A particular disorder might be described as "running in a family" if more than one person in the family has the condition. Some disorders that affect multiple family members are caused by gene mutations, which can be inherited (passed down from parent to child). Other conditions that appear to run in families are not caused by mutations in single genes. Instead, environmental factors such as dietary habits or a combination of genetic and environmental factors are responsible for these disorders. It is not always easy to determine whether a condition in a family is inherited. A genetics professional can use a person's family history (a record of health information about a person's immediate and extended family) to help determine whether a disorder has a genetic component. He or she will ask about the health of people from several generations of the family, usually first-, second-, and third degree relatives.

Genetic testing is a type of medical test that identifies changes in chromosomes, genes, or proteins. The results of a genetic test can confirm or rule out a suspectedgenetic condition or help determine a person's chance of developing or passing ona genetic disorder. More than 1,000 genetic tests are currently in use, and moreare being developed.Several methods can be used for genetic testing: Molecular genetic tests (or gene tests) study single genes or short lengthsof DNA to identify variations or mutations that lead to a genetic disorder. Chromosomal genetic tests analyze whole chromosomes or long lengthsof DNA to see if there are large genetic changes, such as an extra copyof a chromosome, that cause a genetic condition. Biochemical genetic tests study the amount or activity level of proteins;abnormalities in either can indicate changes to the DNA that result in agenetic disorder.Genetic testing is voluntary. Because testing has benefits as well as limitations andrisks, the decision about whether to be tested is a personal and complex one. Ageneticist or genetic counselor can help by providing information about the prosand cons of the test and discussing the social and emotional aspects of testing.

The purpose of this manual is to provide an educational genetics resource for individuals, families, and health professionals in the New York - Mid-Atlantic region and increase awareness of specialty care in genetics. The manual begins with a basic introduction to genetics concepts, followed by a description of the different types and applications of genetic tests. It also provides information about diagnosis of genetic disease, family history, newborn screening, and genetic counseling. Resources are included to assist in patient care, patient and professional education, and identification of specialty genetics services within the New York - Mid-Atlantic region. At the end of each section, a list of references is provided for additional information. Appendices can be copied for reference and offered to patients. These take-home resources are critical to helping both providers and patients understand some of the basic concepts and applications of genetics and genomics.

Genetic changes that are described as de novo (new) mutations can be eitherhereditary or somatic. In some cases, the mutation occurs in a person's egg orsperm cell but is not present in any of the person's other cells. In other cases, themutation occurs in the fertilized egg shortly after the egg and sperm cells unite. (Itis often impossible to tell exactly when a de novo mutation happened.) As thefertilized egg divides, each resulting cell in the growing embryo will have themutation. De novo mutations may explain genetic disorders in which an affectedchild has a mutation in every cell in the body but the parents do not, and there isno family history of the disorder.Somatic mutations that happen in a single cell early in embryonic development canlead to a situation called mosaicism. These genetic changes are not present in aparent's egg or sperm cells, or in the fertilized egg, but happen a bit later when theembryo includes several cells. As all the cells divide during growth and development, cells that arise from the cell with the altered gene will have the mutation, while othercells will not. Depending on the mutation and how many cells are affected, mosaicismmay or may not cause health problems.

A family medical history is a record of health information about a person and his or her close relatives. A complete record includes information from three generations of relatives, including children, brothers and sisters, parents, aunts and uncles, nieces and nephews, grandparents, and cousins. Families have many factors in common, including their genes, environment, and lifestyle. Together, these factors can give clues to medical conditions that may run in a family. By noticing patterns of disorders among relatives, healthcare professionals can determine whether an individual, other family members, or future generations may be at an increased risk of developing a particular condition.
