Every year nine million patients receive urgent medical care out of hours, a period that, as currently defined, accounts for two-thirds of every week. At present, GPs are responsible for providing the service to their patients, though some 70 per cent of this is delivered by GP co-operatives, designed to achieve economies of scale and relieve pressure on individual GPs. Under the terms of the new General Medical Services contract, GPs will be able to opt out of the obligation to provide out-of-hours care for their patients, with the Primary Care Trusts (PCTs) taking over responsibility for the service. The British Medical Association estimated that nine out of ten GPs would opt out. Although the availability of GPs for out-of-hours work is uncertain, the Committee expects that GPs will continue to provide the core of the service. This report finds that PCTs across the country are in varying states of readiness for taking on this service. Whilst the Committee was impressed with some models of out-of-hours service provision, including integration with ambulance services and creative use of skill mix, some PCTs were failing to consider more integrated approaches. An area of concern is the funding of the service, as the previous cost had, in effect, been hidden by the previous practice; so this cost is essentially a new one for the NHS. The Committee supports the introduction of quality standards for all providers of the service.
