La anestesiología es una de las especialidades más completas dentro de la práctica de la medicina, ya que permite al especialista actuar con base en el conocimiento y el razonamiento de las bases anatómicas y fisiológicas, aunadas al pleno dominio de la farmacología y sus aplicaciones en la anestesiología, permitiendo tomar decisiones para una correcta selección de técnicas invasivas en el paciente con el propósito de ofrecer una serie de procedimientos perioperatorios de calidad y seguridad en cualquier escenario clínico y en cualquier instancia que se requiera. Es evidente el aporte científico de finales del siglo pasado y el inicio del actual, enfocado a la biología molecular, específicamente al sistema nervioso, que pretende desenmarañar mediante el estudio y el descubrimiento de un gran número de neurotransmisores, así como una diversidad de mecanismos de acción con diferentes sistemas intracelulares y extracelulares involucrados en el estado anestésico, considerado tradicionalmente como hipnosis, analgesia e inmovilidad ante una noxa específica. Estos conceptos, con base en la medicina basada en evidencias, se han ido modificado en la actualidad con tendencia a demostrar la importancia de alcanzar y mantener concentraciones de un fármaco en uno o varios sitios efectores y/o zonas target, vía plasma, para alcanzar objetivos específicos que modifican la neurotransmisión celular, responsables de la respuesta clínica durante un evento anestésico. El dominio de la farmacología aplicada en anestesiología es fundamental para que cualquier médico anestesiólogo proceda a realizar la adecuada elección, asociación e interacción de fármacos para generar la respuesta clínica deseada, procurando minimizar la incidencia y la intensidad de efectos adversos inherentes al manejo de este tipo de fármacos, al titular los agentes anestésicos dentro de una ventana terapéutica. La sinergia y los efectos aditivos que resultan de la polifarmacia durante un evento anestésico, necesarios y suficientes para producir un estado anestésico adecuado en un tiempo determinado, se basan en el correcto razonamiento de una dosificación segura y confiable, así como en la titulación de la misma durante el evento quirúrgico, diagnóstico y/o terapéutico individualizado para el paciente, para cualquier escenario clínico y ante todo tipo de situación. Interactuar con los receptores específicos y modificar la respuesta farmacodinámica son factores que deben ser tomados en cuenta para que la decisión al implementar una estrategia farmacológica teórica, basada en la cinética del fármaco, involucre una respuesta clínica que permita cuantificar la intensidad de esa estrategia, con el objetivo de alcanzar y manejar las ventanas terapéuticas de cada uno de los fármacos requeridos para minimizar los eventos de infradosificación o sobredosificación, todo esto con base en una relación entre concentración plasmática y concentración sitio efecto, permitiendo una respuesta clínica efectiva y a la vez prevenir o eliminar efectos adversos inherentes a la combinación de varios fármacos en cada paciente.

There is a large influx of trauma, general surgical, pediatric, neurosurgical and obstetric emergencies as the hospital is one of the major public hospitals located in a densely populated region sitting amidst major routes in the city. It also provides services for emergency cardiovascular, cerebrovascular interventions and cadaver transplants. Daily lessons from this tremendous emergency work inspired us to bring out this book. There is plethora of reference material available in basic sciences related to anesthesiology, namely physiology, pharmacology, physics and anatomy. Similarly, a large number of textbooks are on techniques such as general, regional, monitored anesthesia care as well as super specialties like cardiac, neurosurgical and pediatric anesthesia, etc. But there is a limited availability of books and references for emergency anesthesia. One has to scan and integrate parts from various sources. This book is an attempt to assimilate the scattered information and add clinical expertise. Contents of this book have been divided into nine sections to cover majority of the emergencies. General considerations for adults, obstetric and pediatric sections have been written separately to avoid duplication. An additional challenge is cases with various medical disorders for emergency surgeries. Efforts have been taken to accommodate them in first section and obstetric section. The contributors of this book work together at LTMMC and LTMGH and have a well-balanced team with fresh talent bringing the latest technological knowledge and tenured practitioners supplementing it with applied experience. Part of the team worked on collection of the matter from different sources for an ideal scenario, while the senior faculty members used their vast clinical experience for a more practical and realistic application. These combined efforts have been truly beneficial to achieve our goal to rationally mix latest technology with clinical experience and identify potential ways to conduct anesthesia with best results, at least costs and minimum risk.
