This research project set out to identify those factors that are likely to lead to effective and meaningful collaboration among a broad range of stakeholders wishing to collaborate to improve health in rural communities. By studying two different collaborative efforts in rural Alaska that have succeeded in collaboration but have also faced many challenges, benefits of collaboration, challenges to collaboration, factors that contribute to benefits and challenges of collaboration, and important areas for development in collaboration were identified. Through the research study and a literature review conducted within the context of the researcher's professional experience, frameworks and tools were identified that can be used to help facilitate and support collaboration that is effective and meaningful in a community.

There is growing interest and investment in health-related collaboration in the United States. In an environment characterized by increasingly complex health problems, substantial resource constraints, and a fragmented health system, public and private organizations as well as communities are recognizing that most objectives related to health and health care cannot be achieved by any single person or organization working alone. Partnerships that bring together diverse people and organizations have the potential for developing new and creative ways of dealing with today's turbulent environment. Despite its potential advantages, collaboration also presents daunting challenges. Further, documenting the effectiveness of partnerships in improving health and well-being has been difficult. Given the significant difficulties of collaboration and the lack of evidence of its effectiveness, questioning whether the investment in health partnerships is justified seems reasonable. In this paper we address this question by illustrating the connective power of collaboration. We describe how collaboration, by connecting individual-level services, broadening community involvement in population-based health strategies, and linking individual-level services and population-based strategies, can improve the health of communities. We then discuss activities that could assist partnerships in reaching their collaborative potential and conclude by presenting the most compelling reasons for pursuing collaboration.

This is a report of a study with recommendations. Limited access to healthcare across the country is requiring stronger focus on prevention and population health will require better communications and more effective collaboration among hospitals (and their parent systems), public health departments, and other key parties in the health field. Previous studies have examined the nature and extent of collaboration between hospitals and public health departments. However, there is broad consensus that decades of limited communications, lack of mutual understanding, and incongruent goals have inhibited collaboration between hospitals and public health departments. Therefore, the overall purpose of this study is: To identify, compare, and contrast exceptional models of collaborative partnerships involving community hospitals, public health departments, and other stakeholders who share commitment to improving community health and determine the key lessons learned from their experience. The design of this study involves six principal phases: Phase 1. Identify Core Characteristics of Successful Collaborative Partnerships Phase 2. In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.
