The underrepresentation of minorities in health and other professions has long cast a shadow over our nation's efforts to develop a more representative and productive society. Many programs have been developed to enlarge the presence of minorities in health careers, but these efforts have been unable to develop the infrastructure and momentum needed to produce and sustain an adequate number of minority professionals among the ranks of clinicians, researchers, and teachers. This book looks at the historical significance of this underrepresentation, presents data that define the problem, and identifies underlying factors that contribute to the failure to achieve fairness in opportunity. The volume examines programs that have made successful efforts to decrease underrepresentation and sets forth an action and research agenda for further enhancing the numbers of minorities in the health professions.

Racial and ethnic disparities in health care are known to reflect access to care and other issues that arise from differing socioeconomic conditions. There is, however, increasing evidence that even after such differences are accounted for, race and ethnicity remain significant predictors of the quality of health care received. In Unequal Treatment, a panel of experts documents this evidence and explores how persons of color experience the health care environment. The book examines how disparities in treatment may arise in health care systems and looks at aspects of the clinical encounter that may contribute to such disparities. Patients' and providers' attitudes, expectations, and behavior are analyzed. How to intervene? Unequal Treatment offers recommendations for improvements in medical care financing, allocation of care, availability of language translation, community-based care, and other arenas. The committee highlights the potential of cross-cultural education to improve provider-patient communication and offers a detailed look at how to integrate cross-cultural learning within the health professions. This book focuses on the achievement of health equity. It proposes pragmatic, feasible plans to greatly reduce them or perhaps eliminate them. Health is taken here in its most global sense, as defined by the WHO: "Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity", which includes the achievement of one's full career potential (eg: equality of opportunity for minorities to become public health professionals). In this book, the author address disparities existing in several populations to include (but not be limited to) low-income, racial and ethnic minorities, women, children, low literacy, disabled, GLBTI, as well as other groups who are victims of health disparities. While the needs of these populations are discussed openly and candidly, the author vigorously fights stereotyping of specific groups. The book will expose the institutionalised, personally-mediated and internalised mechanisms, which cause or perpetuate differences in opportunities, stresses and exposures as well as in health care access and quality. Currently, health disparities represent the most important challenge of public health. This book proposes to unveil often undetected, neglected aspects of health disparities embedded in all aspects of health and healthcare as well as medical research. Concepts are discussed based on critical appraisal of available evidence in current literature. Uncomfortable issues such as racism and discrimination in health and health care are discussed, respectfully but openly. These issues are never easy to talk about; however, being too comfortable often means very little change. Being uncomfortable can be a stimulus for change.

Affirmative action programs have significantly changed American medicine for the better, not only in medical school admissions and access to postgraduate training but also in bringing a higher quality of health care to all people. James L. Curtis approaches this important transition from historical, statistical, and personal perspectives. He tells how over the course of his medical education and career as a psychiatrist and professor--often as the first or only African American in his cohort--the status of minorities in the medical professions grew from a tiny percentage to a far more equitable representation of the American population. Advancing arguments from his earlier book, Blacks, Medical Schools, and Society, Curtis evaluates the outcomes of affirmative action efforts over the past thirty years. He describes formidable barriers to minority access to medical-education opportunities and the resulting problems faced by minority patients in receiving medical treatment. His progress report includes a review of two thousand minority students admitted to U.S. medical schools in 1969, following them through graduation and their careers, comparing them with the careers of two thousand of their nonminority peers. These samples provide an important look at medical schools that, while heralding dramatic progress in physician education and training opportunity, indicates much room for further improvement. A basic hurdle continues to face African Americans and other minorities who are still confined to segregated neighborhoods and inferior school systems that stifle full scholastic development. In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.
