China’s concentrated HIV epidemic is on the brink of becoming a generalized one and syphilis infection has become a major public health threat. Social factors relating to gender and gender inequality exacerbate the spread of HIV and sexually transmitted infections (STI) in China. A better understanding of the proximate social determinants of HIV related to gender will be crucial to effectively curbing HIV and other STIs in China. Aspects of China’s governance - including administrative procedures, the developing legal system, social institutions, and the public health infrastructure – are instrumental in shaping strategies and responses to HIV. International studies suggest that women who are more economically and socially vulnerable may also have a greater risk of HIV infection, yet few initiatives have focused on discrete areas where achievable and sustainable gender policy measures could be linked to the public health response. This study presents perspectives ranging from criminology to social psychology to better understand how gender perspectives can inform HIV policy in the context of China.

Presents a unique approach to HIV prevention at the intersection of sociological and public health research Although the first AIDS cases were attributed to men having sex with men, over 70% of HIV infections worldwide are now estimated to occur through sex between women and men. In Men at Risk, Shari L. Dworkin argues that the centrality of heterosexual relationship dynamics to the transmission of HIV means that both women and men need to be taken into account in gender-specific HIV/AIDS prevention interventions. She looks at the “costs of masculinity” that shape men’s HIV risks, such as their initiation of sex and their increased status from sex with multiple partners. Engaging with the common paradigm in HIV research that portrays only women—and not heterosexually active men—as being “vulnerable” to HIV, Dworkin examines the gaps in public health knowledge that result in substandard treatment for HIV transmission and infection among heterosexual men both domestically and globally. She examines a vast array of structural factors that shape men’s HIV transmission risks and also focuses on a relatively new category of global health programs with men known as “gender-transformative” that seeks to move men in the direction of gender equality in the name of improved health. Dworkin makes suggestions for the next generation of gender-transformative health interventions by calling for masculinities-based and structurally driven HIV prevention programming. Recent innovative research has identified key factors that put vulnerable South African women at risk of HIV/AIDS and gender-based violence, including high-risk patterns of alcohol abuse and sexual partnering, gender norms that place men in control in sexual relationships, low educational levels and limited access to employment, poor health care, inadequate housing, and sex work. These studies suggest that targeted HIV-prevention interventions can effect improvement for this vulnerable population when programs remain sensitive to gender and cultural differences and expectations and address the social and economic inequalities that make women vulnerable. Solving these problems on a larger economic scale will require institutional participation and political support for women’s equity, HIV-prevention literacy, and a broader HIV-prevention agenda. This can be accomplished with a multilevel, collaborative response from government, community, and international partners using multiple prevention strategies and fostering sustainability.
