Like many people with epilepsy and ID, John has comorbid behavior problems. This association is dealt with in detail later in the book. A clear and consistent approach to this is likely to offer much to John in terms of his quality of life. Other psychosocial concerns will need addressing. Emphasizing and explaining the impact of epilepsy and its treatment on John’s cognitive well-being can help reduce anxiety in caregivers and the family and help establish the importance of treatment. Informing people about the negative impact of seizures while trying to minimize the difficulties in social life imposed by people’s fears of seizures can be a difficult challenge. This is most difficult when discussing seizure-related death. However, our position must be to inform patients and families so they can make their life and treatment choices with complete understanding of choices and risks. Delivering a Package of Care Delivering continuity of health care will be a major need throughout John’s lifespan. This continuity should cover seizure assessment and management yet also be responsive to changes in the care environment. A person with ID who is no longer resident in the family home is likely to undergo many changes in staffing over a lifetime. As for many people, their ability to communicate their health needs will be compromised, and these changes in staff can offer considerable risks in chronic disease management.

This significant volume provides broad coverage of the spectrum of problems confronted by patients with developmental disabilities and the many kinds of occupational therapy services these individuals need. Experts identify exemplary institutional and community service programs for treating patients with autism, cerebral palsy, epilepsy, and mental retardation. A welcome contribution to the meager professional literature on the subject, Developmental Disabilities: A Handbook for Occupational Therapists will be an enormously helpful resource for therapists who work with both children and adults, ranging from mild to severe levels of impairment. You will learn how to establish a therapeutic environment for children with autism, develop a pre-vocational program in a pediatric skilled care facility, use qualitative research to obtain insight into the world of adults with significantly limiting cerebral palsy, and provide early intervention for your developmentally disabled patients.

Brain disordersâ€"neurological, psychiatric, and developmentalâ€"now affect at least 250 million people in the developing world, and this number is expected to rise as life expectancy increases. Yet public and private health systems in developing countries have paid relatively little attention to brain disorders. The negative attitudes, prejudice, and stigma that often surround many of these disorders have contributed to this neglect. Lacking proper diagnosis and treatment, millions of individual lives are lost to disability and death. Such conditions exact both personal and economic costs on families, communities, and nations. The report describes the causes and risk factors associated with brain disorders. It focuses on six representative brain disorders that are prevalent in developing countries: developmental disabilities, epilepsy, schizophrenia, bipolar disorder, depression, and stroke. The report makes detailed recommendations of ways to reduce the toll exacted by these six disorders. In broader strokes, the report also proposes six major strategies toward reducing the overall burden of brain disorders in the developing world.

How does a developmental disability affect an individual throughout the course of life? What impact does the disability have on the individual's family? What strengths do families use to cope with these disabilities? What do they do that works? And, what doesn't work? These are the kinds of questions we have been asking individuals and families in our research over the past 15 years. This book was written to report their stories, and to honor these people who have shared their lives and their cries from the heart with us. It is both a positive book and a realistic book: full of love and grief and tenderness and anger and kindness and sorrow and courage. It is as real as the people who gave us the gift of their lives.

The latest in our successful series of epilepsy edited by this experienced Scandinavian group addressed the clinically challenging area of epilepsy in mentally retarded patients. While epilepsy and intellectual disability are frequently comorbid (each may precipitate the other and both can be a consequence of brain damage), their elucidation can be complex; there is a high risk of misdiagnosis due to the occurence in mentally retarded patients of paroxysmal phenomena which resemble epileptic seizures, and the presentation of other psychiatric problems can obscure symptoms of epilepsy. Treatment too, is problematic with a tendency towards polytherapy with its increased risk of undesirable side effects which are less easily discovered and poorly tolerated in mentally retarded patients.Such patients have in the past usually been institutionalized but they are now more often in ambulatory or primary care, There is thus a greater than ever need to recognize the particular difficulties involved withtheir care. This book clarifies interrelationships between mental retardation, intellectual deterioration, and epilepsy and delivers practical solutions to the problems associated with accurate diagnosis, treatment, and long term management of epilepsy in patients with intellectual disabilities.
