Purpose: To estimate additional ECMO-related costs in adults using the hospital perspective and to determine the proportion of ECMO-related costs in the total cost of the stay. Methods: We performed a retrospective study using our institutional ECMO database considering four main ECMO indications: acute respiratory distress syndrome (ARDS), post- cardiotomy low cardiac output syndrome (PC-LCOS), extracorporeal cardio pulmonary resuscitation (ECPR) and cardiogenic shock (CS). We then prospectively microcosted ECMO implantation and decannulation and used expert advice concerning ECMO daily management to determine additional ECMO-related costs. Total cost of stay was calculated for each indication as the sum of the average cost of stay, estimated using the French national cost database, and additional ECMO-related costs. Results: From 422 hospital stays and 19 microcosted ECMO procedures, we estimated a mean additional ECMO-related cost of €7,421 (min-max; €4,553 - €14,828). Estimated mean total cost of hospital stay for a patient supported by ECMO was €45,305 ± €39,006. Overall, additional ECMO-related costs accounted for 27.2 ± 19.1 % of total hospital stay costs and were highly variable across different ECMO indications. Conclusion: In a large single-center costing study combining gross-costing and prospective microcosting approach, additional ECMO-related costs were high and accounted for a substantial proportion of total hospital stay, with important variations across different ECMO indications. Together, these results advocate for the individualization of ECMO within current costing and funding strategies in ICU.
