In the late 1980s, the National Cancer Institute initiated an investigation of cancer risks in populations near 52 commercial nuclear power plants and 10 Department of Energy nuclear facilities (including research and nuclear weapons production facilities and one reprocessing plant) in the United States. The results of the NCI investigation were used a primary resource for communicating with the public about the cancer risks near the nuclear facilities. However, this study is now over 20 years old. The U.S. Nuclear Regulatory Commission requested that the National Academy of Sciences provide an updated assessment of cancer risks in populations near USNRC-licensed nuclear facilities that utilize or process uranium for the production of electricity. Analysis of Cancer Risks in Populations near Nuclear Facilities: Phase 1 focuses on identifying scientifically sound approaches for carrying out an assessment of cancer risks associated with living near a nuclear facility, judgments about the strengths and weaknesses of various statistical power, ability to assess potential confounding factors, possible biases, and required effort. The results from this Phase 1 study will be used to inform the design of cancer risk assessment, which will be carried out in Phase 2. This report is beneficial for the general public, communities near nuclear facilities, stakeholders, healthcare providers, policy makers, state and local officials, community leaders, and the media.

This, the eighth monograph in the Smoking and Tobacco Control series published by the National Cancer Institute (NCI), is in many respects also the most significant. Contained in this volume are new results from five of the world's largest prospective epidemiological studies defining the magnitude of disease risks caused by cigarette smoking. Thirty years ago, in January 1966, NCI published a similar monograph titled Epidemiological Approaches to the Study of Cancer and Other Chronic Diseases. The report of the Surgeon General's Advisory Committee on Smoking and Health had been released in 1964 and had relied extensively on data from prospective mortality studies to delineate the relationship between cigarette smoking and various chronic diseases. The 1966 NCI monograph provided a detailed examination of the outcomes of several of the large prospective mortality studies presented in the 1964 advisory committee report. At that time, the outcomes available from these studies were based on 3 to 6 years of follow up; with the exception of the American Cancer Society's (ACS) Cancer Prevention Study I (CPS-I), studies in the 1966 NCI monograph did not include substantial numbers of females. This monograph includes three new prospective mortality studies (CPS-II, the Nurses' Health Study, and the Kaiser Permanente Prospective Mortality study, provides the outcomes of the CPS-I study after 12 years of follow up, and provides 26 years of follow up of the study of U.S. veterans. Data from these studies provide the most comprehensive description of the disease consequences produced by smoking available to date and are accompanied by a detailed description of the changes in smoking behaviors of the U.S. population over the past century. Prospective mortality studies continue to play a critical role in quantifying the relative mortality risks of smoking for the individual as well as in estimating the overall disease burden caused by cigarette smoking in our society. The goal of this monograph is to facilitate both these tasks by providing, in one volume, comprehensive descriptions of smoking behaviors and the disease risks that result from those behaviors.

Over the past two decades increasing interest has emerged in the contribu tions that the social sciences might make to the epidemiological study of patterns of health and disease. Several reasons can be cited for this increasing interest. Primary among these has been the rise of the chronic, non-infectious diseases as important causes of morbidity and mortality within Western populations during the 20th century. Generally speaking, the chronic, non infectious diseases are strongly influenced by lifestyle variables, which are themselves strongly influenced by social and cultural forces. The under standing of the effects of the behavioral factors in, say, hypertension, thus requires an understanding of the social and cultural factors which encourage obesity, a sedentary lifestyle, non-compliance with anti-hypertensive medica tions (or other prescribed regimens), and stress. Equally, there is a growing awareness that considerations of human behavior and its social and cultural determinants are important for understanding the distribution and control of infectious diseases. Related to this expansion of epidemiologic interest into the behavioral realm 'has been the development of etiological models which focus on the psychological, biological and socio-cultural characteristics of hosts, rather than exclusive concern with exposure to a particular agent or even behavioral risk. Also during this period advances in statistical and computing techniques have made accessible the ready testing of multivariate causal models, and so have encouraged the measurement of the effects of social and cultural factors on disease occurrence.

Life course epidemiology is concerned with the origins of risk, resilience, and the processes of ageing, and how this information can be of value in a public health context - particularly for preventive health care. Its challenge is to discover, develop and analyse sources of data that cover many years of life, especially the early developmental period when, it is thought, some fundamental aspects of lifetime health begin. It also analyses genetic propensity and environmentalexposures.The rapid development of life course epidemiology, in parallel with new work on developmental biology and the biology of ageing, has bought innovative and ingenious methods of data collection. These require new methodological techniques for the design of observational and quasi-experimental studies of life course pathways to adult health. This book describes these developments, together with arguments for improving the measurement of the social environment and its role in developing individualvulnerability or adaptation. The development of bio-bank large-scale population studies for the investigation of genetic effects is discussed, alongside the challenges this creates for the epidemiologist. The changing design of studies, increasing flow of longitudinal data, management of data,analytic challenges, timing, and both traditional and more recent methods of managing these features in the study of causality, are discussed.Life course epidemiology has an essential role in developing methods to evaluate precisely the impact of interacting developmental, environmental, and genetic effects, knowledge of which is fundamental for the design of effective prevention strategies in public health, as well as for the advancement of understanding in the broader spheres of health and medicine.
