This book considers the family doctor relationship and the process of ending that relationship. What happens when a family doctor or someone like them, deeply committed to long-term relationships, decides to end those commitments? What’s involved? What are the embodied experiences for doctor and patient, for doctor and staff, for physician leader and others? What comes next? This book invites the reader to immerse in personal stories and reflections of family physicians who choose to retire from practice, depart long-standing leadership roles, or shift from one place of deep relational commitments to something else. These stories concern the particulars of family medicine and general practice, but they share much with any vocation rooted in the duties, challenges, and rewards of relationships bound by covenant and not transaction. This book is relevant to all professionals involved in healing relationships.

With family doctors increasingly overburdened, bureaucratized, and burned out, how can the field change before it's too late? Over the past few decades, as American medical practice has become increasingly specialized, the number of generalists—doctors who care for the whole person—has plummeted. On paper, family medicine sounds noble; in practice, though, the field is so demanding in scope and substance, and the health system so favorable to specialists, that it cannot be fulfilled by most doctors. In Searching for the Family Doctor, Timothy J. Hoff weaves together the early history of the family practice specialty in the United States with the personal narratives of modern-day family doctors. By formalizing this area of practice and instituting specialist-level training requirements, the originators of family practice hoped to increase respect for generalists, improve the pipeline of young medical graduates choosing primary care, and, in so doing, have a major positive impact on the way patients receive care. Drawing on in-depth interviews with fifty-five family doctors, Hoff shows us how these medical professionals have had their calling transformed not only by the indifferent acts of an unsupportive health care system but by the hand of their own medical specialty—a specialty that has chosen to pursue short- over long-term viability, conformity over uniqueness, and protectionism over collaboration. A specialty unable to innovate to keep its membership cohesive and focused on fulfilling the generalist ideal. The family doctor, Hoff explains, was conceived of as a powered-up version of the "country doctor" idea. At a time when doctor-patient relationships are evaporating in the face of highly transactional, fast-food-style medical practice, this ideal seems both nostalgic and revolutionary. However, the realities of highly bureaucratic reimbursement and quality-of-care requirements, educational debt, and ongoing consolidation of the old-fashioned independent doctor's office into corporate health systems have stacked the deck against the altruists and true believers who are drawn to the profession of family practice. As more family doctors wind up working for big health care corporations, their career paths grow more parochial, balkanizing the specialty. Their work roles and professional identities are increasingly niche-oriented. Exploring how to save primary care by giving family doctors a fighting chance to become the generalists we need in our lives, Searching for the Family Doctor is required reading for anyone interested in the troubled state of modern medicine.

Daniel is twenty-one years old. He is entering his senior year in college as a pre-med student when he has a tragic diving accident, rendering him a quadriplegic. Though critical care can keep him alive, doctors are unable to stop the continuing deterioration of his condition. He is ventilator dependent, has frequent cardiac arrests, blood clots, fevers, and severe medical problems. Slowly, day by day, Daniel loses more and more ground. He eventually learns that he will never be able to eat or drink, never be able to speak, never be off the ventilator. With the help and support of his family, Daniel decides to remove the ventilator, thus quickly and effectively ending his life. Saying Goodbye to Daniel is the heartrending account of the final two and a half months in the life of Daniel Rothman, told with beautiful simplicity by his mother. As a social worker and philosopher specializing in terminal care issues, Juliet Rothman had for many years been preparing herself unwittingly for the hardest choice a parent can ever offer a child.Saying Goodbye to Daniel is many things. It is a harrowing story of how one family faced the ongoing tragedy of months of hospitalization and acute medical care. It is the moving case study of a young man's death with dignity. It is also a compassionate guidebook that includes an update on how the family coped with the loss of their son and brother, emotionally and spiritually, two years after his death; Juliet's reflections on terminating-care decisions based on her professional and practical experience; and a list of resources dealing with spinal cord injury, loss of a child, grief and bereavement, and advance directives and patient's rights.

Psychosocial Oncology And Palliative Care In Hong Kong written by Richard Fielding and has been published by Hong Kong University Press this book supported file pdf, txt, epub, kindle and other format this book has been release on 1999-11-01 with Medical categories.

As the evidence-base for clinical practice in the management of life-threatening diseases and care at the end of life increases, it is apparent that psychosocial factors play a most profound role, influencing outcomes at every level from quality of life and satisfaction with clinical services through to duration of survival and mortality. This book documents some, but by no means all, of the developments that have occurred in the past decade in the area of psychosocial oncology and palliative care in Hong Kong. Contributions describing interventions by practitioners involved in service development in nursing, social work and clinical psychology, are complemented by chapters describing academic research and theoretical perspectives. The unique cultural mix of Hong Kong is given rich emphasis in the adaptations made by practitioners and academics to the interventions and theoretical issues outlined. As both a documentation of the efforts of some of those who helped psychosocial oncology and palliative care evolve in Hong Kong, and as a reflection of the need to more critically evaluate the impact of intervention efforts in health care, this volume provides a valuable resource. Nurses, social workers, psychologists and doctors involved in delivering or planning cancer treatment or palliative care will find this book useful. This book challenges many attitudes prevalent in Hong Kong and will, we hope, begin to break some of the taboos that continue to generate unnecessary suffering among the people cared for by our health care systems. The valuable experience documented in these pages can help others build the next generation of services to those with life-threatening illness and those at the end of life.
