The subject of the apallic syndrome is one which has long been familiar to me, although I have not personally studied it as deeply as I would have wished. I became acquainted with this syndrome long before the last war, when my neurosurgical colleague Hugh Cairns (1952), made his pioneer contribution under the term "akinetic mutism" . This was an ar resting title, but it was one which did not altogether satisfy some of his colleagues, includ ing myself. We found it difficult to suggest an alternative. That is one reason why I wel come the expression "apallic syndrome" . Forensic practice has forced me from time to time to consider rather more deeply this distressing syndrome, and to try and marshal my ideas in a form which would satisfy my colleagues in the legal profession. More than once I have been instructed to make a medico legal assessment of these unfortunate patients. The points which have concerned my lawyer friends have not been matters of diagnosis, or of morbid anatomy, or of etiology. The fac tual problem which has been put before me was to make some approximate assessment as to the expectation of life. Vague guess-work is unacceptable in such circumstances. What the lawyers require is a precise and dogmatic answer.

Im Mittelpunkt der theoretischen šberlegungen steht die Frage, welche Relevanz das Konstrukt des Koh„renzgefhls fr die Pr„vention postpartaler Depressionen hat. Das Buch zeigt zun„chst auf, dass das Konstrukt des Koh„renzgefhls ein differenziertes Verst„ndnis von Gesundheit in der Beratung erm”glicht. Die Betrachtung von Risiko- und Protektivfaktoren fr die Entstehung einer PPD kann zu einer st„rkeren Ressourcenorientierung in der Beratung beitragen. Es wird er”rtert, ob eine sozial”kologische Orientierung, die an den Ursachen von Problemen ansetzt und auch komplexere Problemlagen in den Blick nimmt, den dafr notwendigen Rahmen bietet. Die Betrachtung der strukturellen Rahmenbedingungen eines Wiener Modellprojekts zeigt, dass durch die Implementation des Beratungsangebots im Klinikkontext die notwendige Niedrigschwelligkeit erreicht wird. Auf inhaltlicher Ebene werden vier wesentliche Beratungsschwerpunkte identifiziert: Soziale Untersttzung, Partnerschaft, Auseinandersetzung mit den Ver„nderungen durch das Elternwerden sowie mit der Gefhlswelt. In Anlehnung an das Life Model der Sozialen Arbeit (Germain/Gitterman 1999) werden schlieálich Methoden vorgestellt, die fr die Arbeit mit den drei Komponenten des Koh„renzgefhls Verstehbarkeit, Handhabbarkeit und Sinnhaftigkeit hilfreich sind. Die Untersuchung gelangt zu der Schlussfolgerung, dass die F”rderung der Eigenst„ndigkeit und des Vertrauens in die eigenen F„higkeiten wesentliche Ziele in der pr„ventiven Beratung sein mssen. Offensivere methodische Elemente erscheinen angesichts der enormen Umstellungen und Neuorientierungen w„hrend einer Schwangerschaft angebracht. Die Arbeit mit dem Koh„renzgefhl in der sozialen Beratung kann eine Klientin auf den Umgang mit dem šbergang zur Elternschaft vorbereiten, indem es auch eine langfristige Perspektive bei der Bew„ltigung der kritischen Lebensphase er”ffnet.
