Drawing upon the empirical scholarship and research expertise of contributors from all settled continents and from diverse life settings and economies, Viral Loads illustrates how the COVID-19 pandemic, and responses to it, lay bare and load onto people’s lived realities in countries around the world. A crosscutting theme pertains to how social unevenness and gross economic disparities are shaping global and local responses to the pandemic, and illustrate the effects of both the virus and efforts to contain it in ways that amplify these inequalities. At the same time, the contributions highlight the nature of contemporary social life, including virtual communication, the nature of communities, neoliberalism and contemporary political economies, and the shifting nature of nation states and the role of government. Over half of the world’s population has been affected by restrictions of movement, with physical distancing requirements and self-isolation recommendations impacting profoundly on everyday life but also on the economy, resulting also, in turn, with dramatic shifts in the economy and in mass unemployment. By reflecting on how the pandemic has interrupted daily lives, state infrastructures and healthcare systems, the contributing authors in this volume mobilise anthropological theories and concepts to locate the pandemic in a highly connected and exceedingly unequal world. The book is ambitious in its scope – spanning the entire globe – and daring in its insistence that medical anthropology must be a part of the growing calls to build a new world.

The pioneering and hugely influential work of Mikhail Bakhtin has led scholars in recent decades to see all discourse and social life as inherently "dialogical." No speaker speaks alone, because our words are always partly shaped by our interactions with others, past and future. Moreover, we never fashion ourselves entirely by ourselves, but always do so in concert with others. Bakhtin thus decisively reshaped modern understandings of language and subjectivity. And yet, the contributors to this volume argue that something is potentially overlooked with too close a focus on dialogism: many speakers, especially in charged political and religious contexts, work energetically at crafting monologues, single-voiced statements to which the only expected response is agreement or faithful replication. Drawing on ethnographic case studies from the United States, Iran, Cuba, Indonesia, Algeria, and Papua New Guinea, the authors argue that a focus on "the monologic imagination" gives us new insights into languages' political design and religious force, and deepens our understandings of the necessary interplay between monological and dialogical tendencies.

Drawing on a growing consensus about the importance of community representation and participation for ethical research, community engagement has become a central component of scientific research, policy-making, ethical review, and technology design. The diversity of actors involved in large-scale global health research collaborations and the broader ‘background conditions’ of global inequality and injustice that frame the field have led some researchers, funders, and policy-makers to conclude that community engagement is nothing less than a moral imperative in global health research. Rather than taking community engagement as a given, the contributions in this edited volume highlight how processes of community engagement are shaped by particular local histories and social and political dynamics, and by the complex social relations between different actors involved in global public health research. By interrogating the everyday politics and practices of engagement across diverse contexts, the book pushes conversations around engagement and participation beyond their conventional framings. In doing so, it raises radical questions about knowledge, power, expertise, authority, representation, inclusivity, and ethics and to make recommendations for more transformative, inclusive, and meaningful community engagement. This book was originally published as a special issue of the Critical Public Health journal.

It then juxtaposes three social innovations targeting niche 'markets' for lower-cost services in the Majority World, against three present-day examples of publicly-financed and -delivered PHC, demonstrating what corporatisation does to PHC, within deeply entrenched colonial-capitalist structures and discourses that normalize inferior care, private profit, and dispossession of peoples.

"The Affordable Care Act set off an unprecedented wave of health insurance enrollment as the most sweeping overhaul of the U.S. health insurance system since 1965. In the years since its enactment, some 20 million uninsured Americans gained access to coverage. And yet, the law remained unpopular and politically vulnerable. While the ACA extended social protections to some groups, its implementation was troubled and the act itself created new forms of exclusion. Access to affordable coverage options were highly segmented by state of residence, income, and citizenship status. Unequal Coverage documents the everyday experiences of individuals and families across the U.S. as they attempted to access coverage and care in the five years following the passage of the ACA. It argues that while the Affordable Care Act succeeded in expanding access to care, it did so unevenly, ultimately also generating inequality and stratification. The volume investigates the outcomes of the ACA in communities throughout the country and provides up-close, intimate portraits of individuals and groups trying to access and provide health care for both the newly insured and those who remain uncovered. Cuba has more medical personnel serving abroad—over 50,000 in 66 countries—than all of the G-7 countries combined and more than the World Health Organization. For the last five decades, they have been a leading force in the developing world, providing humanitarian aid (or “cooperation,” as Cuba’s communist government prefers) and initiating programs for preventative care and medical training. In Healthcare without Borders, John Kirk examines the role of Cuban medical teams in disaster relief, biotechnology joint ventures, and in the Latin American School—the largest medical faculty in the world. He looks at their responses to various crises worldwide, including the 1960 earthquake in Chile, the Chernobyl disaster in the Ukraine, the earthquake that wracked Haiti in 2010 and the subsequent cholera outbreak, and the 2014 Ebola epidemic in West Africa. Kirk issues an informative and enlightening corrective for what he describes as the tendency of the industrialized world’s media to ignore or underreport this phenomenon as one of the positive aspects of the Cuban revolutionary process. In the process, Kirk explores the philosophical underpinnings of human rights and access to medical care at the core of Cuba’s medical internationalism programs and partnerships.
