Whether increasing life expectancy leads to better health remains still controversial. Three topics are explored: (1) vanguard groups which inform about possible levels of health if the general social and environmental conditions were to approach those of the vanguard group; (2) the social and behavioral determinants of health differentiated into proximal and distal factors; (3) vulnerable groups such as migrants and the health differences between migrant groups. Newly available population-based data as well as new study designs and advanced statistical modelling form the basis for the empirical analyses.

This is why I have chosen the country to compare especially the Health Care System with the German one. Not only the economic achievements are comparable, but first of all the Demographic Change. In 2020 28% of the Japanese population will be over 65 years. In Germany it is prognosticated to be around 21% (Tab.1). Moreover the Japanese Health Care System is known as own of the cheapest of the industrialized countries. This leads to the question of adoption of some parts or ideas from the Japanese system to the German one. In my elaboration I want to describe a case of a family in Japan regarding the family and work situation, the Health Care System in general and additionally the system of taking care of the elderly and the children. My example family has got following parts: The mother is 42 years old, she works as a nurse in an outpatient department of a local government. In addition she has to take care of her parents almost every day. The father is 45 years old, works as an engineer and likes his hobby, which is driving motorbikes. The daughter of them is 7 and their son is 13, both go to school. The grandmother has got dementia, she is 76 years old and lives together with her husband, who is 76 and has got diabetes. They live in the neighborhood of their children and grandchildren. Case 1: Values, culture, roles of the family members. Case 2: Health Care System of Japan in comparison to the German one. What happens after a traffic accident of the father? Case 3: Elderly and Child Care in general and in the case of the accident and depression of the mother.

In recent years, Japan and Germany have been facing very similar challenges: aging populations, changing employment structures, long-lasting economic stagnation, and globalization, all of which threaten existing social security arrangements. Both countries are in a number of respects more socially and politically regulated, and in this sense less liberal, than the Anglo-American economies. Nonetheless, during the last couple of years several important social policy reforms have been implemented. A comparison of these reforms may provide valuable insights into the changing character of "conservative" welfare states. This volume concentrates on two fields of social policy: long-term care insurance and public pensions. These social insurances are at the center of current public debate in both countries because population aging translates immediately into a higher demand for care for the elderly and old age security. The contributions range from demographic and policy implications of aging through detailed analyses on the different reform measures to specific aspects such as bioethical or regional policy considerations. The volume is intended not only for experts specializing in social policy research but also for policy makers as well as the general reader interested in the current debates centering on the "restructuring of the welfare state".

The main aim of the book is to provide an interdisciplinary treatment of a set of key issues of current ageing research, i.e., health, competence, and well-being. These key issues are addressed based on three converging research streams: social-ecological research, which assumes that major processes and outcomes of ageing such as day-to-day competence are shaped by social and physical-spatial environments; geropsychology research, which is driven by a life-span developmental conception of ageing; and epidemiology, which offers most fundamental disease, function and prevention-related data. All chapters are then also discussed by renowned ageing experts. The increase in the number and life expectancy of elderly people is a general trend across Europe. Each country responds differently to the increased demands for elderly care, due to differences in their socio-cultural, political, and historical backgrounds. This book describes patterns of caregiving to frail, elderly people in Denmark, France, the Netherlands, and Germany. For each country, characteristic features of professional and informal elderly care are described, specifically focusing on home nursing. Differences in home nursing in these four countries are described from a broad sociological and cultural perspective. Steadily increasing life expectancy is one of the great achievements of industrialised societies over the last century. Life expectancy has not only been growing among the young and those reaching retirement age, but also, especially in recent decades, among people ages 80 and above. These improvements in life expectancy have led to the emergence of the so-called third age, when people retire, but are still you- ful, healthy and able to participate in society. Nevertheless, closer to the end of life, a fourth age of decrepitude and dependence on others has to be anticipated. - spite the postponement of functional limitations and severe disabilities into higher ages, the debate continues over whether the additional years gained are healthy years, or years with severe care need, particularly among the oldest old, the fastest growing segment of the population. Future improvements in life expectancy and the health status of the elderly will determine the need for care in the future. While different assumptions about these trends based on expert opinion or the extrapolation of past experiences can be made, there will always be a degree of uncertainty about future trends. A third - portant factor driving the extent of future care need is, however, already determined by the history of the past century and is embedded in the age structures of our populations.
