Die inhaltlichen Erläuterungen zu diesen Indikatoren finden sich im bereits publizierten in Band 3, 2. Auflage, der Version 4.0 (http://nbn-resolving.de/urn:nbn:de:kobv:83-opus4-42797). Die G-IQI Indikatoren wurden für die Messung von Qualitäts- und Leistungsindikatoren mittels der sogenannten Routinedaten entwickelt (genauer gesagt für die im Rahmen der DRG-Abrechnung erfassten Daten nach § 21 KHEntgG oder § 301 SGB V). Die G-IQI Indikatoren geben eine übersichtliche und medizinische sinnvoll strukturierte Auskunft über wichtige und häufige Krankheitsbilder und Eingriffe vieler medizinischer Fachgebiete. Dort, wo es sinnvoll ist, werden Informationen zur Ergebnisqualität unter anderem in Form der Sterblichkeit angegeben. Die G-IQI Indikatoren decken in der Version 4.0 rund 42% aller Krankenhausfälle in Deutschland ab. Dies ist mehr als bei allen anderen, vergleichbaren krankheitsbezogenen Indikatorsystemen. Über die Sterblichkeitsindikatoren liefert G-IQI wichtige Ergebnisinformationen, die über 51% aller im Krankenhaus auftretenden Todesfälle betreffen. Band 3, 2. Aufl., enthält neben der Erläuterung der Indikatoren eine Auswertung der DRG-Daten aller deutschen Krankenhäuser. So können für alle Indikatoren die Mengen und Referenzwerte auf Bundesebene angegeben werden. Für viele Krankheitsbilder liegen damit erstmals vollständige Informationen zur Versorgungssituation und Versorgungsqualität in Deutschland vor (in Anhang 1 von Band 3 dargestellt). Indikatorensysteme wie G-IQI können für die öffentliche Darstellung (Public Reporting) der Krankenhausleistungen eingesetzt werden. In diesem Sinne werden G-IQI von der Initiative Qualitätsmedizin sowie in der Schweizer Variante (CH-IQI) vom Schweizer Bundesamt für Gesundheit verwendet. Darüber hinaus können Auffälligkeiten in den G-IQI Ergebnisindikatoren Anlass für die Untersuchung und Verbesserung der Behandlungsprozesse beim betreffenden Krankheitsbild sein. Mit Hilfe der Indikatoren ist es möglich, ein effizientes, hochwirksames ergebnisorientiertes Qualitätsmanagement im Krankenhaus aufzubauen. Die hierzu eingesetzten Verfahren werden im genannten Band ebenfalls genauer erörtert. Volume 4 of the fourth revision and extension of the German Inpatient Quality Indicators (G-IQI 4.0) contains the exact indicator definitions based on the German coding system for the year 2014. Thus volume 4 is an extension to the previously published volume 3, 2nd edition, which contains the detailed explanations of the G-IQI system and various evaluations for the German hospitals (http://nbn-resolving.de/urn:nbn:de:kobv:83-opus4-42797). By concept and definition the G-IQI indicators are constructed in a way, that they can be derived from existing administrative data. In Germany, these are available due to the DRG billing process, which covers all inpatient cases except psychiatry. Data are available on the hospital level, at insurers and due to respective legal regulations on the national level (data sets according to §21 KHEntgG and §301 SGB V). G-IQI provide a comprehensive and medically meaningful overview over important and/or frequent inpatient services. They inform about volume and wherever possible about outcome. The G-IQI indicators cover about 42% of all German inpatient cases, which is more than in all other disease-specific indicator systems. The mortality indicators cover over 51% of all inpatient deaths. Besides the user guide and technical specifications volume 3, 2nd ed., also presents an evaluation of the German national DRG data. Thus national reference values and inpatient volumes can be given for all G-IQI indicators (listed in Anhang 1 of volume 1). This provides new information about the state of medical care for many diseases in Germany, which otherwise is not available on a national level. Indicator systems like G-IQI may be used for public reporting. In this way G-IQI is currently utilized voluntarily in Germany by the ‘Initiative Qualitätsmedizin’ and obligatory in Switzerland (via the Swiss variant CH-IQI) for all hospitals by the ‘Bundesamt für Gesundheit’. Furthermore, deviations of outcome from national reference values for certain quality indicators within a hospital may be used to induce further investigations of treatment processes within the hospital in order to find ways for improvement. G-IQI together with internal peer review procedures can be used to build up a very efficient and powerful outcome-oriented quality management system within a hospital. Volume 3, 2nd ed., also outlines how such a system can be set up.

Dort wo es sinnvoll ist, werden neben den Mengeninformationen auch Angaben zur Ergebnisqualität unter anderem in Form der Krankenhaussterblichkeit dargestellt. Mit diesem Band wird die neue und überarbeitete G-IQI Version 4.2 mit den Indikatordefinitionen auf der Basis der deutschen Kodiersysteme des Datenjahres 2015 vorgestellt. Gegenüber der Version 4.0 sind durch viele Modifikationen und Präzisierungen im Detail neue, spezielle Krankheitsbilder hinzugekommen, bei denen die Erfahrung eines Krankenhauses für die betroffenen Patienten von Interesse ist (beispielsweise die Behandlung von Epilepsien im Kinder- und Erwachsenenalter oder die Behandlung von Hirntumoren). Für andere Bereiche wurde die bisher existierende Gliederung grundlegend überarbeitet (beispielsweise bei Hüft- und Knieendoprothesen oder bei Erkrankungen der Wirbelsäule). Im Vergleich zur Version 4.1 enthält die Version 4.2 neue Differenzierungen im Bereich der Eingriffe an der Hauptschlagader sowie eine Präzisierung im Bereich der Operationen an der Lunge. In der neuen Version 4.2 decken die G-IQI Indikatoren gemessen an den DRG-Daten 2013 jetzt 44,9% aller Krankenhausfälle in Deutschland ab. Dies ist mehr als bei allen anderen, vergleichbaren krankheitsbezogenen Indikatorsystemen. Über die Sterblichkeitsindikatoren stellen die G-IQI wichtige Ergebnisinformationen für mehr als 55,7% aller im Krankenhaus auftretenden Todesfälle transparent dar. Die technische Spezifikation der Indikatoren wird ergänzt um eine Auswertung der DRG-Daten aller deutschen Krankenhäuser aus dem Datenjahr 2013. Damit werden für alle Indikatoren auch die Referenzwerte auf Bundesebene angegeben. Für viele Krankheitsbilder bzw. Operationen liegen damit erstmals vollständige Informationen zur Versorgungssituation und Versorgungsqualität in Deutschland vor. Ferner können sich Krankenhäuser, die G-IQI einsetzen, anhand dieser Angaben mit den deutschen Durchschnittswerten vergleichen. Indikatorensysteme wie G-IQI können für die öffentliche Darstellung (Public Reporting) der Krankenhausleistungen eingesetzt werden. In diesem Sinne werden G-IQI von der Initiative Qualitätsmedizin sowie in der Schweizer Variante (CH-IQI) vom Schweizer Bundesamt für Gesundheit verwendet. Vor allem aber dienen die G-IQI Indikatoren im hausinternen Qualitätsmanagement zur weiteren Verbesserung der Ergebnisse. Auffälligkeiten in den G-IQI Ergebnisindikatoren können und sollten Anlass für die Untersuchung und Verbesserung der Behandlungsprozesse beim betreffenden Krankheitsbild sein. Mit Hilfe dieser Indikatoren ist es bei entsprechendem Einsatz möglich, ein effizientes, hochwirksames ergebnisorientiertes Qualitätsmanagement im Krankenhaus aufzubauen. Die hierzu eingesetzten Verfahren wurden mit der Veröffentlichung der Version 4.0 und an anderer Stelle bereits erörtert. The German Inpatient Quality Indicators (G-IQI) have been developed for measuring outcome and volume indicators for specified diseases and/or procedures based on administrative data. In Germany the administrative billing data of the hospitals (the so called DRG-data) are electronically exchanged with the insurance companies and also once a year delivered to the federal office of statistics via standardized data interfaces (specifications according to § 21 KHEntgG and § 301 SGB V). G-IQI provide medically meaningful information about important and/or frequent diseases and procedures, which are treated or performed, respectively, in German hospitals. Wherever possible and medically sensible, information concerning outcome is provided besides volume indicators. In many cases this is the in-hospital mortality for the specific disease or procedure. This volume presents the revised G-IQI version 4.2 together with the technical specifications based on the German coding systems for the data year 2015. Besides modifications and clarifications referring to many details of the version 4.0 new indicators for additional disease and procedure groups have been defined. These refer to many diseases, where the public or the potential patient looking for a hospital would like to get at least information about the experience, which a hospital has in treating this disease. This would for example be the case for the treatment of epilepsy in children or adults or for the treatment of brain tumors. For other areas the previous structure of G-IQI has been revised and the subgroups have been segmented in a different way (for example in case of hip and knee replacement or spinal surgery). Compared to the last version 4.1, the new version 4.2 contains new classifications for aortic surgery as well as a clarification regarding lung surgery. The new G-IQI version 4.2 covers 44.9% of all in-patient cases in Germany according to the German DRG data 2013. This is more than for other disease specific indicator systems. The in-hospital mortality indicators cover 55.7% of all in-hospital deaths. The technical specification of the indicators given in this volume is augmented by a complete analysis of all German inpatient episodes 2013 using the G-IQI indicators (18 million inpatient cases). Thus for every indicator the national volume and the respective reference value is given in this publication. For many diseases and procedures covered by G-IQI this is a unique reference. Furthermore hospitals using G-IQI can use these values to compare themselves to the national average. Indicator systems like G-IQI can be used for public reporting. In this way G-IQI is used by the ‘Initiative Qualitätsmedizin’, an association of currently 330 German hospitals publishing their results based on these indicators. Furthermore the Swiss Bundesamt für Gesundheit uses a Swiss variant of these indicators (CH-IQI) for an obligatory public reporting for all Swiss hospitals. Predominantly, however, G-IQI indicators are used within the internal quality management of the hospitals for further improving their outcome. Results which deviate from the federal average may and should be a reason for investigating the treatment processes of the respective disease, which may for example involve peer reviews of medical records. Using the indicators as a trigger tool it is possible to install an efficient and highly effective outcome oriented quality management within a hospital. The methods used for this purpose have been described in other publications.

Recent decades have seen remarkable advances in the treatment of upper gastrointestinal malignancies, i.e., adenocarcinoma and squamous cell carcinoma as well as gastrointestinal stromal and other rare tumors of the esophagus and stomach. While, historically, surgical resection has been the sole treatment for these tumors, multimodal therapies have meanwhile proven their efficacy. At present, pre- and postoperative chemotherapy and radiotherapy, targeted drug therapy, and stage-specific surgical approaches are all indispensable cornerstones of an individualized treatment for upper gastrointestinal malignancies. With such multimodal treatment, better outcomes comprising improved quality of life and prolonged survival have been achieved for patients. However, for many tumor entities and stages, the ideal combination and sequence of treatments is still being evaluated in clinical trials. Moreover, the value of novel approaches such as immunotherapy or robotic surgery remains a matter of research.
