Inhaltsangabe:Einleitung: Krankenhäuser mit geburtshilflichen Abteilungen sind Anbieter und Erbringer medizinischer Leistungen im Zusammenhang mit Schwangerschaft, Geburt und Wochenbett. Diese Leistungen werden unter komplexen Rahmenbedingungen, die dynamischen Veränderungen unterliegen und unter Wettbewerbsbedingungen erbracht. Wettbewerb zwischen geburtshilflichen Abteilungen um ausreichend viele Patientinnen besteht aufgrund der anhaltend sinkenden Geburtenzahlen seit mehr als zehn Jahren. Seit der verpflichtenden Umstellung der Vergütung stationärer Leistungen vom Selbstkostendeckungsprinzip auf das Fallpauschalensystem ab dem Jahr 2004 hat sich dieser Wettbewerb erweitert. Alle Leistungen sind seither auch wirtschaftlich - effizient zu erbringen. Die Betrachtung der gegenwärtigen und zukünftig zu erwartenden Veränderungen der Rahmenbedingungen zeigt die Notwendigkeit zur Erhöhung der Effizienz. Nur so stehen die zum Erhalt der Wettbewerbsposition notwendigen finanziellen Mittel in ausreichendem Umfang und zum richtigen Zeitpunkt zur Verfügung. Übliche Maßnahmen zur Erhöhung der Effizienz sind die Reduktion der Sachkosten und des Personals. Auf diese Weise ist die Effizienz oft nur kurzfristig zu erhöhen. Kurz-, mittel- und langfristig wirksame Maßnahmen werden durch den häufig bestehenden Mangel an Prozessorientierung zu wenig fokussiert und genutzt. Vielfach fehlt es auch an einer individuellen Handlungsgrundlage, die ein systematisches, kontinuierliches Agieren innerhalb der Komplexität der Rahmenbedingungen und im Wettbewerb ermöglicht. Hier setzt diese Arbeit an. Sie beschreibt die praxisorientierte Entwicklung eines Konzepts zur Erhöhung der Effizienz für geburtshilfliche Abteilungen. Mit einem individuell entwickelten Konzept steht eine solche Handlungsgrundlage zur Verfügung. Die Praxisorientierung bezieht sich auf die Berücksichtigung der Rahmenbedingungen und der heterogenen Ausgangslagen der Abteilungen. Diese und theoretische Grundlagen sind Thema der ersten beiden Kapitel. Das dritte Kapitel beschreibt schrittweise die Konzeptentwicklung. Es zeigt vielfältige Maßnahmen zur kurz-, mittel- und langfristigen Erhöhung der Effizienz auf mehreren Ebenen im Prozess der Leistungserbringung. Die Umsetzung einer der Maßnahmen des Konzepts in die Praxis ist Inhalt des vierten Kapitels. Die Arbeit endet mit der Zusammenfassung und einem Schlusswort.
