements of the heart are neither initiated nor maintained by the nervous system, they are capable of being both modified and arrested through its agency. For the nervous system is supplied for the purpose of co-ordinating the action of the heart with the wants of the system. Without this co-ordination the heart would beat with the same unvarying monotony from the cradle to the grave, - neither exercise nor fever could quicken the pulse, nor could the tenderest emotion suffuse the cheek, the poetry of life would be lost, and life itself would soon follow, as no provision could be made for the requirements of the organs in their varying states of activity and quiescence. Unfortunately the penalty we pay for our present state of being is, that the very springs of life and of enjoyment may incidentally become the cause of much suffering, nay, even of death itself. The same agency by which a warm and comforting potion makes our heart beat faster, and sends a pleasant glow through our frame on some bitter day, cost Alexander the Great more men from drinking the icy waters of the Oxus than he lost in all the many battles of his Eastern campaign. Scattered throughout the substance of the heart, especially at the junction of the sinus venosus with the auricle, and in the auriculo-ventricular sulcus, there are many nervous ganglia which were formerly regarded as the source from which emanated the motor energy of the heart. Now that this idea has been given up, there is no satisfactory explanation of the function of these ganglia to be given; we cannot even with See regard them as reservoirs of energy, 2 for the energy dwells in the fibres of the heart, and is only regulated and modified through the agency of the nervous system. Over the surface of the heart, and particularly at its base, there is a network of nervous filaments connected at one extremity with these intra-cardiac ganglia, and at the other separating into three distinct nervous cords, each of which plays a special part in regulating the cardiac movements. One of these nerves comes from the sympathetic, and two from the cerebro-spinal system. From the sympathetic system, mainly through the last cervical and first dorsal ganglion, there passes to the heart an accelerating influence, which, according to Von Bezold, not only increases the rate, but also the force of the pulsations. Excerpt from Edinburgh Medical Journal, Vol. 21: Containing the Monthly Journal of Medicine and the Edinburgh Medical and Surgical Journal; Part 2, January to June, 1876 The physical examination of a patient with such a train of symptoms would discover some degree Of uterine enlargement, varying according to the Size of the morbid mass occupying its interior. It is rarely SO large as to form an abdominal tumour, ex cept where we have to do with a myo-sarcoma, or a fibroid tumour undergoing sarcomatous degeneration, when we may find a supra pubic mass of considerable dimensions, with the vascular bruit and other characteristics of such growths. Where the growth Springs from the cervix, or the uterus has become inverted, a polypoidal body will be felt occupying the vagina. The uterus, though eu larged, remains freely movable, unless the tumour be so great as to cause some impaction in the pelvis, or in ammatory adhesions have formed among the pelvic organs - conditions which are both comparatively rare in the true sarcoma. The sound will pass without difficulty into the interior of the uterus, and detect the extent of increase of the cavity and the presence of a foreign body in which bleeding is easily excited. But it is only when the canal is dilated with a tent - and, as I have Shown, the textures are SO soft and expansible that a Single good-sized sponge-tent may suffice for the dilatation - that the Size, consistence, and attach ments of the new growth can be discovered, and that a fragment of it can be Obtained for microscopical investigation. A woman, aged sixty, was gored by a cow five days before admission to hospital. The wound measured 5 inches by 2, and extended from the crest of the left ilium downwards. Omentum protruded, measuring 5 inches long, 3 inches broad, and 15 inches thick. It was flattened, had the shape of the tongue, looked exactly like a bit of pancreas, was of a brown colour, foetid, and adhered to the lips of the wound. There was no pain or fever, and the bowels acted normally. A ligature was applied. The mass separated, the wound healed kindly, and the old woman made a good recovery. Dr Greene remarks that, had the wound been in the epigastrium, he would have believed that the protruded mass was pancreas.
