Frailty is a condition/phenotype of decreased functional capacities across multiple bio-physiological indicators as a person ages. Older adults, in many instances, can suffer from frailty syndromes and their consequence of poor health, including e.g. increased risk of falls, disabled mobility, etc. The appropriate characterization of frailty and its related physical and mental components is thus of great importance to help predict the risk of frailty syndromes, and to optimize management and rehabilitation programs. The ultimate goals is to minimize the burden of frailty in older adults. The frailty syndromes are generally measured by using different established protocols, such as Fried’s Frailty Criteria, and conceptualized by simply accumulating the measured syndromes. Recent research works have shown that frailty not only arises from diminished physical function, but also from multiple other types of functional decline, such as cognitive impairment, and abnormalities in vascular system (e.g., hypertension). Moreover, these conditions are not distinct or isolated from each other, but closely interact and interconnected. These interactions may play critical roles in the development of frailty. Therefore, it is worthwhile to re-conceptualize “frailty” by implementing advanced technologies to measure the characteristics of functions that are pertaining to frailty, and to explore the inter-relationships between them. Thus, we can discover novel indicators to predict frailty and can develop novel strategies to optimize the management and rehabilitation of frailty. The overall goal of this research topic is to share and discuss recent achievements on understanding the pathology of frailty and its related bio-physiological characteristics, to identify novel indicators that can help predict the incidence of frailty, and to help optimize strategies for the prevention and management of frailty in aging population.

It is important to prevent and manage the frailty of the elderly because their muscle strength and physical activity decrease in old age, making them prone to falling, depression, and social isolation. In the end, they need to be admitted to a hospital or a nursing home. When successful aging fails and motor ability declines due to illness, malnutrition, or reduced activity, frailty eventually occurs. Once frailty occurs, people with frailty do not have the power to exercise or the power to move. The functions of the heart and muscles are deteriorated more rapidly when they are not used. Consequently, frailty goes through a vicious cycle. As one's physical fitness is deteriorated, the person has less power to exercise, poorer cognitive functions, and inferior nutrition intake. Consequently, the whole body of the person deteriorates. Therefore, in addition to observational studies to identify risk factors for preventing aging, various intervention studies have been conducted to develop exercise programs and apply them to communities, hospitals, and nursing homes for helping the elderly maintain healthy lives. Until now, most aging studies have focused on physical frailty. However, social frailty and cognitive frailty affect senile health negatively just as much as physical frailty. Nevertheless, little is known about social frailty and cognitive frailty. This special issue includes original experimental studies, reviews, systematic reviews, and meta-analysis studies on the prevention of senescence (physical senescence, cognitive senescence, social senescence), high-risk group detection, differentiation, and intervention.

This open access book aims to provide a comprehensive but practical overview of the knowledge required for the assessment and management of the older adult with or at risk of fragility fracture. It considers this from the perspectives of all of the settings in which this group of patients receive nursing care. Globally, a fragility fracture is estimated to occur every 3 seconds. This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.

Frailty assessment has been suggested to improve cardiovascular disease (CVD) risk management. Frailty is characterized as having a lack of reserve for tolerating health stressors. Few studies have tried standardizing frailty criteria to the population being assessed or examined the association of CVD risk profile before the onset of CVD. The purpose of my thesis is to: (1) determine if standardized frailty models better discriminate CVD risk compared to the traditional approach; and, (2) determine if there are differences in CVD risk profile based on frailty status in a sample of middle-aged and older women. The standardized model improved CVD risk discrimination by 39.4% (p=0.016) and 20.5% (p = 0.017) in the two composite CVD risk scores assessed. Comparison of CVD risk profiles between frailty groups identified variables indicating higher CVD risk as frailty status increased. Frailty assessment have the potential to identify women with elevated CVD risk.
