ABSTRACT: Background: Published research addressing thoracic spine pain is less prevalent in comparison to cervical and lumbar pain. Because there are similar anatomic features between the lower cervical spine and upper thoracic spine, thoracic spine manipulation (TSM) is applicable when treating both regions. The purpose of this review was to determine the effectiveness of TSM in reducing pain and improving function in both the thoracic spine and the cervical spine due to the biomechanical link between the 2 regions. Our patient is a 26 year old female with a 7 year history of thoracic spine pain, cervical stiffness, and a history of herniated discs in the thoracic region, with the specific location and extent of the injury unknown. Methods: A comprehensive search was performed using the PubMed, MEDLINE, CINAHL, and PEDro databases. The search yielded 14 articles that met predefined inclusion and exclusion criteria. These articles were then appraised and annotated for quality and applicability to the clinical question. Articles were synthesized in order to make a final clinical decision. Results: A total of 7 high quality articles were selected for review, including 5 RCTs and 2 systematic reviews. Discussion/Conclusion: According to the literature, TSM is a beneficial intervention for patients with thoracic pain and cervical pain with immediate and short-term reductions in pain and disability. Clinicians should consider using TSM for patients with cervical spine pain and acute or subacute thoracic spine pain. TSM is not recommended as a first line of treatment for our patient due to the lack of literature investigating the use of TSM for patients with chronic thoracic spine pain.

This report presents results from the RAND study on the appropriateness of spinal manipulation and mobilization of the cervical spine. The study was designed to ascertain the clinical criteria for the appropriate use of cervical manipulation and mobilization to treat conditions such as neck pain and headache and to document treatment complications. A review was conducted of more than 500 articles from the medical and chiropractic literature. From this extensive review, a set of indications was created for manipulation and mobilization for neck pain and headaches and for subcategories of patient types. A panel was convened of back-pain experts from the disciplines of orthopedics, chiropractic, family medicine, and neurology to rate for appropriateness the indications for spinal manipulation and mobilization. The panelists rated the set of indications individually and then were convened as a group to rate the same indications following reporting and discussion of the individual ratings. This report presents the results of the final ratings and describes the methodology. It should be of interest to clinicians who perform manipulation and mobilization of the cervical spine, to clinicians who work with patients with cervical problems, and to health researchers and others concerned with the appropriate indications for performing manipulation and mobilization.

Neck pain is a common musculoskeletal disorder. It can be disabling, and constitutes a high health and economic burden. A large majority of neck pains are of mechanical origin. There has been a growing research interest on optimal interventions but there are no 'gold standards'. Spinal manipulation a form of manual therapy is often used in clinical practice to treat individuals with neck pain.However, in recent time manipulation applied directly to the cervical region has come under close scrutiny because of the potential risk of cervical arterial dysfunction. On the other hand, there is growing evidence for the use of thoracic spine manipulation in treating neck pain. Despite the relatively large number of randomized controlled trials carried on its effectiveness, the evidence of the effectiveness of thoracic manipulation in treating neck pain has been described as inconsistent and inconclusive. Therefore, this systematic review critically appraised, synthesized, and provides a clearer and more accurate overall assessment on the effectiveness of thoracic manipulation in reducing pain and disability in adults with mechanical neck pain.
