The U.S. military has been continuously engaged in foreign conflicts for over two decades. The strains that these deployments, the associated increases in operational tempo, and the general challenges of military life affect not only service members but also the people who depend on them and who support them as they support the nation â€" their families. Family members provide support to service members while they serve or when they have difficulties; family problems can interfere with the ability of service members to deploy or remain in theater; and family members are central influences on whether members continue to serve. In addition, rising family diversity and complexity will likely increase the difficulty of creating military policies, programs and practices that adequately support families in the performance of military duties. Strengthening the Military Family Readiness System for a Changing American Society examines the challenges and opportunities facing military families and what is known about effective strategies for supporting and protecting military children and families, as well as lessons to be learned from these experiences. The United States is in the midst of the largest military demobilization in its history. This is leading to an increase in the demand for mental health clinicians who can provide services to hundreds of thousands of military veterans and members of the military. Nearly two million Americans have been deployed to the wars in the Middle East, and thousands of them have been deeply affected, either psychologically, physically, or both. Projections suggest that 300,000 are returning with symptoms of PTSD or major Depression; 320,000 have been exposed to probable Traumatic Brain Injuries; and hundreds of thousands are dealing with psychological effects of physical injuries. Other veterans and members of the military without injuries will seek treatment to help them with the psychological impact of serving in the military, being deployed, or transitioning and reintegrating back into the civilian world. As an example, hundreds of thousands of service members are also leaving the armed forces earlier than they anticipated and will need to quickly adjust to life as civilians after assuming that they would have many more years in the military. Many will be leaving the military because of demobilizations and downsizing due to budget cuts. Current proposed cuts will shrink the military force to the same size it was in 1940. The Pew Center reports that 44% of veterans from the current wars are describing their readjustment to civilian life as difficult, and many of them are and will be turning to civilian mental health and primary care clinicians for assistance. The Handbook of Psychosocial Interventions for Veterans and Service Members is a one stop handbook for non-military clinicians working with service members, veterans, and their families. It brings together experts from the Department of Defense, the Department of Veterans Affairs, veteran service organizations, and academia to create the first comprehensive guidebook for civilian clinicians. The rights of pregnant workers as well as (the lack of) paid maternity leave have increasingly become topics of a major policy debate in the United States. Yet, few discussions have focused on the U.S. military, where many of the latest policy changes focus on these very issues. Despite the armed forces' increases to maternity-related benefits, servicewomen continue to be stigmatized for being pregnant and taking advantage of maternity policies. In an effort to understand this disconnect, Megan McFarlane analyzes military documents and conducts interviews with enlisted servicewomen and female officers. She finds a policy/culture disparity within the military that pregnant servicewomen themselves often co-construct, making the policy changes significantly less effective. McFarlane ends by offering suggestions for how these policy changes can have more impact and how they could potentially serve as an example for the broader societal debate.

Over the past two decades of conflict, women have served with valor and continue to serve on combat aircraft, naval vessels, and in support of ground combat operations. The expansion of roles for women in the Armed Forces has evolved since the early days of the military when women were restricted by law and policy from serving in certain occupations and units. Women have not been precluded by law from serving in any military unit or occupational specialty since 1993 when Congress repealed the remaining prohibitions on women serving on combatant aircraft and vessels. However, Department of Defense (DOD) policies have prevented women from being assigned to units below brigade level where the unit's primary mission was to engage directly in ground combat. This policy barred women from serving in infantry, artillery, armor, combat engineers, and special operations units of battalion size or smaller. On January 24, 2013, then-Secretary of Defense Leon Panetta rescinded the rule that restricted women from serving in combat units and directed the military departments and services to review their occupational standards and assignment policies and to make recommendations for opening all combat roles to women no later than January 1, 2016. On December 3, 2015, Secretary of Defense Ashton Carter ordered the military to open all combat jobs to women with no exceptions. This most recent policy change followed extensive studies that were completed by the military departments and by the Special Operations Command (SOCOM) on issues such as unit cohesion, women's health, equipment, facilities modifications, propensity to serve, and international experiences with women in combat. These studies also included a review and validation of gender-neutral occupational standards for combat roles where such standards existed. On March 10, 2016, Secretary Carter announced that the Services' and SOCOM's implementation plans for the integration of women into direct ground combat roles were approved. Some concerns about the implementation of the new policy remain, including the recruitment, assignment, and career management of women into the new roles, and the impact of integration on unit readiness. Congress has oversight authority in these matters, and may also consider issues such as equal opportunity, equal responsibility (such as selective service registration), and the overall manpower needs of the military.

In the World War I era, veterans fought for a unique right: access to government-sponsored health care. In the process, they built a pillar of American social policy. Burdens of War explores how the establishment of the veterans’ health system marked a reimagining of modern veterans’ benefits and signaled a pathbreaking validation of the power of professionalized institutional medical care. Adler reveals that a veterans’ health system came about incrementally, amid skepticism from legislators, doctors, and army officials concerned about the burden of long-term obligations, monetary or otherwise, to ex-service members. She shows how veterans’ welfare shifted from centering on pension and domicile care programs rooted in the nineteenth century to direct access to health services. She also traces the way that fluctuating ideals about hospitals and medical care influenced policy at the dusk of the Progressive Era; how race, class, and gender affected the health-related experiences of soldiers, veterans, and caregivers; and how interest groups capitalized on a tense political and social climate to bring about change. The book moves from the 1910s—when service members requested better treatment, Congress approved new facilities and increased funding, and elected officials expressed misgivings about who should have access to care—to the 1930s, when the economic crash prompted veterans to increasingly turn to hospitals for support while bureaucrats, politicians, and doctors attempted to rein in the system. By the eve of World War II, the roots of what would become the country’s largest integrated health care system were firmly planted and primed for growth.
