At the end of the twentieth century, Asia is in the middle of an economic, political, and social transformation. Many countries are poised to enter the next century on a rising curve of rapid economic growth and human resource development. Despite an increasing proportion of national income devoted to health, government resources are not sufficient to maintain existing health systems, meet increased demand, and improve the quality of care. Attitudes about the role of government in financing health care services have also changed. As a result, many countries in Asia have adopted a strategy of encouraging the expansion of the private health sector in health. This book describes these policy shifts and the health sector reform issues in the countries of the region. The actual process of change in six countries: India; Indonesia; Korea; the Philippines; Thailand; and Vietnam is also documented. This collection of papers, by eleven leading international experts, was prepared for the Asian Development Bank?s Second Regional Conference on Health Sector Reform: Issues Related to Private Sector Growth, held in Manila in 1996. Management Sciences for Health commissioned these papers and managed the conference for the ADB. The ADB is a regional multilateral development bank with 58 member countries which provided development assistance through loans and grants to 36 countries in Asia and the Pacific. Management Sciences for Health is a non-profit organization dedicated to helping health care policy makers and managers develop and implement practical strategies to meet the challenges faced by the public and private sectors of health systems.

This book empirically examines health care financing reforms and popular responses in three major cities in East Asia: Shanghai, Singapore, and Hong Kong. It adopts a new revised version of the theory of historical institutionalism to compare and explain the divergent reform paths in these three places over the past three decades. It also examines forces that propel institutional change. The book provides three detailed case studies on the development of health care financing reforms and the politics of implementing them. It shows that health care systems in Shanghai, Singapore, and Hong Kong were the products of Western presence in the nineteenth century. It illustrates how greater attention is paid to the roles played by ideas, actors, and environmental triggers without abandoning the core assumptions that political institutions and policy feedback remain central to impact health care financing reforms. It shows that health care financing reform is shaped by a complex interplay of forces over time. It is easy for reformers and their overseas advisers to develop conceptually strong plans for health sector reform that appeal to governments, donors, and academics. Transforming these plans into successful action has proved to be extremely difficult. Toward the goal of improving assistance to this sector, the Economic Development Institute (EDI) of the World Bank held a seminar in Ashgabat, Turkmenistan in June 1996. This publication provides a summary of the proceedings and papers presented at that seminar. It was attended by delegations from five countries in Central Asia and from Azerbaijan and Mongolia. The content of this volume emphasizes topics central to the restructuring of health sector financing and health care delivery systems at a time of transition to market economies and to democracy.
