Over 12 million people with epilepsy have seizures that cannot be controlled by antiepileptic drugs. The term "drug-resistance" is abundantly used in the epilepsy literature but the definitions proposed differ considerably. It is used indifferently in a number of different settings: as a criterion for selection of patients eligible for new antiepileptic drug trials, for the selection of surgical candidates, for the design of epidemiological studies, for the design of studies on quality of life, for the definition of the epileptic encephalopathies (in comparison to more benign epilepsy syndromes), to mention but a few. As a result, available studies are usually not comparable and referral to epilepsy specialists is unacceptably delayed. Epilepsy is one of the most common neurological chronic disorders characterized by unpredictable recurrent seizures, which leads to brain dysfunction, cognitive, psychological, and social disorders. Various treatment options for epilepsy are: antiepileptic drugs, surgery, neuromodulation, gene therapy and ketogenic diet. However, definitions, diagnosis and treatment of epilepsy are still challenging issues for clinicians and researchers. Antiepileptic drugs play a central and crucial role in the treatment of epilepsy patients. Brivaracetam Cannabidiol (CBD), Cenobamate, Everolimus and Fenfluramine are the five novel anticonvulsant drugs during the last 5 years. Brivaracetam anticonvulsant effects are due to its highly selective affinity for synaptic vesicle protein 2A (SV2A) in the brain. It is act as an SV2A ligand. Among these drugs Cannabidiol is one of the prominent phytocannabinoids derived from the cannabis sativa. The anticonvulsant mechanisms of action of CBD including antagonism of G protein-coupled receptor 55 (GPR55), desensitization of transient receptor potential vanilloid type 1 (TRPV1) channels, and potentiation of adenosine-mediated signaling through inhibition of the equilibrative nucleoside transporter 1 (ENT-1) and enhancement of GABAergic transmission. The mechanisms of the antiseizure effect of Cenobamate are: increasing the inactivated state of voltage-gated sodium channels, and acting as a positive allosteric modulator of the GABA-A receptors by binding at a non-benzodiazepine site. Everolimus is a mammalian target of rapamycin (mTOR) inhibitor. Fenfluramine is able to enhance extracellular serotonin levels, modulate serotonergic and other neurologic receptors. Nevertheless, the exact mechanism of action of most of these drugs is unknown. At least 30% of people with epilepsy have drug-resistant epilepsy (DRE), therefore, non-pharmacological treatments have been considered. the Vagus Nerve Stimulation (VNS), Trigeminal Nerve Stimulation (TNS), Transcranial Direct Current Stimulation (tDCS), Transcranial Magnetic Stimulation (TMS) are the most common neuromodulation-based approaches. Also, in recent years, a new field of GABA-, adenosine-, galanin-, and neuropeptide Y- based cell and gene therapies for refractory epilepsy has emerged. In spite of these advances, significant gaps in knowledge still remain. In particular, the mechanisms underlying epilepsy, including status epilepticus, drug and surgical treatments are still not well elucidated. Novel insights on pathophysiological mechanisms of epilepsy and new treatment options prompted us to collect the most recent publications on pharmacological and non-pharmacological treatments of epilepsy. So, in this Research Topic, we will focus on the advances in the field of treatment in epilepsy, the pathophysiology and molecular aspects of seizure attacks and novel antiseizure medications.

Over 15 million people worldwide suffer from localization-related drug-resistant epilepsy. These patients are candidates for targeted surgical therapies such as surgical resection, laser thermal ablation, and neurostimulation. While seizure localization is needed prior to surgical intervention, this process is challenging, invasive, and often inconclusive. In this work, I aim to exploit the power of multimodal high-resolution imaging and intracranial electroencephalography (iEEG) data to map seizure networks in drug-resistant epilepsy patients, with a focus on minimizing invasiveness. Given compelling evidence that epilepsy is a disease of distorted brain networks as opposed to well-defined focal lesions, I employ a graph-theoretical approach to map structural and functional brain networks and identify putative targets for removal. The first section focuses on mesial temporal lobe epilepsy (TLE), the most common type of localization-related epilepsy. Using high-resolution structural and functional 7T MRI, I demonstrate that noninvasive neuroimaging-based network properties within the medial temporal lobe can serve as useful biomarkers for TLE cases in which conventional imaging and volumetric analysis are insufficient. The second section expands to all forms of localization-related epilepsy. Using iEEG recordings, I provide a framework for the utility of interictal network synchrony in identifying candidate resection zones, with the goal of reducing the need for prolonged invasive implants. In the third section, I generate a pipeline for integrated analysis of iEEG and MRI networks, paving the way for future large-scale studies that can effectively harness synergy between different modalities. This multimodal approach has the potential to provide fundamental insights into the pathology of an epileptic brain, robustly identify areas of seizure onset and spread, and ultimately inform clinical decision making.
