Winner of a HIGHLY COMMENDED AWARD in the Neurology category of the 2012 BMA Medical Book Competition. More than five million people die from stroke each year. Stroke is also the leading cause of adult disability, affecting over 20 million people worldwide, with two-thirds of stroke victims left permanently disabled. We now know that speed of intervention is critical and patient outcomes are determined by a very narrow therapeutic window, during which physicians need to be able to make their choice of treatment rapidly, based on the best evidence available. The purpose of this book is to review that evidence, for both medical and surgical therapies as well as preventative strategies, drawing on the expertise of leading international authorities to distinguish the issues for which there is a consensus on best practice from those for which the evidence remains inconclusive. For example, for patients with acute ischemic stroke the first choice is intravenous thrombolytic therapy, but does a treatment window of 0-3 hours deliver outcomes significantly better than one of 3-4.5 hours? Arterial hypertension is the single most important modifiable risk factor for ischemic stroke, but what are the effects of blood pressure reduction among acute stroke patients? How effective is blood pressure lowering in primary intracerebral hemorrhage? What are the arguments in favour of continuation versus temporary discontinuation of antihypertensive therapy immediately after acute ischemic stroke? Endovascular procedures (carotid angioplasty/stenting) are increasingly important but what do the data tell us about their safety? Or durability? How do they compare with carotid endarterectomy? The management of unruptured intracranial aneurysms is rife with controversies; clip, coil, wait and watch? The authors examine the evidence for and against. Other sections examine the impact of various risk factors, and special populations. A section on stroke after cardiovascular surgeries reviews the incidence and management of stroke following a whole range of procedures. Another examines special risks during pregnancy. Lastly, the book reviews our understanding of childhood stroke and presents current best practice recommendations for this most challenging group of patients.

Stroke is the leading cause of disability and the third leading cause of death in the developed world. The past decade has witnessed a paradigm shift in the management of stroke, with the understanding that care of acute stroke patients by specialists working in dedicated stroke units greatly improves patient outcomes; as a consequence, stroke units now exist in all major hospitals in the UK and Europe. Interventions to treat stroke have also increased in complexity, and the discipline of stroke medicine is now recognised as a medical specialty. This book is a concise, accessible and authoritative source of relevant and focussed information about stroke disease. It contains a thorough review of the management of cerebrovascular disease - everything you need to function effectively on an acute stroke unit. Tables and diagrams aid quick reference and easy comprehension. The most up-to-date and clinically relevant resource on the market, Stroke in Practice equips all medical professionals with evidence-based, practical knowledge and a comprehensive understanding of treatment of stroke. 'This text is directed at the non-specialist and emphasises practicality over academic niceties. If it inspires enthusiasm for a fascinating condition and convinces the reader that stroke is preventable, treatable and yet potentially devastating for patients and their families, it will justify its existence.' from the foreword by Kennedy R Lees

Clinical Pathways In Stroke Rehabilitation written by Thomas Platz and has been published by Springer Nature this book supported file pdf, txt, epub, kindle and other format this book has been release on 2021-01-14 with Medical categories.

This open access book focuses on practical clinical problems that are frequently encountered in stroke rehabilitation. Consequences of diseases, e.g. impairments and activity limitations, are addressed in rehabilitation with the overall goal to reduce disability and promote participation. Based on the available best external evidence, clinical pathways are described for stroke rehabilitation bridging the gap between clinical evidence and clinical decision-making. The clinical pathways answer the questions which rehabilitation treatment options are beneficial to overcome specific impairment constellations and activity limitations and are well acceptable to stroke survivors, as well as when and in which settings to provide rehabilitation over the course of recovery post stroke. Each chapter starts with a description of the clinical problem encountered. This is followed by a systematic, but concise review of the evidence (RCTs, systematic reviews and meta-analyses) that is relevant for clinical decision-making, and comments on assessment, therapy (training, technology, medication), and the use of technical aids as appropriate. Based on these summaries, clinical algorithms / pathways are provided and the main clinical-decision situations are portrayed. The book is invaluable for all neurorehabilitation team members, clinicians, nurses, and therapists in neurology, physical medicine and rehabilitation, and related fields. It is a World Federation for NeuroRehabilitation (WFNR) educational initiative, bridging the gap between the rapidly expanding clinical research in stroke rehabilitation and clinical practice across societies and continents. It can be used for both clinical decision-making for individuals and as well as clinical background knowledge for stroke rehabilitation service development initiatives.

Each chapter will review current evidence-based practice guiding clinical stroke care. The book will address the American Stroke Association's Stroke Chain of Survival addressing prehospital care of the stroke patient and the development of stroke systems of care to provide all people in the United States access to acute stroke care. Additionally, the book will cover the current role of state legislation in stroke care and the evolution of hospital stroke certification. The book will serve as a clinical resource providing detailed comprehensive medical and nursing care of all stroke subtypes while also addressing the system of stroke care in which medical, nursing and interprofessional care provided. As such, the book will serve as a clinical resource to medical and nursing caregivers providing direct patient care as well as stroke coordinators, program directors and other hospital administrators developing stroke programs. The book will also be a clinical resource for stroke interprofessional team members such as physical therapists, occupational therapists, Stroke care is most successful at improving patient outcomes when delivered by an interdisciplinary team. Each chapter will address the critical role of the interprofessional team and highlight comprehensive care of the stroke patient rather than focusing only on nursing care. Books published to date focus solely on the medical or nursing care of the stroke patient without attention to the stroke system of care and the role of the multidisciplinary team in improving stroke outcomes. Additionally, each chapter will highlight ongoing research trials and opportunities, with the recognition that the scientific foundation for acute stroke care is rapidly evolving.

This focused book deals clearly and concisely with the principles of management of patients with acute ischemic stroke (AIS). It emphasizes the published and verifiable evidence in support of these principles, and highlights the areas of limited evidence. Best evidence is provided for the current standard treatment of acute ischemic stroke, including intravenous chemical thrombolysis, intra-arterial approaches, blood pressure management, and anti-thrombotic therapy. More modern techniques are also described, such as the use of mechanical devices to evacuate a thrombus, induced hypertension, hyper-oxygenation (hyperoxia), and neuroprotective or neuroregeneration agents. Hallmark features include: Focused on acute ischemic stroke, the most rapidly growing area of stroke management Evidence based: Provides best evidence for treatment Clearly discusses areas and topics where best evidence has not yet been established Lays out treatment plans and protocols in handbook fashion for ease of use and reference, even in emergency situations Includes latest and cutting-edge techniques and equipment for dealing with AIS Acute Ischemic Stroke: An Evidence-based Approach is an indispensable reference work for neurologists, neurocritical care medics, intensivists, hospitalists, emergency room physicians, neurology residents and fellows. It is also a helpful resource for medical students, physician assistants, and nurse practitioners in hospital and neurocritical care facilities.
