In today’s healthcare environment, the real driver is value. Maximizing outcomes and minimizing costs or providing similar outcomes with lower cost in terms of actual numbers or recovery time will dominate our healthcare delivery system. An excellent example of this evolution is in the ?eld of cardiovascular intervention. Dr. Jacques Kpodonu’s Manual of Thoracic Endo-Aortic Surgery is a most timely and descriptive work in this burgeoning ?eld. It provides an excellent overview of current technology and techniques, plus the endovascular expert can drill down into the details of the devices and technologies described and signi?cantly enhance their knowledge and perspective. Procedural detail, excellent graphics, pertinent imaging studies, and relevant instruction are the mainstays of this text. The ?rst chapter comprises the key overview and very comprehensively covers the platform – the hybrid OR, the tools, the accessories, and the medium – superb, state-of-the-art imaging modalities required to perform these advanced procedures. The text then moves into more technical details including options and instruction regarding access approaches and potential pitfalls of such. This is followed in en- ing chapters by dissemination of precise information, techniques, and indications for the most popular and utilized grafts including Boston Relay Stent Graft, Cook TX2 Zenith Stent Grafts, as well as the Gore and Medtronic Aortic Stent Grafts.

Open total arch replacement (TAR) remains the mainstay management strategy for thoracic aortic diseases involving the aortic arch. TAR evolved from the 2-stage conventional elephant trunk (CET) technique to the hybrid frozen elephant trunk (FET) which combined open surgical repair (OSR) with thoracic endovascular aortic repair (TEVAR) into a 1-stage procedure. Although FET has been able to achieve superior results to CET, including excellent survival, it still carries a risk of certain complications that may even require secondary reintervention. The era of elephant trunk is being overtaken by the new generation of TEVAR devices being used for total endovascular aortic arch (or endoarch) repair. Total endoarch repair (TER) is currently indicated in patients deemed high-risk for open surgery; however, it has shown strong potential for becoming the gold stand treatment for aortic arch pathologies. Despite the minimally-invasive nature of TER providing an obvious advantage over OSR in certain cases, TER remains associated with comparable mortality rates and key complications such as technical failure, neurological injury, need for reintervention, and loss of or failure to achieve target vessel patency. Upon comprehensively searching the literature, the technical success of TER ranged from 91%-100%, mortality 0%-19%, stroke 0%-16.7% and reintervention 0%-30.3%, using different commercially available endografts. Given its novelty, further studies with larger cohorts and longer follow-up periods are necessary to solidify the evidence on TER, taking into account the significant learning curve associated with TEVAR. In addition, studies directly comparing arch OSR to TER are warranted to determine superiority. This review aimed to highlight the evolution of aortic arch repair, focusing on TER device development, intervention criteria and clinical outcomes.

Aortic aneurysms are very silent, insidious, and sudden complicated pathologies with high incidence of sudden death. The aorta has several different parts, which are affected by aneurysmal pathology independently and separately or conjointly. Preventive medicine and determinative genetic heritage are the first steps during diagnosis and precaution. Preventive screening of the general population seems the best way for diagnosing early asymptomatic aneurysms with increased health cost. Because sudden death is the first symptom of aortic aneurysms, early interventional treatment is essential to protect fatal complications. In spite of open surgical repair which is the standard procedure, isolated or hybrid endovascular approaches are used widespread in the last decade due to reduction of procedural complications. This book is composed of the main topics on aortic aneurysms and surgical treatments to extend scientific and therapeutic perspective and vision of cardiac teams.
