The evidence review for dietary guidance is the first step in establishing the Myanmar food based dietary guidelines (FBDGs) for pregnant and lactating women and children under 5 years old. The review process followed a systematic approach to gather, assess and analyse data and information related to food, diet and nutrient intakes of the Myanmar people, agricultural production and food supply, food sustainability, cultural food preference and behaviour, prevalence of malnutrition and diet-related diseases. The evidence review provides justifications for making FBDG recommendations based on up-to-date and country-specific information, as well as the global evidence base. The evidence review increases the transparency of the evidence-based process in establishing the Myanmar FBDGs. Moreover, The evidence review for dietary guidance is the first structured evidence review to guide FBDGs development in Myanmar.

This report presents the outcomes of a scoping review conducted to identify and map existing evidence on the effects of school food and nutrition policies on health-related outcomes in children of school age as part of the initial preparation for undertaking the guideline development process by the WHO Nutrition Guidance Expert Advisory Group (NUGAG) Subgroup on Policy Actions. In order to align with WHO’s Nutrition-friendly Schools Initiative (NFSI) framework, school-based food and nutrition interventions were assessed in terms of the impacts in four key policy areas, namely – the school community, the school curriculum, the school food and nutrition environment, and school nutrition and health services.

What foods should Americans eat to promote their health, and in what amounts? What is the scientific evidence that supports specific recommendations for dietary intake to reduce the risk of multifactorial chronic disease? These questions are critically important because dietary intake has been recognized to have a role as a key determinant of health. As the primary federal source of consistent, evidence-based information on dietary practices for optimal nutrition, the Dietary Guidelines for Americans (DGA) have the promise to empower Americans to make informed decisions about what and how much they eat to improve health and reduce the risk of chronic disease. The adoption and widespread translation of the DGA requires that they be universally viewed as valid, evidence-based, and free of bias and conflicts of interest to the extent possible. However, this has not routinely been the case. A first short report meant to inform the 2020 review cycle explored how the advisory committee selection process can be improved to provide more transparency, eliminate bias, and include committee members with a range of viewpoints. This second and final report recommends changes to the DGA process to reduce and manage sources of bias and conflicts of interest, improve timely opportunities for engagement by all interested parties, enhance transparency, and strengthen the science base of the process.

Federal guidance on nutrition and diet is intended to reflect the state of the science and deliver the most reliable recommendations possible according to the best available evidence. This guidance, updated and presented every 5 years in the Dietary Guidelines for Americans (DGA), serves as the basis for all federal nutrition policies and nutrition assistance programs, as well as nutrition education programs. Despite the use of the guidelines over the past 30 years, recent challenges prompted Congress to question the process by which food and nutrition guidance is developed. This report assesses the process used to develop the guidelines; it does not evaluate the substance or use of the guidelines. As part of an overall, comprehensive review of the process to update the DGA, this first report seeks to discover how the advisory committee selection process can be improved to provide more transparency, eliminate bias, and include committee members with a range of viewpoints for the purpose of informing the 2020 cycle.

The Office of Dietary Supplements requested the Tufts Medical Center Evidence-based Practice Center (EPC) to conduct an exercise to identify the issues and challenges of including evidence-based methods as a component of the process used to develop nutrient reference values (such as the Dietary Reference Intakes [DRI]) issued by the Institute of Medicine (IOM). This work was performed under a task order issued by the Agency for Healthcare Research and Quality EPC program. The Tufts EPC assembled a group of nutrition experts from academic institutions and relevant federal government agencies, led participants in teleconferences and meetings, conducted exercises in formulating questions that would be amenable to systematic reviews of the scientific literature, and identified the challenges and limitations of applying this method to processes previously used to establish nutrient reference values. This report summarizes the impetus behind this project, approach taken, and the lessons learned. Nutrient reference values have significant public health and policy implications. This type of dietary guidance is needed for planning diets, assessing the adequacy of diets in individuals and populations, developing nutrition education and guidance, and for setting reference values for nutrition labeling. The IOM Food and Nutrition Board has issued reports on the DRIs for a wide range of nutrients. Six reports have been published and are organized around groups of nutrients. Contents of the reports include a summary of what is known about the nutrient function in the human body, selection of indicators of adequacy of nutrient intakes or nutrient levels, factors that may affect how the nutrients are utilized and that affect requirements, and how nutrients may be related to the prevention of chronic disease across age groups. Various study committees were convened to evaluate a body of available scientific evidence for specific nutrients. Primarily human studies were reviewed and selected animal studies were used when human data are absent or conflicting. Available evidence was weighted according to quality, peer review status, biological plausibility, and whether similar estimates would be derived from different indicators. However, the process of establishing DRIs has been variable and has evolved as experience accrued from study committees. Concern has been expressed that in some cases the methods used to determine DRIs have suffered from a lack of transparency and consistency. Moreover, differences in the reference values derived by various groups of nutrition experts worldwide have been noted for the same nutrient when all presumably have used the same body of available evidence. Given the importance of defining reliable nutrient reference values, there is a need for an explicit, objective, and transparent process to set these values. Evaluating evidence is a major component of informing the process. Because new studies are constantly being published, it would also be desirable to have a framework that allows efficient updating when new information becomes available. Although the mandate to different committees around the world has previously been different, with the globalization of the food supply and health risks it should be expected that evaluating similar evidence will result in similar recommendations. Over the past 15 years, the concept of evidence-based medicine building upon the foundation of systematic reviews, meta-analyses and related methods as important tools for evidence-based practice, has gained widespread acceptance in the evaluation of medical evidence for healthcare decision-making. The application of this approach to evaluating the nutrition literature could provide for the transparent, comprehensive and objective evaluation of scientific evidence and could provide support for a framework for a consistent approach to establishing nutrient reference values for all dietary components.

Diseases associated with dietary excess and imbalance rank among the leading causes of illness and death in the United States. Major diseases in which diet plays a role include coronary heart disease, some types of cancer, stroke, hypertension, obesity, osteoporosis, and non-insulin dependent diabetes mellitus. All are major causes of morbidity and mortality in this country. Although diet is associated with multiple health outcomes, the relationships between specific dietary elements and specific health outcomes have been studied extensively. The role of the primary care provider in either providing direct diet counseling or enlisting the help of other health professionals has been studied extensively, but controversy remains about the effectiveness of different strategies. In .Evaluating Primary Care Behavioral Counseling Interventions: An Evidence-based Approach, . Whitlock et al. described a detailed framework for primary care counseling. To address the question of the role of diet in chronic disease as well as dietary assessment and counseling in primary care, staff of the RTI . University of North Carolina Evidence-based Practice Center undertook this systematic evidence review (SER) on behalf of the US Preventive Services Task Force (USPSTF). It updates the chapter on dietary counseling from the second edition of the Guide to Clinical Preventive Services. In 1996, the USPSTF had recommended counseling adults and children over 2 years of age to limit intakes of fat and cholesterol, to maintain caloric balance in diets, and to emphasize foods containing fiber; the Task Force concluded then that the evidence was insufficient to recommend for or against counseling the general population to reduce dietary sodium. The Task Force also concluded that evidence was insufficient to show that nutritional counseling by physicians has any advantage over counseling by dietitians or community interventions. This SER enabled the USPSTF to reconsider the issues it previously addressed and to make recommendations concerning ways to promote healthy dietary practices in America. To guide the work more precisely, we identified 7 key questions: 1. What is the relationship between dietary patterns and health outcomes? 2. What are valid, feasible tools for assessment of dietary risk? 3. What are the adverse effects of dietary assessment? 4. What is the efficacy of primary care counseling and dietary behavior change interventions? 5. What are the adverse effects and associated costs of dietary behavior intervention? 6. Which of the following system influences facilitate or impede dietary intervention: features of the health care team? features of the practice setting? features of the health care system? 7. Can dietary supplements improve nutrition in patients identified as undernourished

However, Ethiopia currently lacks this important public health tool. This study seeks to propose key diet and physical activity recommendations through soliciting experts’ perspectives and critical review of relevant documents. A qualitative study was conducted in Addis Ababa city among key informants. Experts from eight organizations and institutions having interest in nutrition and health issues were purposively selected. Using a topic guide, data were generated through face-to-face in depth interviews with 10 experts. The interviews were audio-recorded, transcribed and thematic analysis was performed. To complement with the interview of key informants, reviews of selected global as well as national documents were carried out, and the data generated thereof were summarized. Diet and health are closely related and nutrient deficiencies, diet–related chronic diseases as well as their risk factors are rampant, while food security remains a concern. Diet and nutrition literacy is said to be low, in Ethiopia. World-wide, about one-third of countries (five in Africa) have dietary guidelines, whose core messages are reasonably similar. Cognizant of the prevailing situation, some national strategic documents and interviewed experts are in favor of placement of dietary guidelines. The dietary habits and lifestyles of the populace are sub-optimal and they need to be improved timely through multi-sectorial efforts that include nutrition information and education, based on appropriate dietary guidelines.

Leading nutrition organizations are using systematic reviews (SRs) to develop evidence-based nutrition and research agendas, revise dietary guidelines, formulate public health policies and support dietetic practice guidelines with the goal of improving patient outcomes and practitioner effectiveness. The Office of Dietary Supplements (ODS) in collaboration with other institutes and centers of the National Institutes of Health (NIH) uses SRs to identify research needs and set research priorities. In 2001, the American Dietetic Association (ADA) began carrying out SRs on a wide range of nutrition-related diseases. Evidence-based guidelines are being developed to provide an additional tool for food and nutrition professionals to apply the best research results to their practice, with the goal of improving patient outcomes and practitioner effectiveness. In addition, the Food and Drug Administration (FDA) has developed a draft guidance document of an evidence-based review system to evaluate publicly available scientific evidence for health claims on food and supplement products. The complexity of relationships between nutrition and health and the lack of widely accepted guidance on how to address nutrition issues have impeded the transfer of evidence-based methodologies from medicine to the field of nutrition. While the concepts and methods of evidence-based medicine can be applied to nutrition questions, there are important differences between evaluations of drug therapies and nutrient-related health outcomes. For SRs of medical interventions, there exist checklists to improve SR reporting quality (i.e., clarity and transparent reporting of SR methods and results) such as MOOSE (Meta-analysis of Observational Studies in Epidemiology) and QUOROM (Quality Of Reporting Of Meta-analyses). While these represent consensus guidelines to improve the quality of SRs in general, they do not provide guidance for reporting or analyses of variables unique to the field of nutrition. Standardized guidance for researchers conducting SRs on nutrition-related topics could benefit the users of these reviews. Our aim was to examine the reporting quality of existing SRs linking micronutrients and health outcomes, and identify areas for improvement. We also performed exploratory analyses to evaluate factors that may be associated with reporting quality, such as the designs of primary studies (interventional versus observational studies), years of publications, methods of evidence syntheses (meta-analyses or qualitative synthesis), and impact factors of journals that published these reviews.
