The results of this study indicated that people with NSCLBP have a range of environmental and exercise preferences that are important to consider in exercise prescription. Enablers for exercise participation included shared decision making, effective communication, a history of exercise ability and participation, familiarity with exercise environments and the fitness culture, the option of individualised or supervised programs, use of preferred environments, family support, variety and fun, motivating strategies and education. Barriers included time constraints, cost, boring programs, symptom aggravation, consequences of stigma, dissatisfaction with formal exercises and distaste of gym "culture". The qualitative research results informed the development of a questionnaire that clinicians can use to explore participant exercise preferences and inform exercise prescription. Monash University SCERH granted approval for an RCT that compares an exercise rehabilitation program informed by a participant preferences questionnaire compared to a program without this guideline for patients with NSCLBP (CF08/2015 - 2008000990). The protocol has been registered and published and pilot work to refine the trial methods is underway. The qualitative study will evaluate clinician and participant experience. In the final phase of this research, focus groups were conducted to explore the views and practices of physiotherapists who prescribe exercise for people with NSCLBP, how they determine exercise prescription, and what they perceive as enablers and barriers to participation in prescribed exercise programs. Clinicians described a dilemma created by diagnostic uncertainty and problems gaining patient confidence in the therapeutic encounter. Ongoing research addresses the potential advantages associated with improving clinician skills in non-judgemental and reflective listening, and providing effective support in the face of resistance to lifestyle changes.
