E. coli O157:H7, since its discovery as a human foodborne pathogen in 1982, has been recognized as a disease agent causing hemorrhagic colitis, hemolytic uremic syndrome and thrombotic thrombocytopenia purpura. Since E. coli O157:H7 is a more environmentally sensitive organism than other E. coli strains, and since foods containing low numbers of cells have caused illness, there have been difficulties in isolating and identifying the organism from food. The proceedings of this workshop document the present methods to detect E. coli O157:H7 and other verotoxigenic organisms in food. Data gathered from surveys of animals at the slaughterhouse, retail meats and ready-to-eat foods in the US and Canada are also presented. In addition, plans for further methodology research, including collaborative studies; and recommendations concerning the best procedures for detection of these pathogens are outlined.

Verocytotxin producing Escherichia coli (VTEC), and in particular, strains of serogroup o157, have emerged as significant pathogens causing a range of severe and potentially fatal illnesses. The European Union has recognised the threat posed by E. coli O157:H7 and the need to devise control strategies based on an understanding of VTEC pathogenicity, transmission, survival and growth. It also acknowledges the imortance of informing farmers, veterinarians, food producers and health authorities so that each of these groups can act appropriately to reduce the overall hazards posed by these organisms. To contribute to the development and dissemination of effect control strategies, the European Commission funded a Concerted Action Project "A European study on animal, food, and biomedical aspects of verocytotoxigenic E. coli including serotype O157:H7, an emerging pathogen" (CT98-3935) within the Agriculture and Agro-industry Framework IV Research Programme (1998-2001). This book, compiled under the auspices of the above project, integrates contributions from project participants and invited contributor, to provide a comprehensive overview of the current state of research on VTEC. USDA's Food Safety and Inspection Service (FSIS) is formulating risk assessments to identify important foodborne hazards; evaluate potential strategies to prevent, reduce, or eliminate those hazards; assess the effects of different mitigation strategies; and identify research needs. These risk assessments, in brief, empirically characterize the determinants of the presence or level of microbial contamination in vulnerable foodstuffs at various points leading up to consumption. One of the initial efforts in the undertaking is a risk assessment of the public health impact of E. coli O157:H7 in ground beef. In addition to soliciting public input, FSIS asked the Institute of Medicine (IOM) to convene a committee of experts to review the draft and offer recommendations and suggestions for consideration as the agency finalizes the document. This report presents the results of that review.

This Brief will review the methods that are currently available for the detection, isolation, and typing of pathogenic E. coli with a particular focus on foodborne diseases caused by the Shiga toxigenic E. coli group, which have been implicated in a number of significant outbreaks in recent years. Pathogenic forms of E. coli can cause a variety of diarrheal diseases in hosts due to the presence of specific colonization and virulence factors, and pathogenicity-associated genes, which are generally not present in other E. coli. Six pathotypes of pathogenic E. coli are recognized (Shiga toxigenic E. coli, Enteropathogenic E. coli, Enterotoxigenic E. coli, Enteroinvasive E. coli, Enteroaggregative E. coli and Diffusely Adherent E. coli) and certain strains among these groups are major public health concerns due to the severity of disease that they can cause. Methods to detect and isolate these pathogens from a variety of sources are constantly evolving. In addition, the accumulation of knowledge on these pathogens allows for improved intervention strategies.

E. coli serotype O157: H7 is a rare variety of E. coli, a normal inhabitant of the intestines of all animals, including humans. The pathogen produces large quantities of one or more related potent toxins, called Shiga toxins, which cause severe damage to the lining of the intestine and to other target organs, such as the kidneys. E. coli O157: H7 was first recognized as a cause of illness in 1982 during an outbreak of severe bloody diarrhea that was later traced to contaminated hamburgers. It has since been implicated in a number of outbreaks of intestinal distress. The most severe outcome in the general population is typically hemorrhagic colitis, a prominent symptom of which is bloody diarrhea. Some victims, particularly the very young, may develop hemolytic uremic syndrome. Eating meat, particularly ground beef, that has not been cooked sufficiently to kill E. coli O157: H7 is thought to be the primary cause of infection. Among other known sources of infection are consumption of contaminated sprouts, lettuce, salami, and unpasteurized milk and fruit juice; swimming in or drinking contaminated water; and contact with the stools of infected animals or people
