Introducción. La obesidad infantil es un problema de salud cada vez más frecuente. Su importancia reside en su persistencia en etapas posteriores de la vida y su relación con entidades patológicas, la enfermedad cardiovascular y diabetes tipo 2. Objetivos. Conocer la evolución de la obesidad y el sobrepeso y factores de riesgo cardiovascular (FRCV) en una cohorte de niños seguidos desde los 2 hasta los 18 a 19 años de edad, en una consulta de pediatría de Atención Primaria. Pacientes y métodos. Se realizó un estudio longitudinal prospectivo de cohorte. Y longitudinal retrospectivo para riesgos. La población está constituída por los niños seguidos en las consultas de pediatría de un Centro de Salud desde los 2 hasta los 18 años de edad. Se mide Índice de Masa Corporal (IMC) desde los 2 a los 18 años. Se mide tensión arterial y parámetros bioquímicos en sangre: glucosa, transaminasas, colesterol total (CT), y unido a lipoproteínas de baja (LDL) y alta densidad (HDL), triglicéridos (TGD), insulina y HOMA a los 18 años de edad. Resultados. Corresponden a 153 personas de 18 años de edad. Presentaron sobrepeso un 18,3% (26,56% en varones y 12,35% de mujeres) y obesidad 7,18% (6,25 % de los varones y 7,86% de mujeres). Presentaron hipertensión el 7,8%; 5,2%, elevación de triglicéridos y 17% disminución de HDL. Un 3,4% presentan cifras de colesterol superiores a 200 mg/dL. No se registró ninguna cifra de glucemia elevada. Un 4,2% presentó una insulinemia superior a 20 mcU/mL, índice de HOMA superior a 3,8 un 5,08% y superior a 3, un 14,4%. Un 4,13% cumple criterios de síndrome metabólico. Un 72,7% de las personas obesas presentó algún FRCV, 19,3% de las personas con normopeso y un 31,1% en el grupo de sobrepeso. Refieren antecedentes familiares de FRCV en un 53,9% de los casos. Conclusiones. Obesidad y sobrepeso son patologías frecuentes en nuestro medio al inicio de la edad adulta. Los niños que sufren obesidad a partir de los 6 años presentan riesgo de presentar obesidad y FRCV al inicio de la edad adulta.
