Today, our food systems are making us ill, driving climatic change and undermining the health of ecosystems. This new ‘Food Systems Delivering Better Health’narrative recognizes that food lies at the heart of human, ecosystem and animal health. We must change the way we think about, produce, distribute, consume, dispose of and value food, for better health outcomes. The narrative focuses on five interconnected and interrelated pathways: unhealthy diets and food insecurity; zoonotic pathogens and antimicrobial resistance; unsafe and adulterated foods; environmental contamination and degradation and occupational hazards. With the new narrative the World Health Organization aims to reach decision makers, policy makers, practitioners, investors and researchers across the food system to guide policy and practice, for transformative systems change to shift food systems towards the delivery of sustainable healthy diets for all. We must harness momentum and engagement to transform our food systems to better health and well-being of our planet and all its inhabitants.

WHO is promoting the implementation of a package of game changing food systems actions, that complement each other and focus on improving the nutritional quality of food along the food supply chain and creating healthier food environments. Today’s food systems are simply failing to deliver healthy diets for all. If food systems are transformed, they can become a powerful driving force towards ending hunger, food insecurity and malnutrition in all its forms. WHO is promoting this package of actions also in the context of the UN Food Systems Summit, the Tokyo Nutrition for Growth Summit 2021, as well as for the uptake of the CFS Voluntary Guidelines on Food Systems and Nutrition. The package of actions comprises of taxing unhealthy options and subsidising healthy options, protecting children from harmful marketing of food and beverages, ensuring clear and accurate nutrition labels, reducing the levels of salt, sugars and harmful fats in processed food and drink products, fortifying staple foods by adding extra vitamins and minerals, ensuring nutritious foods for healthier diets are served or sold in schools, hospitals and other public places, improving surveillance of and response to foodborne diseases and assessing and strengthening national food control systems. This information brief outlines for each of these actions what it is about and why it is important, with reference to WHO guidance. Moreover, country case studies are presented and examples of SMART commitments for interested countries.

Considering the detrimental environmental impact of current food systems, and the concerns raised about their sustainability, there is an urgent need to promote diets that are healthy and have low environmental impacts. These diets also need to be socio-culturally acceptable and economically accessible for all. Acknowledging the existence of diverging views on the concepts of sustainable diets and healthy diets, countries have requested guidance from the Food and Agriculture Organization of the United Nations (FAO) and the World Health Organization (WHO) on what constitutes sustainable healthy diets. These guiding principles take a holistic approach to diets; they consider international nutrition recommendations; the environmental cost of food production and consumption; and the adaptability to local social, cultural and economic contexts. Malnutrition in all its forms is a major challenge everywhere in the world, and particularly in low and middle income countries. To reduce malnutrition, innovations in food systems are needed to both provide sufficient options for consumers to obtain diets with adequate nutritional value, and to help consumers make conscious and unconscious choices to choose healthier diets. A potential solution to this challenge is food systems innovations designed to lead to healthier diets. In this paper, we lay out a multidisciplinary framework for both identifying and analyzing innovations in food systems that can lead to improvements in the choices available to consumers and their diets from a health perspective. The framework identifies entry points for the design of potential food systems innovations, highlighting potential synergies, feedback, and tradeoffs within the food system. The paper concludes by providing examples of potential innovations and describes future research that can be developed to support the role of food systems in providing healthier diets.

In recent years, the world has seen unprecedented attention and political commitment to addressing malnutrition. Milestones such as the Scaling Up Nutrition (SUN) Movement, the Lancet Maternal and Child Nutrition Series, and the Second International Conference on Nutrition (ICN2) have marked the rapid rise of nutrition on the global policy and research agenda. These developments reverse years of relative neglect for nutrition. Undernutrition is a global challenge with huge social and economic costs. It kills millions of young children annually, stunts growth, erodes child development, reduces the amount of schooling children attain, and increases the likelihood of their being poor as adults, if they survive. Stunting persists through a lifetime and beyond—underweight mothers are more likely to give birth to underweight children, perpetuating undernutrition across generations. Undernutrition reduces global gross domestic product by US$1.4–$2.1 trillion a year—the size of the total economy of Africa south of the Sahara.

This report presents the latest updates related to food security and nutrition in Europe and Central Asia, including estimates on the cost and affordability of healthy diets. It also explores how governments are supporting the food and agriculture sector and how to repurpose policies and incentives to make healthy diets more affordable and agrifood systems more environmentally sustainable. The new estimates confirm that the prevalence of hunger at chronic or severe levels is relatively low in the ECA region, through the prevalence of food insecurity at moderate or severe levels can be quite high. The region is seeing alarmingly high – and rising – rates of overweight and obesity. The COVID-19 pandemic has added 25.5 million people in the region to the ranks of the moderately or severely food insecure, leaving them without access to safe, nutritious and adequate food. The war in Ukraine has made the situation worse. Almost all ECA subregions are experiencing increased costs and reduced affordability of healthy diets because of higher food prices and lower incomes. This report contains an in-depth analysis of the repurposing of food and agricultural policies to ensure the food systems transformation is better suited to addressing the “triple challenge” of achieving food security and good nutrition for better health, providing livelihoods to farmers and others connected to the sector, and reducing the nature and climate footprint of the sector. This report also reviews complementing policies within and outside of agrifood systems to assess whether repurposing efforts are impactful in the ECA region.
