Die Anreizmechanismen im deutschen Krankenversicherungssystem begünstigen auf Grund ihrer Ausgestaltung opportunistisches Verhalten der Akteure im Gesundheitswesen und verhindern in diesem Zusammenhang eine optimale Allokation der Ressourcen im deutschen Krankenversicherungssystem. Bestehende Mechanismen zur Eindämmung der identifizierten Fehlanreize werden auf ihre Ausgestaltung und damit verbundene Effektivität hin analysiert. Es werden Lösungsansätze dargestellt, wie die vorhandenen Konzepte zur Eindämmung der Fehlanreize sinnvoller umgesetzt werden können und / oder alternative Konzepte präsentiert. Relevante Entwicklungen im Gesundheitssektor werden mit dem Fokus auf die Finanzierung eines zukunftsfähigen Gesundheitssystems untersucht. Die Hauptakteure in Form von Leistungserbringer, Leistungsempfänger und Leistungsfinanzierer werden bezüglich ihrer Verhaltensanreize, der Beziehungen untereinander und der Potenziale zur Ausschöpfung von Effizienzreserven kritisch durchleuchtet.

Bestehende Mechanismen zur Eindämmung der identifizierten Fehlanreize werden auf ihre Ausgestaltung und damit verbundene Effektivität hin analysiert. Es werden Lösungsansätze dargestellt, wie die vorhandenen Konzepte zur Eindämmung der Fehlanreize sinnvoller umgesetzt werden können und / oder alternative Konzepte präsentiert. Zu Beginn wird in Kapitel 2 die Grundstruktur des Krankenversicherungssystems in Deutschland in Form der zwei Versicherungszweige gesetzliche Krankenversicherung und private Krankenversicherung dargestellt. Aus volkswirtschaftlicher Sicht ist die wirtschaftliche Gesundheitsversorgung der Gesamtbevölkerung von besonderem Interesse, so dass sich die vorliegende Arbeit in der Analyse auf die gesetzliche Krankenversicherung, die 87 % der Versicherten in Deutschland versorgt, beschränkt. Kapitel 3 gibt eine Antwort auf die Frage, welche für den Gesundheitssektor relevanten Entwicklungen eine Handlungsnotwendigkeit, vor dem Hintergrund der Finanzierung eines zukunftsfähigen Gesundheitssystems, begründen. Hier werden die Rolle des demographischen Wandels und der Einfluss des medizinisch-technischen Fortschritts analysiert. Darauf aufbauend werden in Kapitel 4 die Hauptakteure des Gesundheitswesens dahingehend analysiert, welche Verhaltensanreize im deutschen Krankenversicherungssystem für den jeweiligen Akteur bestehen, wie sich dieses Verhalten in der Beziehung mit anderen Akteuren äußert und welches Potenzial in Hinblick auf eine wirtschaftlichere Ausgestaltung des Krankenversicherungssystems besteht. Im Rahmen der allgemeinen Diskussion um die Kostenentwicklung im Gesundheitssektor stellen Fragen der Wirtschaftlichkeit im Krankenhaussektor seit langer Zeit ein zentrales Thema dar. Vor diesem Hintergrund lässt sich bei den Veränderungen in den Rahmenbedingungen von Krankenhausbetrieben in Form der Krankenhausgesetzgebung eine zunehmende Tendenz zu einer marktorientierten Steuerung feststellen: Die Einführung eines leistungsorientierten und pauschalierten Vergütungssystems auf der Basis von Diagnosis Related Groups (DRG) ab dem Jahre 2004 wird für die Leistungserbringer einen erhöhten Kosten- und Wettbewerbsdruck bedeuten. Die vorliegende Arbeit greift diesen Aspekt als Ausgangspunkt für ein Gesamtkonzept der marktorientierten Steuerung im Krankenhaussektor auf. Dabei werden die durch die Reform angestoßenen Aus- und Wechselwirkungen auf das Krankenhausfinanzierungs- und Planungssystem (Steuerung von Krankenhäusern) diskutiert. Außerdem werden Anpassungsnotwendigkeiten für die betriebliche Steuerung der Leistungsprozesse in Krankenhäusern (Steuerung in Krankenhäuser) theoretisch und empirisch analysiert.
