One of the most valuable root-cause analysis tools in the system safety toolbox is fault tree analysis (FTA). A fault tree (FT) is a graphical diagram that uses logic gates to model the various combinations of failures, faults, errors and normal events involved in causing a specified undesired event to occur. The graphical model can be translated into a mathematical model in order to compute failure probabilities and system importance measures. A FT can model all aspects of a system, including hardware, software, human actions and the environment. FTs are employed to evaluate large complex and dynamic systems, in order to understand and prevent potential safety and reliability problems. Using the rigorous and structured methodology of FT construction allows the systems analyst to model the unique combinations of fault events that can cause an UE to occur. Given that for centuries, the standard tool to understand diseases in tissues was the microscope and that its major limitation was that only excised tissue could be used, recent technology now permits the examination of diseased tissue in vivo. Optical coherence tomography (OCT) has promising potential when applied to coronary artery disease. OCT has the capability to identify coronary plaque and to distinguish between plaques that are stable and unstable. The objective of this research was to develop reliability-based design methods for reinforced protective concrete structures. The analysis and design methods in the Air Force Protective Construction Design Manual (PCDM) provided the nominal (deterministic) methods that were used to perform probabilistic response analyses and the reliability assessments. One of the key objectives of this effort was to develop simplified rehability-based design methods, in the form of reliability-based design factors (RBDFs), that can be used by designers who are unfamiliar with probabilistic methods. Research was conducted in five fundamental areas: (1) airblast and aboveground structure response; (2) groundshock and belowground structure response; (3) fragmentation effects; (4) projectile penetration; (5) protective structure systems reliability. In each of the areas, emphasis was on reinforced concrete structures; however, the load uncertainty analyses are applicable to structures of any materials. In s instances we found significant biases in the PCDM models. In those cases where we found significant biases that could not be corrected without changes to the fundamental approach, RBDFs were not developed.
