Operation on fractures of shafts and joints is chiefly indicated in the upper arm, forearm, femur and tibia. Injuries in these areas can almost always be successfully treated by the techniques described in the "AS IF Manual of Internal Fixation," followed by early exercises. The fixation of small fragments which are of such biomechanical importance, has hitherto given indifferent results, especially when the distal parts of the hand and foot are involved. The implants and instruments developed by Dr. Heim and tried out by AS IF members during recent years have now been assembled into a "small fragment set," which is now available. These implants have significantly expanded the scope of AS IF instrumentation for internal fixation. Pannike first compared the application of the small fragment set with the conventional methods used in hand surgery. Heim and Pfeiffer in cooperation with Meuli, illustrate in this Volume the small fragment set, elucidating in a didactic manner the composition of the set, the indications for its use, and its method of application to all suitable fractures. Enough good results have been secured to establish the usefulness of this development. The following volume may therefore be regarded as an important supplement to the ASIF Manual. It makes it clear again that every internal fixation requires a high level of technical skill, a sense of responsibility, and very gentle handling of the soft tissues.

The AO documentation was established at the beginning of 1959, only 3 months after the founding of the AO. We used from the very beginning our X-ray cards and code sheets. Progress was rapid. Towards the end of 1959, we had already over 1000 X-ray cards with miniaturized X-rays representing the initial X-ray, the one after surgery, and the one at the 4-month follow-up. From the very beginning, the documentation was one of our four main pillars and provided us with manifold services. The docu mentation helped us in teaching the use of our instruments and implants, made it possible for us to establish a quality control system, helped us establish the indications for treatment, made it possible for us to plan graphically the operative procedures, and was of enormous help for us when we were developing our operative techniques. As an added bonus, the documentation provided us also with many slides of X-rays of the cases which we had documented. These we used in our 1ectures on the AO method. The X-ray cards of the documentation became also the basis for the AO technical commission in its evaluation of the instruments. Furthermore, the chief of each participating clinic received duplicates of all miniaturized X-rays which the documentation received from his institution. This he1ped him to see immediately what his co-workers were able to achieve and whether his co-workers had grasped the essence of the AO method and its applications.

Excerpt from On Fractures and Dislocations According to the degree of separation of the fragments at the point of fracture, we speak of a complete and of an incomplete form. To the latter belong the fissure fracture, which traverses the bone without altering its external form, and the greenstick fracture, which is most commonly seen in the bent tibite of rickety children, although it is also observed in other long bones of young adults, and in some of the flat ones. In complete fractures the line of cleavage may have a varying direction, and we distinguish between transverse, oblique, longitudinal, and spiral fractures if small fragments of bone are completely detached at the point of fracture, with or without their periosteal covering, we speak of a splintered fracture. We may have also a multiple fracture of one and the same bone (at its upper and lower ends, and its middle), and finally simultaneous fractures of several bones (for instance, of the parallel bones of forearm and leg), or of bones widely separated from each other. It is of some interest to decide whether a fracture has occurred from direct or from indirect violence, - for instance, if the ulna is broken in parrying a severe blow we have a direct fracture; if a child falls on its hand and sustains a fracture of the clavicle or the lower end of the humerus the fracture is indirect. Since in the former contusion and haemorrhage are present at the site of fracture, so the direct form, as a rule, must be held to be a more severe lesion than the indirect.
